Interactions of stress and reproduction - a personal view.

In the first part of this article, the authors discuss the effect of stress upon reproduction. The paper begins with a discussion of the various stressful factors that influence the functions of reproductive organs under various circumstances, including menarche, gestation and lactation. In general, physical and emotional stress has a negative impact upon reproductive function. Chronic stress may delay pubescence and, during the reproductive years, may impair oogenesis and spermatogenesis. This process may result in transitory infertility in women and permanent sterility in men. If associated with pregnancy, stress may be conducive to spontaneous abortion or premature birth. The second part of the article deals with stressful stimuli attributable to the reproductive process, the problem of infertility, techniques of assisted reproduction, and the management of unwanted gestations including induced abortion, fetal reduction, spontaneous miscarriage as well as in utero fetal death. The stress generating effects of singleton and multiple pregnancies and births are also outlined in some detail. In the third part of this article discuss effect of stress upon the fetus directly or through the maternal organism.

Fetal ethology and its relevance to perinatal medicine.

Fetal ethology means fetal activity and behavioral states in the uterine environment. Behavioral states may be defined by various measurable physiological variables, such as heart and respiratory rates, eye movements, body tone and body movements. Fetal motor behavior provides information about the early stages of the neurodevelopmental process. Movement was the first aspect of fetal behavior, the clinical significance of which became was recognized. Fetal behavior provides an opportunity for studying motor, sensory and cognitive functions in the fetus. Behavior can be utilized to study the immediate well-being of the fetus. The latter can be compromised as a result of a number of pathological factors. In high risk pregnancies, the study of fetal behavior may help the clinician to establish the optimum time for the delivery.

Postpartum intracranial haemorrhage in normotensive users of bromocriptine for ablactation.

The authors describe three cases of severe intracranial haemorrhage, associated with marked blood pressure elevation, which occurred between the 6th and 10th days postpartum in women taking bromocriptine for ablactation. All of these patients were young, normotensive and apparently healthy during the pregnancy. These incidents probably present the 10-12th reported cases of gross intracranial bleeding among users of bromocriptine in the puerperium. The results suggest that early diagnosis and prompt surgical intervention may improve the outcome for this rare but potentially catastrophic postpartum complication.

The abnormalities of the presentation in twin pregnancy and perinatal mortality.

The author conducted a retrospective review of the obstetric patient material in his department between January 1, 1975 and December 31, 1992, concerning the clinical implications associated with various lies and presentations of twins. The material included 329 twins (1.07% of all births). Breech, or transverse lies complicated 186 (56.53%) twin gestations. The frequency of premature newborns was among 186 malpresented twin pairs 85 (45.7%) and 101 (54.3%) were mature. The percentage of malpresented twin fetuses increased with maternal age and parity. Abnormal presentations were more frequent (78.49%) among B than A fetuses (54.84%). Perinatal mortality was more frequent among B than A fetuses and more frequent between malpresented twin pairs than the vertex-vertex presented twins.

A study of early fetal growth patterns in twin pairs.

The relationships of sitting heights and body weights of fetal twin pairs were analyzed in comparison with established growth rate standards of singleton fetuses. The apparent rate of growth of individual twins scattered around the average growth curve in the same manner as singletons. In contrast, members of 12 twin pairs with < 125 mm sitting heights, were closely similar in terms of bodily dimensions. Among the 7 pairs with crown-rump lengths of > 125 mm, 4 were discordant, suggesting a deceleration of growth in one of the twin fetuses, starting after the 18th week of gestation.

Sex ratio of spontaneously aborted fetuses and delivered neonates in second trimester.

The author investigated in patient material from 12 years 251 cases involving spontaneous abortion and delivery in the second trimester. The material included 28 sets of twins and one set of triplets, thus the total number of fetuses and neonates was 281. The sex ratio was 136.1, higher than that documented natural sex ratio in the second trimester or at term. The ratio approaches that experienced in cases of perinatal death.

Neonatal sex ratio in pregnancies of adolescent mothers.

The authors have reviewed the sex ratio in 353 offspring of adolescent parturients based on a 5-year material of their department. The 174 males and 179 females provided a sex ratio of 97.2; less than the usual ratio of 106. Interestingly, females are prevalent not only among low-birth-weight but also among average-birth-weight babies of adolescents. The hypothesis is presented that the anatomy of the cervix and the chemical composition of the cervical mucus may affect the sex ratio in some manner.
