[Non-traditional procedure of revascularization of the lower extremities with bone transplants in unreconstructable arterial occlusions].

An unusual method for salvation of lower extremities with critical ischemia is described. A free bone fragment is formed by osteotomy and distracted by an osteotomy device during a period of 31 to 36 days with subsequent fixation (during a total of 45 to 60 days). This treatment is usually combined with lumbar sympathectomy. This procedure intends a stimulation of the peripheral circulation. It was used in 92 patients with non-reconstructable distal arterial occlusions in stage III and IV. In spite of some complications and failures the extremity could be saved in 77% of the cases. Controls over 2 to 36 months in 67 patients revealed a favourable result lasting over one year in 43 of the cases.

[Simultaneous intrathoracic restoration of the coronary and brachiocephalic blood flow].

The treatment of 3 patients with combined occlusive affection of the brachiocephalic and coronary arteries is discussed. A one-stage intrathoracic operation was carried out. Prosthetics of one or two brachiocephalic branches was conducted at first after which a double of triple aortocoronary shunt was formed under conditions of extracorporeal circulation with the use of cardioplegia. The outcome of treatment was favourable. The formation of an aortobifemoral shunt will be undertaken in one of the patients in a short time. Various problems of tactical and surgical character in one-stage correction of the coronary and brachiocephalic blood flow are discussed.

Arteriosclerosis↗

[Freezing of the extremity with acute ischemic gangrene before amputation].

This is a report on 153 patients (from 22 to 86 years of age) with acute ischemic gangrene of the lower limbs and severe concomitant diseases (ischemic heart disease, arterial hypertension, diabetes mellitus, etc.). Freezing of the necrotic limb with application of a tourniquet and general medical treatment were performed previous to amputation. Freezing was accomplished by dry ice and common ice in 16 patients and by a special refrigerator in 137 patients. 17 patients in a very grave condition died during refrigeration. Amputation of the lower limb was performed in 136 patients, 110 of them tolerated the operation favourably. Complications of wound-healing were observed in 23.1%.

Acute Disease↗

[Cooling of the extremities in acute ischemic gangrene].

This is a report on 39 patients (from 36 to 84 years of age) with acute ischemic gangrene of the lower limbs and severe concomitant diseases (ischemic heart disease, arterial hypertension, diabetes mellitus, etc.). Cooling of the necrotic limb with the application of a tourniquet and general nonoperative treatment were conducted in preparation for amputation. Cooling was accomplished with dry ice and common ice in 16 patients and with a special refrigerator in 23 patients. Six patients whose condition was very grave died during refrigeration. Amputation of the lower limb was performed in 33 patient, 26 of them tolerated the operation favourably. Wound complications were encountered in 9 patients.

[Causes of unsatisfactory results of the treatment of chronic ischemia of the lower limbs using G. A. Ilizarov's method].

The G. A. Ilizarov's method was used for the treatment of 46 patients with occlusion of the arteries of the lower extremities at stages III and IV of ischemia. The treatment has proved to be ineffective in 10 patients. The use of a method in active aggravation of the inflammatory-necrotic process, postoperative arterial spasm and thrombosis, excess in the permissible rate of the bone split distraction, wrong choice of the level of osteotomy were the main causes of unsatisfactory outcomes.

Arterial Occlusive Diseases↗

[The cleft sternum].

An analysis of 14 patients with cleft sternum is presented. Eleven patients were operated upon. A classification of this defect is proposed. The clinical picture of each type of cleft sternum is given with a description of the operative technique. There were no lethal outcomes. Remote results were good.

[Choosing the method of anesthesia in aortofemoral shunting].

The influence of 4 kinds of anesthesia (neuroleptanalgesia, peridural anesthesia by trimecaine hydrochloride, peridural morphine analgesia, fluoroxene anesthesia) upon the process of reestablishment of blood circulation in lower extremities and possibilities of using a rheographic method for the intraoperative assessment of the efficiency of the aorto-femoral shunting were studied in 112 patients with the help of rheography of the shin and great toes. The authors recognize that trimecaine hydrochloride anesthesia and peridural sympathetic block allow with the help of rheography to timely detect an inadequacy of the aorto-iliac reconstruction in 9 cases.
