[Hemostasis disorders in patients with pulmonary tuberculosis].

Coagulation studies in 82 patients with pulmonary tuberculosis showed that latent intravascular coagulation occurs not only in patients with active disease, but also in those with marked residual changes. The level of Cl-esterase inactivator may serve as an indicator of the disease prognosis: if its concentration is low, the prognosis is poor. Total fibronectin was measured in 76 patients with different tuberculosis activity. The results suggest that this protein's blood levels reflect the activity of the specific process in the lungs. Normal fibronectin concentrations indicate cure of tuberculosis.

[Initial and minor symptoms of tuberculosis in children].

The authors studied the diagnosis of initial and minor tuberculous manifestations in risk-group children using conventional and compementary methods. The latter comprised aspirin and blood chemiluminescence (CL) tests. The aspirin test rejected the infection as a cause of fever in 11.2% of the examinees. The addition to leukocytes of M. tuberculosis (BCG and staphylococcus strains) induced an intensive CL response to BCG mycobacteria observed in minor tuberculosis, whereas infected subjects with associated diseases displayed enhanced CL in response to staphylococcus.

[Tuberculosis morbidity in medical workers and the measures for their social protection].

Tuberculosis morbidity among the medical workers has been studied in 39 territories of Russia. It has been established that medical personnel contract tuberculosis much frequently than the population of Russia on the whole. Morbidity is particularly high among the personnel of institutions of the antituberculous service, being 293.3 per 100,000 working subjects. Tuberculosis morbidity is higher among females (78.8%) and in subjects aged before 50 years (81.4%). Success of tuberculosis control among the medical personnel can be provided by actively solving the problems of tuberculosis prevention, including the organization of health improvement measures under conditions of sanatorium and measures aimed at social protection.

[Study of lipid peroxidation in patients with tuberculosis of the female genital organs during subcutaneous tuberculin test].

The level of MDA in erythrocytes and plasma was determined in 71 female patients with a chronic inflammatory process in the genitals before and after subcutaneous tuberculin administration. In patients with active genital tuberculosis the MDA level rose significantly: in erythrocytes 24 hours and in plasma 72 hours after subcutaneous tuberculin administration. The MDA level remained unchanged in the rest of the patients.

[The status of and outlook for preventive fluorography in the RSFSR].

Fluorography is now the basic method for active detection of tuberculosis and other diseases of the thoracic organs and its detection amounts to 65% of active tuberculosis patients. There has been a tendency to decrease the number of individuals undergoing prophylactic fluorographic examination in the past 3-4 years, which is due to diverse reasons which can be united into 3 main groups. Recommendations are given on the differentiated rate of fluorographic examinations in relation to the incidence rates and socioeconomic conditions of a region. Factors are presented, which form the basis for forming population groups at a high risk for tuberculosis. Data are given on the development of a fluoromobile and a low-dose fluorograph of new design in the USSR, which provides high-grade fluorograms at a radiation load per patient 10 times less than the routine fluorographs.

[The effect of an ozone-oxygen mixture on Mycobacterium tuberculosis and conditionally pathogenic microorganisms].

The impact of a gaseous ozone-oxygen mixture (OOM) on Mycobacterium tuberculosis (MBT) and opportunistic microorganisms was studied. It was demonstrated that on 15- and 30-min exposure, OOM caused a significant decrease in the number of colonies of the grown microorganisms as compared with the control. The OOM mixture produced the highest effect of the MBT suspension, which is likely to be related to a greater surface of ozone contact with a cell than with a dense medium, and higher concentration of ozone and its highly active radicals that arise from a treatment process. After treatment with a gaseous mixture, 80-90% of the microbial cells lost their reproductive ability. The strains grown after OOM action retain drug sensitivity of the original strain. The results suggest that the use of a gaseous OOM mixture is promising in the treatment of tuberculosis to cleanse the destruction cavities and pleural empyemas, including those of nonspecific etiology.

[Thrombohemorrhagic and acute circulatory complications of the surgical treatment of tuberculosis].

The results of a number of studies on the hemostasis system in pulmonary tuberculosis patients are presented, and the analysis of pathophysiological processes of great importance in the development of intravascular hemocoagulation is made. It is clearly proved that an indispensable condition for the development of a complex of measures to increase the effectiveness of preventive treatment of thrombohemorrhagic and acute dyscirculatory complications after surgical management of tuberculosis patients is to define concrete biochemical mechanisms fundamental for intravascular hemocoagulation when certain hemostatic abnormalities occur during operation.

