[Real embologenicity of thromboses of lower extremity veins].

The aim of the present work was to study real embologenicity of thromboses of lower extremity veins. We carried out a comparative evaluation of the embologenic risk of different thromboses of the superficial and deep veins associated with hospital treatment. The study performed allows the following conclusions to be drawn: venous thromboses in the system of the inferior vena cava are marked by an unpredictable course and require the follow up of the thrombotic process and active conservative treatment within the period not less than 7 days from the time of the first manifestations of thrombosis; thrombosis of the femoral vein as well as thrombosis of the sural veins have the highest embologenic risk (6.45 and 6.25%, respectively). Patients with ascending thrombophlebitis of the greater saphenous vein remain at high risk for embologenic process and require urgent operation.

[Antibiotic production by variants of Penicillium chrysogenum isolated after protoplast formation and exposure to mutagens at the protoplast stage].

A procedure for protoplast formation in the penicillin-producing organism Penicillium chrysogenum was developed. The yield of the protoplasts was high, the protoplasts were stable and capable of regeneration. Two types of the protoplast regeneration were revealed. The spores and protoplasts were treated with UV light and N-nitroso-N'-methyl biuret and their effect on production of the antibiotic by the isolated variants was studied. It was shown that the protoplasts of P. chrysogenum were more liable to the mutagenic effect of UV light and nitroso methyl biuret than the fungus conidia. It is possible to use this specific feature in intensification of selection aimed at isolation of highly productive strains of P. chrysogenum.

[Utilization of automatic systems for determining the biological activity of antibiotics by the agar diffusion method].

The Automatic Zone Analyzer AR-140 of Millipor intertech., inc. is an apparatus used for measurement of inhibition growth zones of test-microbes and estimation of antibiotic activity. The analyzer provides automatic registration of the data with high levels of accuracy and at least 99% reproducibility. The analyzer gives an effect in saving time as compared to the manual method by 5-7 times for the single-dose procedure and by 10-12 times for the three-dose procedure.

[Bacteria-infectious viruses in Penicillium chrysogenum strains with differing antibiotic activity].

A new virus capable of replication on E. coli C was isolated from Penicillium chrysogenum. Six strains of P. chrysogenum differing in the cultural-morphological and biochemical properties and the titer of virus PBV5 were studied. The data provided a supposition that there were definite connections between the titer of virus PBV5 in the mycelium and the fungus ability for the antibiotic production.

[PBV5 virus induction in Penicillium chrysogenum mycelium].

The effect of some physical (increased temperature, UV light) and chemical (N-nitrosomethylbiuret, proflavin, acridine orange) factors on the virus titer in 2 strains of Penicillium chrysogenum was studied. Thermoinactivation of the virus and its induction under the effect of UV light and proflavin were shown. Slight induction was observed with N-nitrosomethylbiuret and acridin orange. It was supposed that induction of the virus in the mycelium of Penicillium chrysogenum was spontaneous.

[Antibodies against synthetic fragments of the prion protein for the diagnosis of bovine spongiform encephalopathy].

Seven peptides matching fragments of the prion protein and containing from 17 to 31 amino acid residues were synthesized to obtain antibodies for diagnostics of bovine spongiform encephalopathy. Rabbits were immunized with either free peptides or peptide-protein conjugates to result in sera with a high level of antipeptide antibodies. Immunohistochemical assay revealed sera against four free peptides and a protein-peptide conjugate, which effectively bind to the pathogenic isoform of the prion protein in brain tissue preparations from cattle afflicted with bovine spongiform encephalopathy and do not interact with normal brain preparations. The resulting antipeptide sera can be used in developing a diagnostic kit for bovine spongiform encephalopathy.

[Comparative evaluation of the efficacy of using terazosin and tamsulosin in patients with benign prostatic hyperplasia].

AIM: To evaluate effectiveness and safety of terazosin vs tamsulosin in patients with benign prostatic hyperplasia (BPH); to elucidate effectiveness of the use of tamsulosin followed by terazosin and vice versa. MATERIAL AND METHODS: Group 1 of BPH patients received terazosin (5 mg/day) followed in a month by tamsulosin (0.4 mg/day). Group 2 received the same drug in the opposite sequence. The trial included 60 patients with symptoms of the lower urinary tracts due to BPH. The results were assessed on the treatment day 5-7, 14, 30 and 60 one month after the treatment termination. RESULTS: Terazosin and tamsulosin relieved clinical symptoms, reduced QOL and Qmax. One month after the treatment the symptoms partially returned to baseline, Qmax returned completely. In cross-over, positive trends were weaker. CONCLUSION: Clinical efficiency of both drugs was comparable, there was no cross effect. A month interval between the treatments in this case was insufficient. This should be taken into account when planning further studies.

[Urodynamic criteria of predicting efficacy of therapy with alpha1-adrenergic blockaders in patients with benign prostatic hyperplasia].

