Left ventricular function immediately after exercise in elite cyclists.

To determine the response to exercise of the left ventricle of endurance-trained athletes, 6 elite (world class) cyclists were compared to 6 untrained healthy control subjects. In athletes the stroke volume increased with exercise. In untrained volunteers the stroke volume did not change with exercise. This difference of the response of the stroke volume to exercise reflected a difference of the left ventricular end-diastolic volume. In athletes the left ventricular end-diastolic volume tended to increase. In control subjects the end-diastolic volume decreased. In conclusion, athletes increased cardiac output by increasing stroke volume and heart rate, whereas control subjects increased their cardiac output only by increasing their heart rate.

[The use of highly dispersed iron particles for increasing the sensitivity of the luminescent microscopic method of determining Mycobacterium tuberculosis].

A highly dispersed ferromagnetic powder obtained by a plasmochemical method (particle size was 100-500 A) was treated by means of an ultrasonic disperser: suspension was added to the trisodium phosphate homogenized and neutralized sputum (0.3 mg of the initial powder per 1 ml of sputum) of patients with various forms of pulmonary tuberculosis. The sputum was then incubated at slight stirring for 40 min and centrifuged; the precipitate was used to prepare smears which were stained with auramine; mycobacteria were detected by luminescence microscopy. The ferromagnetic suspension was found to increase luminescence microscopy sensitivity to 85.4%. The efficacy of the method was 30.8% more than that in the cultivation of infectious material in solid nutrient media.

[Clinico-instrumental characteristics of the state of the myocardium of patients with long Q-T interval syndrome detected during a population survey].

A comprehensive clinical and instrumental study involving the collection of complaints, resting ECG records, bicycler ergometry, Holter monitoring, echocardiography was performed in 80 Novosibirsk's residents (66 males and 14 females) aged 25-64 years who had the prolonged Q-T interval syndrome detected during a population survey. ECG frequently showed the early ventricular repolarization syndrome (23%) in males and left ventricular hypertrophy (36%) in females. Bicycle ergometry increased Q-T interval in 11% of males and decreased or unchanged it in the remaining cases. Holter monitoring revealed cardiac arrhythmias in 39% of males and 64% of females, supraventricular and monotopic ventricular premature beats being prevalent. The method was found to have advantages in detecting arrhythmias. Echocardiography performed in males made it possible to identify ventricular septum hypertrophy (40%) and mitral valve prolapse (20%). The examinees with the prolonged Q-T interval syndrome mostly had arterial hypertension, coronary heart disease, and alcoholic cardiomyopathy.

[Treatment of acute puerperal mastitis].

The results of the treatment of 205 patients with puerperal mastitis were studied: 56 with the serous-infiltrative form underwent the conservative therapy, 149 were operated on. The timely rational operative intervention in patients with suppurative puerperal mastitis in combination with adequate conservative therapy provides a favourable outcome of the disease.

[The clinico-laboratory characteristics of cases of diseases connected with viruses of the California encephalitis complex in the inhabitants of Moscow].

To study the role of viruses of the California encephalitis virus complex (the family Bunyaviridae) in infectious pathology, 187 fever patients admitted to the Clinical Infectious Hospital in May-September 1986 were examined. In 10 of these patients the neutralization test revealed the presence of diagnostically significant changes in neutralizing antibodies (neutralization indices), which was indicative of the role played by Tahyna virus or other related viruses belonging to the California encephalitis virus complex in the etiology of the diseases. The analysis of the clinical picture showed that in all patients the disease took an acute course in its initial stage, starting with shivering and characterized by high fever, headache, pronounced toxicosis, the possibility of the formation of intracerebral hypertension and pneumonia.

[Significance of viruses of antigen complex of California encephalitis in pathology].

A study was undertaken to examine 320 patients with seasonal fevers occurred in June to September. These included fever of unknown etiology, acute respiratory virus diseases, pneumonias, bronchitis, enterovirus diseases, and serous meningitis. Serological tests revealed that in 20 (6.3%) of them the viruses of the complex in California encephalitis (Tyagin's virus or antigenically related virus) contributed to the etiology of the disease. Major clinical symptoms of the disease were defined in this group of patients. The disease appeared as neuroinfections (serous meningitis), influenza-like conditions (fever, symptoms of intoxication), occasionally, infiltrative changes in the lung. Thus, the viruses of the antigen complex of California encephalitis makes an etiological contribution to infectious abnormalities.

[Cancer of the jejunum and Fraumeni syndrome].

An observation of multiple primary tumors of various histogenesis and a rare combination (uterine body carcinoma, mammary fibroadenoma, bronchial carcinoid, jejunum carcinoma) developing in a female patient at various periods of her life is described. The autosome-dominant type of the transfer proves the possibility of a genetic predisposition of the tumor development (the patient's mother and brother also had tumors). This observation demonstrates the variety of tumor combinations and tumor topography in cases of genetically predetermined blastomogenic process. This may represent a basis for distinguishing separate variants of Fraumeni syndrome.

[Detection of long Q-T interval in mass screening of the population].

The incidence of lengthened Q-T interval in an open population of 45- to 54-year-old males was assessed in a random representative sample. Lengthened electrocardiographic Q-T interval was detected in 3.5% of cases. The identification of individuals with lengthened Q-T intervals at population screenings, their follow-up and the development of preventive measures against ventricular tachyarrhythmias and sudden death are shown to be warranted.

[Extraction of acid alpha-glucosidase from human spleen].

An efficient method for isolation of acid alpha-glucosidase from human spleen is developed. The method involves chromatography of the enzyme on rho-aminophenyl-alpha-D-glucopyranoside covalently bound to CH-Sepharose 4B, with subsequent gel filtration on Sephadex G-200. The enzyme was homogeneous by the polyacrylamide gel electrophoresis data; it was purified about 1500-fold, as compared with the crude extract (the total yield 12.5%). Besides acid alpha-glucosidase, the preparations of alpha-L-fucosidase, alpha-D-galactosidase and beta-N-acetylglucosaminidase were isolated and purified 200-, 130- and 280-fold, respectively. The nature of interaction between acid alpha-glucosidase and immobilized rho-aminophenyl-alpha-glucopyranoside is discussed.

[Sutureless anastomoses in gastrointestinal surgery with and without steady magnetic field (experimental study)].

Experimentally in 52 dogs, sutureless gastro-duodenal and ceco-jejunal side-to-side anastomoses were done by means of devices proposed by N.N. Kanshin for connecting void organs, and the features of accretion of the organ walls under the effect of both simple mechanical compression and that with the help of a steady magnetic field were studied. In both variants the process of anastomosis formation completes at 6-7 days after the operation and natural elimination of the compressing elements occurs at 7-8 days. Morphological examinations of sutureless anastomoses formed under a magnetic field or without it revealed no differences between them. The experimental results demonstrated a number of significant advantages of the sutureless methods of tissue connection over the traditional manual intestinal suture.