[The use of a concentrate of antithrombin III and disaggregants in the preoperative preparation of patients with pulmonary tuberculosis].

A study of the blood coagulation system in 32 patients with pulmonary tuberculosis before surgical intervention showed that reduction of the antithrombine III activity and related enhancement of the thrombin formation reaction and suppression of deaggregation capacities of thrombocytes reflect the latent process of pathological intravascular blood coagulation. The authors suggest justification of restoring the reduced antithrombine blood capacitance and normalization of the deaggregation capacity of thrombocytes by means of the antithrombine III concentrate and disaggregants in the complex preoperative care.

[The effect of carnosine on the healing of a lung wound].

The influence of carnosine (beta-alanine, alpha-histidine) on the process of lung wound reparation was studied in 90 guinea-pigs. Its efficacy was evaluated microscopically, histologically and by means of electronic microscope. It was established that carnosine, as compared to controls nearly twice accelerates reparative processes in the injured lung by activation of fibroblast proliferation, connective tissue generation and intracellular regeneration. In type II epithelial cells more intensive formation of osmiophilic bodies and lamellar bodies contained in them is observed. The alveolus formation in the wound lips by the 7-8th day after lung injury is going on under the carnosine influence, which is due, probably, to massive excretion of osmiophilic contents from type II epithelial cells and surfactant production. This may be connected with rapid decrease of atelectasis and rapid restoration of lung airness within the wound.

[Preliminary results of a study on shortening periods of treatment and temporary disability of patients with tuberculosis of the respiratory organs].

Clinical trials aimed at decreasing the terms of treatment and temporary disability in various categories of new cases of tuberculosis of the respiratory organs were conducted. It was shown to be possible at the account of decreasing the hospitalization period and increasing the role of controlled outpatient therapy.

[Experimental regeneration of the lungs].

In experiments on 150 guinea pigs, processes of lung wound healing were followed and the more active cellular sources of regeneration were identified. Measurements of cyclic adenosine monophosphate components in the course of lung healing indicated that histologic and electron-microscopic studies would be more informative if done at days 1, 3, 4, 7, and 14 after lung damage. Both cellular and intracellular mechanisms of regeneration were involved in the healing processes. The type II pneumonocyte be designated as a cambial cell of pulmonary parenchyma, for this cell displays the highest proliferative potency and is responsible for the epithelial lining of newly formed alveoli. The formation of these is shown to be assured through functional activity of all cellular alveolar elements (involving intracellular hyperplasia and hypertrophy of ultrastructures and especially dynamic morphological and functional alterations in osmiophilic bodies of type II pneumonocytes) and capillary neoformation.

[Mechanism of the hypotensive effect of sodium oxybutyrate in pulmonary hypertension in experimental tuberculosis in dogs].

The mechanism of sodium oxybutyrate effect on the lung vessels under normal conditions and under conditions of experimental tuberculosis in dogs was studied by the time course of pressure in the pulmonary artery and the content of serotonin, histamine and adenosine in blood of the aorta and pulmonary artery after the drug intravenous administration. To confirm the antihypoxic effect of sodium oxybutyrate, the levels of lactate, pyruvate, ATP, ADP and AMP were investigated. The levels of adenosine were judged indirectly by the activity of 5-nucleotidase, adenosine desaminase and AMP. It was shown that the decrease in pressure in the pulmonary artery after the use of sodium oxybutyrate was connected with the drug antihypoxic effect which led to lower levels of histamine in the aorta blood under the normal conditions and to higher levels of adenosine under the conditions of tuberculosis.

[Obtaining hybridomas producing monoclonal antibodies with different specificities against Mycobacterium tuberculosis of the human and bovine types].

A procedure for isolation of hybridomes producing monoclonal antibodies (McAB) to tubercle bacilli is described. Specificity of the McABs was studied with the solid phase radioimmune and immunoenzyme tests. Supernatant of tubercle bacilli destroyed with ultrasound was used as antigens. The McABs did not practically react with antigens of the tubercle bacilli atypical forms. Five ascitic monoclonal hybridomes were isolated. Four of them produced antibodies with selective specificity to antigens of bovine tubercle bacilli (M. bovis-8 and BCG) and one produced antibodies to antigens of human tubercle bacilli (H37Rv).