AIM: As baseline urodynamic parameters do not correlate well with response to medical treatment for lower urinary tract symptoms (LUTS), this study was designed to asertain whether urodynamic parameters correlate better with response to alpha 1-adrenoblockers. METHODS: 23 men (mean age 64.6 +/- 9.3 years) with LUTS have undergone urodynamic examination prior to initiating therapy with terazosin titrated to 5 mg and followed for 2.5 months. Parameters of evaluation included IPSS symptom score, peak flow rate (Qmax), maximal detrusor pressure (Pmax), detrusor pressure at maximal flow (Pdet) and detrusor contraction duration (DCD). The correlation and predictive value of therapeutic response and baseline urodynamic parameters were assessed. RESULTS: For the entire group, IPSS decreased from 13.25 +/- 3.39 to 7.38 +/- 3.21 (44.6%), Qmax increased from 9.75 +/- 1.95 to 11.00 +/- 3.23 ml/s (21%), Pmax decreased from 109.75 +/- 35.72 to 93.88 +/- 20.91 cm. H2O (14.4%), Pdet decreased from 84.00 +/- 23.71 to 74.00 +/- 15.53 cm. H2O (16.8%), DCD increased from 127.63 +/- 53.99 to 130.13 +/- 45.41s (2.7%). There was a weak correlation between either Pmax or Pdet and therapeutic response (defined as a IPSS decrease of 40% and an increase in Qmax of 30%). There was a significant correlation with therapeutic response of DCD > 60 s (p < 0.05, r = 0.56). CONCLUSION: The pilot data suggest that DCD may be a useful urodynamic parameter to predict and optimize therapy with alpha 1-adrenoblockers.

[Results of monotherapy with terazosin (kornam) in chronic infectious prostatitis patients].

AIM: To evaluate effectiveness of alpha 1-adrenoblockers (a1-AB) in patients with chronic non-infectious prostatitis (CP). MATERIAL AND METHODS: a1-AB effectiveness was studied in 28 volunteers with non-infectious CP (type IIIA). All the patients received terazosin (kornam, Slovenia) in a dose 5 mg/day after dose titration for 2-3 weeks (the initial dose was 1 mg/day). The assessment of the efficiency was standard with the use of frequency and severity of symptoms scales as well as questionnaire NIH CPSI. RESULTS: Symptomatic improvement was achieved in 27 patients (96%). Dysuria and intensity of pain diminished in 24 (82%) and 26 (93%) patients. Voiding disorders proved most sensitive to a1-AB. Quality of life rose 2-fold. Probability of the recurrence 1 month after terazosin therapy was 0.29, after 6 months--0.43. Recurrent dysuria occurred in 33%, pain--in 58%. CONCLUSION: Terazosin monotherapy in patients with non-infectious CP results in reduction of the symptoms severity and pollakiuria frequency, frequency of recurrences and in improvement of quality of life. a1-AB in CP do not prevent recurrence in half the cases but the symptoms severity in recurrent disease is much weaker.

[Use of alpha1-adrenergic blockaders in voiding disorders in women].

UNLABELLED: Alpha-adrenoblockers are commonly used to treat LUTS suggestive of benign prostatic hyperplasia in men. It is well known that they do not modify natural history of the disease but only relax the smooth muscle. Several investigations confirmed the presence of alpha 1-AR in the spinal cord, bladder neck, urethra and periurethral tissue in women (so-called female prostate). So, "prostatism-like" symptoms seem to be not gender-specific. Therefore, alpha-adrenoblockers may be used in the treatment of urinary disorders in women. 32 women (mean age 26.5 years, range 18-65) with functional BOO took a terazosin course after clinical and urodynamic examinations. The patients were divided into three groups: detrusor sphincter dyssinergia, primary bladder outlet obstruction and learned voiding dysfunction (dysfunctional voiding). Patients with urethral stricture, pelvic organ prolapse, bacteriuria were excluded. The initial dose was 1 mg/day, then it was increased to 5 mg/day. The treatment course lasted 2 months. Patients recorded the time and volume of every void three days before treatment and last three days of drug therapy. All the patients were satisfied with the results of the treatment. We noticed an increase in the maximal flow rate (55.4%), a decrease of delay time (48.9%) and nocturia (52.9%). CONCLUSIONS: Specific agents, such as alpha-adrenoblockers, in low dosage may be effective in women with urinary tract disorders in case of careful and comprehensive evaluation of functional BOO.

[Induction of immune response by synthetic fragments of the bovine prion protein and their analogues in mice of various lines].

