[The surgical procedure in tumor-induced colonic obstruction in patients at an elevated surgical risk].

A comparative assessment of results of palliative and radical operations in 250 patients has proved efficiency of radical operations which were followed by less lethality rate in patients with high operative risk. A conclusion is made that wider indications to radical operations for patients of older age groups even those with different risk factors is one of perspective ways to improve results of surgical treatment of patients with obturative obstruction caused by colonic tumour of any localization.

[Surgical treatment of acute pancreatitis associated with biliary calculi].

Results of surgical treatment of acute pancreatitis combined with concrements of biliferous tract in 197 patients (108 females and 89 men) at the age of 25-78 years are analysed. In all cases there was calculous cholecystitis, that was in 126 (64%) cases combined with choledocholithiasis. In 141 (71,6%) patients there was edematic pancreatitis, in 56 (28,4%)-destructive pancreatitis. 29 patients had fatty pancreonecrosis, 12 patients-hemorrhagic pancreonecrosis, 15 patients-mixed forms of pancreonecrosis. All the patients were operated on. Urgent surgery have been done in 62 (31,5%) patients, planned-in 135 (68,5%). The mortality rate after the urgent surgery was 16 (25,8%), after planned-5 (3,7%). The total postoperative mortality rate is 10,6%. The causes of mortality were: liver and kidney insufficiency (5), heart failure (5) infection (14).

[An experimental and clinical validation for the use of a collagen sponge for treating the suppurative-inflammatory complications of wound healing in emergency abdominal surgery].

In use of a collagen sponge in 253 patients with purulent inflammatory complications after appendectomy, a wound is cleaned 10 days, and epithelialization of its edges occurs 7.6 days earlier as compared to the control group patients. Duration of hospital stay of the patients reduces by 11.4 days.

[Tactics of the surgical treatment of acute intestinal obstruction].

The results of treatment of 184 patients with acute ileus of non-tumour genesis operated on at the clinic within the period of from 1981 to 1990 are presented. In elimination of ileus without the surgical intervention, together with intensive therapy, intestinal decompression was used. Of 19 patients with acute ileus associated with peritonitis operated on, 12 underwent resection of the intestine. The use of enterosorption as well as extracorporeal hemosorption contributed to elimination of endogenous intoxication.

[Diagnosis and surgical treatment of non-neoplastic diseases of the external bile ducts].

In a work, the results of examination and operative treatment of 159 patients with non-tumor diseases of the extrahepatic bile ducts are analysed. After the operation, 55 (32.7%) developed complications, 12 (7.5%) patients died.

[External drainage of the bile ducts via the stump of the cystic duct to prevent postoperative complications in elderly patients].

Operated on were 165 elderly and 68 young patients for calculous cholecystitis complicated by choledocholithiasis and obstructive jaundice. Drainage through the cystic duct stump is the most effective method for external drainage of the common bile duct in operative interventions on the bile ducts in elderly patients, which permits to reduce considerably the incidence of postoperative complications.
