Exercise-induced changes in the activity of serum lactate dehydrogenase (LDG) and its isoenzymes, CK, aspartate and alanine aminotransferase, were examined in postmyocardial-infarction coronary patients with second-type diabetes mellitus and 18 chronic coronary patients with second-type diabetes mellitus. Patients from both groups showed increased total LDG and LDG-5 activity at rest and reduced total LDG, LDG-1, LDG-2 and LDG-3 activity in response to exercise, which may be an evidence of prevailing anaerobic glycolysis as a manifestation of tissue hypoxia. Rationed bicycle ergometric exercise produces no rise in blood CK, aspartate and alanine aminotransferase activities in coronary patients with second-type diabetes mellitus, suggesting that exercise of this kind has no damaging effect on myocardial and skeletal-muscle myocytes.

Serotonin (5-HT) and histamine metabolism was studied in 50 patients with diabetes melitus. Simultaneously the blood and urine content of their precursors and metabolites tryptophane, 5-hydroxytryptophane (5-HTP), 5-hydroxyindolylacetic acid (5-HIAA) and histidine was examined. An increase in 5-HT metabolism intensification (the augmented 5-HTP and 5-HT blood levels and enhanced 5-HTP and 5-HIAA excretion with the urine) was determined, whereas the blood and urine contents of histamine and histadine were within normal. Moreover, significantly higher increase in 5-HT blood level and enhanced 5-HIAA excretion with the urine were seen in patients with juvenile diabetes mellitus comparatively to those with insulin-depending type of the disease. Possible significance of changes, being discovered in 5-HT metabolism of patients with diabetes mellitus, in the disease pathogenesis is discussed.

The authors present the results of treatment of the patients with diabetes mellitus with hypoglycaemic preparations (sulphonylurea preparations of the second generation): glibenclamide, glibornuride and glydiazinamide. The following preparations were used: glibenclamide of Soviet make, maninil, daonyl, euglucon-5, euglucon, glutryl, glydiazinamide, glipizid. The treatment was carried out in 298 patients. The hypoglycaemic action of the active substances contained in one tablet of each of the preparation was approximately the same. Preparations of the second generation proved to be highly effective without any side-effects which required suspension of the treatment. The best results were obtained in complex therapy with the preparations of the second generation and with biguanides of prolonged action.
Some indices of the system of homeostasis, lipid metabolism, lipid peroxidation (LPO), the levels of IRI, C-peptide, glycosylated Hb and glycemia (on an empty stomach) were investigated in 131 patients with noninsulin dependent diabetes mellitus (the patients ranged in age from 40 to 60). Correlation analysis has shown that derangements in the system of homeostasis, lipid metabolism and LPO are correlated and interrelated, resulting in disorder of the arterial blood flow in the lower limbs and hence to the development and progression of diabetic angiopathies. The authors have proposed a scheme of LPO involvement in the development of lower limb diabetic angiopathies.