The antibodies to the bovine prion protein were produced by immunizing mice of three lines with five synthetic fragments of the protein and their six analogues. The analogues contained the amino acid substitutions that, according to theoretical calculation, should lead to an increase in the immunogenic activity of peptides. All the peptides, except for one, induced the formation of antibodies. All the sera containing the antipeptide antibodies were tested by an immunohistochemical method. The sera that were effectively bound to the brain preparations from the bovine with spongiform encephalopathy were identified; it was shown that they do not interact with the preparations of normal brain. Therefore, it was shown that the immunization of mice with the synthetic fragments of a prion protein helps obtain specific antibodies suitable for the study and diagnostics of prion diseases.

[Double-blind placebo-controlled trial of terazosine efficacy in patients with chronic abacterial prostatitis].

A double-blind placebo-controlled trial of teraosin efficacy (Kornam, Lek, Slovenia) was made in 51 patients with chronic abacterial prostatitis/chronic pelvic pain syndrome (CAP/CPPS) of category IIla according to NIH. All the patients were given a 2-week induction course of placebo followed by teraosin treatment (5 mg/day, n = 29) or placebo (n = 22) for 8 weeks. The participants of the study were followed up for 12 months. Pretreatment differences between the groups by NIH-CPSI system, symptoms frequency scale, leukocyte count of the prostate and uroflowmetry were insignificant. Teraosin and placebo patients showed a noticeable improvement (39.7 and 9.9% by symptoms frequency scale, respectively; by 36.4 and 6.6% by the linear scale, respectively). The drug reduced pain and dysuria, improved quality of life considerably (by 36.2%). Maximal urine flow accelerated by 22.96 and 10.01% in teraosin and placebo groups, respectively. Leukocyte count fell two-fold in the study group and lowered insignificantly in the placebo group. The recurrence-free interval was 25 and 9 weeks, respectively. Thus, teraosin monotherapy improves quality of life in CAP/CPPS patients, significantly relieves symptoms and prolongs recurrence-free interval vs placebo.

[Validity of using physical therapy in combined treatment of chronic prostatitis].

To evaluate efficacy of combined physiotherapy in patients with chronic prostatitis (chronic bacterial, chronic abacterial prostatitis), an open comparative trial was made by specialists of the Research Institute of Urology in 2003-2004 of the unit Andro-Gin. Before the treatment, a standard examination was made including analysis of case history and complaints, rectal palpation, questionnaire filling-in, prostatic secretion tests, PCR diagnosis, transrectal ultrasonic scanning and uroflowmetry. In group 1 (chronic bacterial prostatitis) given monotherapy with an etiotropic drug (ED) or combination of ED with Andro-Gin treatment, a significant improvement was achieved by the scale NIH-CPSI, Sorensen scale (p < 0.05). In group 2 (chronic abacterial prostatitis with inflammation) subgroups C,D,E patients showed significant improvement by the scales NIH-CPSI and Sorensen (p < 0.05). The highest symptomatic effect was recorded in the subgroup D in combined treatment with ED and Andro-Gin physiotherapy. In group 3 low NIH-CPSI scale score occurred due to alleviation of pain in subgroup F (p < 0.05), In subgroup G symptoms by the above scales did not change. Uroflowmetry featured moderate dynamics of the increment in maximal voiding speed. Voiding improved significantly in subgroup F in patients with chronic abacterial prostatitis in the absence of inflammation.

[Quality of life in patients after radical prostatectomy].

According to the literature, after radical prostatectomy the patients had normal micturition and erectile function in 50 and 10-30%, respectively. A total of 647 patients with diagnosis prostatic cancer (PC) stage T1a-T4 were examined in two clinics in 1997-2003. Of them, 43 patients (PC stage T1a-T3a) have undergone retropubic radical prostatectomy (RRP) by a modified nerve-preserving technique (preservation of the urinary bladder neck, puboprostatic joints in prostate size > 60 cm). Preoperative and postoperative examinations for urine continence and erectile function after RRP were made in 20 patients. The proposed operative technique of nerve-preserving RRP preserves urine continence and erectile function in many cases. This improves social adaptation of the patients and extends indications for operative treatment.

[The hemagglutinating properties and adsorption on erythrocytes of Coxsackie B-3 viruses and their selected bentonite variants].

The paper is devoted to the study of the Coxsackie B-3 viruses and their bentonite variants passaged on the primary and transplanted cell cultures for their hemagglutinating (HA) properties and a degree of adsorption on erythrocytes. It is shown possible to detect the HA activity of the Coxsackie B-3 viruses which are passaged on the transplanted cell cultures: the variant Abent has the hemagglutinating activity relative to human and rabbit erythrocytes. The variant Abent+ of the Coxsackie B-3 virus passaged on the cell culture Vero is established not to be absorbed on erythrocytes. The variant Abent and the initial population of the Coxsackie B-3 virus are adsorbed on the human erythrocytes by 90-99%. Coxsackie B-3 virus and its bentonite variants reproduced on the primary cell culture of human embryonic fibroblasts and possessing the hemagglutinating activity are 100-1000 times better adsorbed on human erythrocytes than the viruses passaged in the transplanted culture.
