[Clinico-experimental aspects of liposomal therapy of bronchial asthma patients with hydrocortisone therapy].

AIM: Clinical and experimental validation of liposomal therapy with hydrocortisone as a perspective treatment of bronchial asthma (BA). MATERIAL AND METHODS: Multistage trials of the liposomal treatment were made on 96 animals and in 137 BA patients. The decision on this treatment was based on the activity of bronchial inflammation and absence of contraindications. Liposomes were introduced endobronchially after a session of jet high-frequency artificial lung ventilation. RESULTS: Clinical endoscopic remission was achieved shortly after the treatment start in most of the patients. External respiration function and gas composition of blood improved, concentration of malonic dialdehyde lowered while catalase and glucose-6-phosphate dehydrogenase activity enhanced, inhalation sympathomimetics requirement reduced. CONCLUSION: Clinicomorphological and laboratory evidence obtained indicate high efficiency of liposomal therapy as an alternative to basic treatment of BA.

[A comparative assessment of the effect of different methods of glucocorticosteroid therapy on the hypophyseal-adrenal system in bronchial asthma patients].

As shown by morphofunctional examinations of hypophyseal-adrenal system in 137 BA patients and post mortem examinations of adrenal cortex in 72 BA cases, most pronounced defects in glucocorticosteroid (GCS) biosynthesis and metabolism were recorded in patients on long-term GCS regiments (5-10 years and more). They displayed low levels of 11-OCS and 17-OCS, enhanced transcortin binding capacity, diminished adrenocortical reserves. Beclometasone propionate, endobronchial liposomes with hydrocortisone had no negative adrenal effects.

[The morphofunctional state of the thyroid in bronchial asthma patients].

Altogether 275 patients with bronchial asthma (BA) were examined for hypothalamo-pituitary-thyroid function by means of clinico-radionuclide and histomorphological studies with measurements in blood of thyrotropin, triiodothyronine, thyroxine, thyroxine-binding globulin, thyroxine efficacy ratio and cholesterol as well as of 131I absorption by the thyroid. Thyroid function was established to undergo biphasic changes. In patients with mild BA, thyroid function was activated in the paroxysmal period. Provided the disease progressed, the thyroid developed hypofunction, which was confirmed by the histomorphological findings. The rifothyronine test enabled one to establish that patients with grave BA given prednisolone and dexamethasone for a long time developed primary and secondary hypothyrosis, respectively.

[Antimicrobial activity of chlorine-substituted 4-alkyl- and arylphenols].

The results of the study on the antimicrobial activity of 12 chlorine-substituted 4-alkyl- and arylphenols against Staphylococcus, Streptococcus, E. coli, Salmonella typhosa, Salmonella typhimurium, Shigella dysenteriae, Proteus, Klebsiella rhinoscleromatis, Candida, Trichophyton gypseum and Microsporum lanosum are presented. It was found that introduction of the alkyl substitute to position 4 of orthochlorphenol increased the activity of the substances against the majority of test microbes. The antimicrobial effect of the substances depended on the length and structure of the alkyl radical. With an increase in the number of the carbon atoms in the alkyl radical from 3 to 7 and in the presence of the cyclic radical C5 - C6 the antimicrobial activity of the substances increased.

[Results of direct revascularization of choroid: an experimental study].

The morphology of histological slices of tissue at the site of direct revascularization of the choroid was assessed in various periods (15 days, 1, 3, and 6 months) after surgery. The surgical method was as follows: a collagen hemostatic sponge, a strip of subconjunctival tissue, and tissue of the external rectal muscle were put onto an exposed 4 x 4 mm site of the choroid and closed with an episcleral flap fixed with nodular sutures without pulling. Experiments were carried out on 13 rabbits (26 eyes). Direct revascularization of the choroid was performed in all animals. The animals were sacrificed by air embolism 15 days, 1, 3, and 6 months after surgery. Vascular filling was carried out. The eyes were enucleated, paraffin slices were prepared from tissues at the site of operation, and the slices were stained with hematoxylin-eosin, tholuidine blue, and after Van Gieson. After direct revascularization of the choroid with episcleral, muscular, and subconjunctival tissue and collagen sponge numerous full-value vascular formations are seen at the site of the intervention as early as 3 months after surgery; these vascular formations connect the extraorbital tissues rich in vessels with the vascular coating; the number of newly formed vessels increases by 6 months postoperation. The results of experiments give good grounds for clinical use of the method.
