[Arthrology: processes of differentiation and integration].

Modern arthrology, the science of disease of and damages to the joints, is a multidisciplinary section of medicine (rheumatology, orthopedics, pediatrics, allergology, oncology, and so forth). About 4% of the population of the earth suffer from different diseases of the joints. It is noted that the prevalence of joint pathology is non-uniform in different countries and continents. In the XX century, orthopedic arthrology, surgical methods of treatment are developing fairly intensively. The following issues have become a matter of growing concern: the effect of ionizing radiation, including low doses, on the articular cartilage, the role played by stress, disruption of the ecological balance in the genesis of arthrological disease, connection between dysplasia of the joints and subsequent dystrophic processes in them (the sequence dysplasia-dysplastic arthrosis), disturbances of the load of the joints as a pathogenetic mechanism of dystrophic and necrotic processes. In arthrology, special attention should be paid to aseptic necrosis of the jointed bones and to protruding coxarthrosis. The topicality of joint endoprosthetics is stressed as is the necessity of the setting up of the arthrological centres where endoprosthetics and restorative surgery might be up to standard. Unfortunately, the manufacture of joint endoprostheses in this country lags behind the requirements. Lately the problem of infantile and juvenile arthritides has become very urgent, posing the problem of the development of arthrological techniques for diagnosis and surgical treatment of the joints. The Academy of Medical Sciences of the Russian Federation should strengthen its coordinating and integrating roles in further development of arthrology.

[Changes in the spinal cord and the characteristics of the regenerative processes in experimental shortening of the spine].

In the experiment with 22 dogs the authors have studied the effect of the spine shortening on the undamaged and damaged spinal cord. Observation period ranged from 1 week to 6 months. Partial resection of the vertebral body without spine shortening, practically, have not affected the spinal cord of an animal. Spine shortening without the spine cord transection is accompanied by the spinal cord crumpling and partial damaging. Combination of this operation with submembrane transection of the spinal cord permits to avoid an obvious scar formation but results in considerable cyst-formation that prevents regenerative process in the spinal cord. Further studies should be conducted in terms of the cyst-formation process prevention.

[Development of the arthroplasty method in coxarthrosis].

On the basis of analysis of surgical treatment of 753 patients with coxarthrosis during 1970-1987, the authors set forth the stages of arthroplasty development in the USSR, using the experience of the Kharkov research institute of orthopaedics and traumatology named after M. I. Sitenko on the problems of arthrodesis operations, endoprosthetics and biological arthroplasty. In the article are presented the peculiar features of diagnostic and surgical tactics in case of patients with idiopathic and dysplastic coxarthrosis; operation technique for each form is presented in details. The authors have developed a new perosseous access to the hip joint, which made a good showing and permits to preserve the integrity of periarticular muscles and neurovascular formations. Application of the given access made possible the extension of indications for arthroplasty in case of grave forms of coxarthrosis, met earlier in a reserved manner.

[Specific features of osteosynthesis using a pivotal compression-distraction apparatus].

The article is devoted to the analysis of 5-year-long application experience of the rod-based apparatus with external fixation. The apparatus have been developed at the institute named after prof. M. I. Sitenko. Certain advantages and indications for rod-fixation in traumatology have been substantiated. In urgent traumatology the apparatus made the best showing in treatment of the open fractures. Possibility of these apparatus application as the treatment-and-transportation immobilization facilities in case of mass entrance of victims has been demonstrated. Rod-based apparatus are widely used for fixation in case of correcting osteotomies, limb stretching (especially of the femoral segment) in children as well as for fixation of the instable spine damages. Complications in case of rod application manifest themselves as suppuration of the soft tissues and osteomyelitis. But their number is lower as compared with the wire apparatus application.
