[A morphological evaluation of the action of collagenase from the king crab Paralithodes camtschatica on the experimental wound process].

The paper presents the results of experimental morphological evaluation of the effect of a new proteolytic enzyme collagenase of crab Paralithodes camtschatica on wound healing in infected and aseptic rabbit wounds. The enzyme was applied on wounds using gauze and gelevin. The findings show that this protease is highly effective for debridement of infected wounds, the effect increasing at gelevin addition. To reach maximal therapeutical effect and diminish the inhibition effect of collagenase on granulation tissue, it is recommended to reduce the dose of protease during debridement process. Clinical application of crab collagenase must be individual as well as duration of wound enzymotherapy.

[A comparative study of charcoal sorbents for local wound treatment].

The laboratory and medico-biological findings made it possible to choose the most suitable carbonaceous sorbents for local management of wounds, basing on the fact that every carbonaceous material has strictly selected capacity for protein and microbe sorption, and also can be the carrier of the water-miscible drugs. The "Dnieper-MN" carbonaceous fabric and "Aktilen" fibrous sorbent have the best features for sorption. It is possible to use these materials for the construction of bandages to treat wounds and burns which have little exudation. "Karpema" and "AUTM-2" sorbents, which are characterized by the low degree of activation, can be used for the manufacturing of surgical pads. "Karpema" being strong and having a less fiber structure, has a good electrical conductivity and, thus, can be used as medical electrodes.

[Biologically active helevin-based compositions in the treatment of pleural empyema].

The article analyses the method of topical open treatment of postoperative pyothorax in 17 patients with the use of biologically active helevin-based compositions. The control group was made up of 11 patients with a similar suppurative process who received topical treatment by the same method but with the use of ointments with antibiotics and antiseptics on a fat base. With the use of biologically active compositions (BAC) on a helevin-base, the empyema cavity was completely freed from the pyonecrotic masses and its walls filled with granulation tissue on day 7 of treatment. In the control group the cavity was completely cleansed only on day 19 of treatment. It is shown that BAC possesses a high antibacterial effect not only due to the antibacterial components but because it is based on the laws of physical antiseptics.

[Principles of surgical treatment of inguinal hernia].

The work is based on the analysis of the results of surgical treatment of inguinal hernias in 198 patients in the period from 1983 to 1988. Most of the patients were over 60 years of age, a lesser number were under 35 (11.1%). Hernias of a complicated and transitional form were found in 174 patients and mild forms only in 24 patients. The hernias were recurrent in 45 and sliding in 59 patients. Proceeding from the specific features of the surgical anatomy of the inguinal space, all patients, irrespective of their age and the form and type of the hernia, were treated by operation with plastics of the opening of the hernial sac to the back of the elements of the spermatic cord. The only exception were patients under 35 years of age with a mild form of oblique inguinal hernia, in which case variants of operations with enforcement of the anterior wall of the inguinal canal were carried out. The Schouldies and McVay methods were preferred, in recurrent and all complicated forms of inguinal hernias they were combined with operations of the Endrews and Postempsky types; the last named was mostly used in individuals over 60 years of age and in repeatedly recurring hernias. The late-term results were studied in follow-up periods of 2 to 7 years in 176 patients in the group of 198 individuals who were operated on. In 77.7% of them 3-6 years have passed since the operation. Almost all patients of mature age have resumed their occupation, there are complaints neither of pain at the site of the operation nor of changes in sexual potency. Recurrent hernias were not detected.

[Principles of surgical treatment of postoperative ventral hernia].

The article deals with the results of surgical treatment of postoperative ventral hernias in 398 patients who underwent operation in 1982-1990; 68.5% of patients had large and giant hernias. In 198 patients (48.4%) the hernias were recurrent. One to 9 concomitant diseases were encountered in 88% of cases, which complicated the course of the postoperative period significantly. Reconstructive operations with the use of auto-allo-xenografts and netted synthetic materials were undertaken when the defect measured 15-20 cm in diameter, as well as in cases of multiple, though lesser in diameter, defects in patients with diseases of the cardiovascular system and lungs in the stage of sub- and decompensation, in hernias of long duration, in large irreducible hernias, and in patients over 60 years of age. A combination of grafts with netted synthetic materials was used when a defect measuring 21 cm or more in diameter was encountered together with the above situations. The late-term results were followed-up in periods of 12 months to 8 years. In 254 cases (69%) auto-allo-xenografts and synthetic materials and, in difficult situations, their combinations were used, with good results in 96% of patients. After plastics in 254 patients with the use of additional materials 2 recurrences (0.7%) were revealed; 3 recurrences (2.6%) were encountered among 113 cases in which the defect was closed with local tissues by means of sutures.

