Left ventricular systolic and diastolic functional abnormalities in asymptomatic patients with non-insulin-dependent diabetes mellitus.

The aim of this study was to evaluate the relations between left ventricular (LV) functional abnormalities, microangiopathy, and autonomic dysfunction in patients with non-insulin-dependent diabetes mellitus (NIDDM). We studied 66 normotensive patients with NIDDM of > or =4 years' duration (age, 51 +/- 4.5 years; 35 men) and no clinical evidence of cardiac disease. Twenty-one healthy subjects matched for age and sex served as a control group. Echocardiography and Doppler studies were performed to assess LV systolic and diastolic function. Microangiopathy was assessed by fundus examination. Autonomic function was assessed by standing blood pressure and heart rate response to Valsalva maneuver. Patients with NIDDM had a lower ejection fraction (58% +/- 11% versus 66% +/- 4%, P <.0001), E-F deceleration slope (382 +/- 75 versus 427 +/- 31 cm/s(2), P <.05), and E velocity (55 +/- 11 vs. 58 +/- 6 cm/s, P =.02) of the mitral diastolic flow, compared with control subjects, respectively. Patients with ejection fraction <50% had a higher prevalence of retinopathy (65% versus 29%, P <.005), abnormal blood pressure response to standing (53% versus 8%, P <.0005), and proteinuria (65% versus 27%, P =.006). An inverse correlation was found between the duration of diabetes and both the ejection fraction (r = -0.53, P <.05) and E/A ratio (r = -0.4, P <.005). E/A ratio <1 was associated with a higher prevalence of retinopathy (49% versus 20%, P =.01) and abnormal blood pressure response to standing (29% versus 4%, P <.005). Patients with NIDDM and no symptoms of cardiovascular disease have a reduced LV systolic and diastolic function as compared with healthy subjects. LV systolic and diastolic abnormalities are correlated with the duration of diabetes and with other diabetic microangiopathies such as diabetic retinopathy and neuropathy.

Characteristics of colorectal cancer that produce positive immunochemical occult blood test results on stool obtained by digital rectal examination.

OBJECTIVE: To clarify the clinicopathological features of colorectal cancer that show positive results on an immunochemical fecal occult blood test of stool obtained by digital rectal examination. METHODS: In a cross-sectional study, 9952 subjects received both an immunochemical fecal occult blood test of stool obtained by digital rectal examination and colonoscopy annually over a nine-year period of medical checkups; 64 patients with colorectal cancer were identified. The study subjects comprised 39 patients with colorectal cancer who had positive results (positive group) and 25 patients with colorectal cancer who had negative results (negative group) on an immunochemical fecal occult blood test of stool obtained by digital rectal examination. The positive and negative groups were compared in terms of their individual factors, such as site, size, Dukes classification and histological type of the cancer lesions. RESULTS: The prevalence of rectal cancers was higher in the positive group than in the negative group (P<0.05), but there were no differences between the two groups with respect to any other factors. CONCLUSIONS: These findings indicate that stool obtained during the digital rectal examination is unsuitable for detecting fecal occult blood, especially for the detection of proximal colon neoplasms.

[Antibiotic-resistant cystitis in the course of urinary bladder calculi after vesicoureteral reflux operation many years ago].

This is the introduction of chronic cystitis in course of urinary bladder calculi, 12 years after vesicoureteral reflux operation. The nucleus of the calculus (3 x 3 x 4 cm) were unresorbed dexon sutures left after the surgical treatment. Inaccuracy of radiological and ultrasonography examination delayed the settlement of the diagnosis about 18 months, and patient was caused of many months antibacterial therapy. After removal the calculus from the urinary bladder, dysuric disorders abated quickly and than after 8 weeks antibacterial therapy pathological changes in urine analysis disappeared, urine culture was sterile.

An immunologic study of psoriasis.

Thirty-five patients with psoriasis and 18 normal controls were studied in order to assess the role of humoral immunity in psoriasis. Direct immunofluorescent study of skin biopsy specimens taken from the lesions revealed deposits of immunoglobulins (IgA, IgG, IgM) and complement components (Clq, C3), mainly in the stratum corneum. Deposits were also found in Munro's microabscesses, spongiform pustules of Kogoj, upper layers of the stratum malpighii, dermal blood vessels, and cells of the dermal inflammatory infiltrate. Uninvolved skin of the patients and normal controls revealed negative fluorescent results. There was a significant increase in the mean levels of IgA and IgG in patients with psoriasis, whereas IgM was within the normal range. The present study showed humoral abnormalities in psoriasis that may be involved in its pathogenesis.

