Idiopathic midline destructive disease--does it exist?

There is controversy whether idiopathic mid-line destructive disease remains a valid entity, some authors considering that such cases are unrecognized malignant lymphoma or Wegener's disease. A case is presented in which numerous nasal biopsies over several years failed to show evidence of either lymphoma or Wegener's disease. Molecular genetic analysis of archival tissue did not demonstrate a monoclonal lymphoid population. It is proposed that idiopathic mid-line destructive disease remains a valid description in a small number of cases.

A common carotid artery aneurysm causing severe dysphagia.

Extracranial carotid artery aneurysms are rare, but are occasionally recognized as causing dysphagia. These aneurysms generally occur at or above the bifurcation. Here we present what we believe is a unique case: a common carotid artery aneurysm compressing the upper oesophagus, and mimicking the obstruction of a post-cricoid carcinoma.

Cervical osteomyelitis and cord compression complicating pharyngeal myotomy.

Cervical spine osteomyelitis is rare. Haematogenous spread is the usual method by which organisms reach the spine. A review of the English literature reveals very few reported cases of local spread of infection to the spine. Here we present the case of a 68-year-old woman with cervical spine osteomyelitis and cord compression complicating pharyngeal myotomy.

Aged↗

Metastatic adenocarcinoma of the temporal bone.

Metastatic carcinoma involving the temporal bone is extremely rare. A case is reported with an unusual presentation--recurrent episodes of acute mastoiditis. Mastoid exploration and biopsy established the diagnosis. We believe this to be the first reported case to present in this manner.