[Surgical treatment of postoperative infected hernias].

The article deals with the results of examination and treatment of 153 patients with postoperative ventral hernias. In 109 (71.3%) of these patients the hernia formed due to suppuration of the wound after the first operation; 48 (31.4%) hab on admission a source of purulent surgical infection in the zone of the future operative intervention. According to the findings of microbiological examination of intraoperative biopsies and smears taken from wounds and fistulas, the patients were divided into three groups (group 1, 10(1) microbial bodies per kg tissue; group 2, 10(2); group 3, 10(5)). Pharmacological studies showed that the necessary concentration of broad-spectrum antibiotics is produced only when they are injected by the infiltrative method. On basis of this, the authors developed a scheme for the prevention of purulent complications in herniotomy with the use of biological and synthetic prostheses for each group of patients. The method produced good results in the majority of patients with minimal suppurations (3.9%) and recurrent hernias (0.65%).

[The system of designation of surgical suture materials].

The authors suggest a unified system of designation of type--sizes of surgical suture materials of various nature and structure, which removes the existing discrepancy between the numbering of foreign and Soviet threads as well as of Soviet threads produced by different enterprises of the country. Introduction of the new system of metric sizes of the suture threads will allow surgeons to be properly orientated in the choice of the necessary suture material and make easier the work of services engaged in the development and realization of surgical suture materials.

[Quantitative evaluation of humoral and cellular factors of antibacterial protection in patients with postoperative hernia complicated by suppurative process].

The article presents material on the study of the dynamics of humoral factors of the organism's antibacterial protection in correlation with factors of cellular immunity in 78 patients with postoperative ventral hernias complicated by a pyoinflammatory process. Factors of cellular immunity were studied in 33 patients, humoral factors of the organisms' antibacterial protection, in 45 patients. A distinct relation of the character of disorders of these factors to the type of the operation was revealed and their prognostic significance for clinical medicine was determined.

[Clinico-morphological changes in a burn wound after electric stimulation with pulsatile current].

The healing of sluggishly postburn wounds after local electrostimulation (ES) with the use of current-conducting carbon tissue as the electrode was studied in 12 patients. Analysis of the clinical data which were corroborated by the results of morphological and biochemical studies confirmed the marked stimulating effect of local ES which facilitates successful taking of dermal autografts and healing of burn wounds. The obtained data are evidence that the method of ES through a current-conducting carbon tissue is promising in the complex of measures for local treatment of burn wounds.

[Prosthesis of the testis in monorchism and anorchism].

The authors describe the method for prosthetics of the testes in mon- and anorchism, by which the operations were carried out on 12 patients whose ages ranged from 14 to 22 years. Dow Corning (USA) prostheses were used in 3 cases, Soviet--made endoprostheses designed at the Scientific Research Institute of Rubber and Latex Articles, USSR, jointly with the Vishnevsky Institute of Surgery were applied in 9 cases. There were no postoperative complications. The late-term results were studied in follow-up periods of 1 to 4 years and proved to be satisfactory. The method deserves to be introduced into the practice of plastic surgery.

[The use of hydrogel emboli in the treatment of pulmonary hemorrhage in the records of the Tula Regional Hospital].

The results of 59 embolizations of the bronchial and intercostal-bronchial arteries in 33 patients with pulmonary hemorrhage and hemoptysis allow hydrogel emboli to be considered the optimal material for occluding vessels to achieve stable hemostasis. Successive introduction of spherical and then larger cylindrical emboli is the most rational method. Embolization of the bronchial arteries by hydrogel emboli also promotes regression of the inflammatory process in the lung and reduces the time needed for treatment.