Studies on infertility in males.

One hundred and fifteen infertile men were examined for circulating spermagglutinating antibodies by the Kibrick spermagglutination test; thirty-three (28%) were found to have positive agglutination titers--1:32 or more in thirteen samples. This high figure may be explained by the high incidence of genital tract infection and of urinary schistosomiasis in our study group. Of the 33 men who demonstrated autoantibodies in their sera, 21 had microscopic agglutination of more than 10%. There was a positive correlation between the serum autoantibody titer and spermagglutination. Eight cases (6.9%) of sperm-immobilizing antibodies were found.

Immunologic studies of male infertility.

Blood and semen samples were obtained from 13 men who had self-agglutination of spermatozoa and spermatozoal antibodies in their sera. The seminal plasma of these men produced spermatozoal immobilization and agglutination when added to normal semen from healthy men. Analysis of the seminal plasma samples showed that immunoglobulin G (IgG) levels were higher in the study group than in the control subjects. They contained IgA and complements C3 and C4, whereas only 2 of 15 samples from normal men contained IgA, and only one case contained C3 and C4. IgM was detected in 10 samples of the study group and in one sample of the control group. IgG levels in the blood of the study group were significantly higher than those in the blood of control subjects. Therefore, the immunoglobulins which probably play a role in the mechanism of autoagglutination and immobilization of spermatozoa are IgG, IgA, and IgM, and complements C3 and C4.

A comparative clinical evaluation of a new topical steroid 'halcinonide' and hydrocortisone in steroid-responsive dermatoses.

Fifty patients with symmetrical, bilateral lesions of psoriasis, eczematous dermatitis, atopic dermatitis, or neurodermatitis participated in a double-blind paired comparison study in which 0-1% halcinonide (in a cream formulation containing also neomycin and nystatin) was applied to the lesions on one side of the body and 1% hydrocortisone cream to those on the opposite side for two to three weeks. The number of excellent responses to therapy showed the halcinonide combination to be significantly superior (p less than 0-01) to the control cream in all diagnostic categories if considered collectively, and in psoriasis if the responses were grouped according to diagnosis. No adverse reactions occurred during the trial.

Prognostic value of adenosine single-photon emission computed tomographic thallium imaging in medically treated patients with angiographic evidence of coronary artery disease.

BACKGROUND: This study examined the prognostic value of adenosine single-photon emission computed tomographic thallium imaging in medically treated patients with angiographic evidence of coronary artery disease (CAD). METHODS AND RESULTS: Patients who underwent coronary revascularization within 3 months of this study were excluded. There were 177 patients aged 64 +/- 11 years; 74 had one-vessel, 57 had two-vessel, and 46 had three-vessel CAD (> or = 50% diameter stenosis). During a mean follow-up of 22 +/- 13 months, there were 14 events (cardiac death or nonfatal myocardial infarction). Cox survival analysis with important clinical, catheterization, and scintigraphic variables identified the size of perfusion abnormality as the strongest predictor of events (chi 2 = 9). Life-table analysis showed that patients with perfusion defects of 15% or greater of the myocardium had a worse prognosis than had patients with no or smaller defects (Mantel-Cox statistic = 13; p < 0.001). CONCLUSIONS: Thus adenosine single-photo emission computed tomographic thallium imaging provides important prognostic data in medically treated patients with CAD. The extent of thallium abnormality is the most important predictor of events.

Independent and incremental prognostic value of exercise thallium single-photon emission computed tomographic imaging in women.

BACKGROUND: This study examined the independent and incremental prognostic value of exercise thallium single-photon emission computed tomographic imaging in 212 women who also underwent coronary angiography. METHODS AND RESULTS: The left ventricular ejection fraction was normal (65% +/- 15%). During a mean follow-up of 40 months, 27 women had events (cardiac death or nonfatal myocardial infarction). Univariate Cox survival analysis showed several variables to be different between patients with events and those without events: age, exercise heart rate, the extent of coronary artery disease, reversible thallium defects, number of segments with reversible abnormality, and size of perfusion abnormality. Multivariate survival analysis showed that a large perfusion abnormality and age were the independent predictors of events. Actuarial life-table analysis showed that women with a large thallium abnormality (> or = 15% of the myocardium) had significantly worse event-free survival rates than had women with no or small abnormalities (Mantel-Cox statistic = 16; p = 0.0001). CONCLUSIONS: Thus exercise thallium-201 single-photon emission computed tomographic imaging provides independent and incremental prognostic information to clinical, exercise, and coronary angiographic results in women. The presence of a larger thallium abnormality identifies women at high risk of cardiac events.
