[A psychoautonomic syndrome in the residual period of a modern "mild" military craniocerebral trauma].

As many as 18 patients with a history of military service in Afghanistan, who suffered (7.2 years before on the average) commotion due to the explosion wave were examined. The vegetative status was studied by different methods including cardiointervalography, as was the patients' emotional and personality sphere. The data obtained were compared to those obtained in three control groups (practically healthy subjects; those who suffered a mild home craniocerebral injury; healthy subjects who fought in Afghanistan and were not victims to craniocerebral injury). The main group patients demonstrated noticeable changes in the vegetative tone, vegetative reactivity and vegetative supply to physical activity as well as a high level of anxiety, which forms vegetodystonia. Attempts were made to correct the psychovegetative syndrome by central electroanalgesia in the tranquilization mode (9-10 daily sessions). The lowering of the tension of regulatory body systems functioning and the reduction of anxiety and depression tendencies were recorded.

[Autonomic disorders in the formation of the clinical manifestations of the initial forms of cerebrovascular lesions].

Among 103 persons aged 20 to 57 years, related occupationally to emotional stress, 48 persons were found to have vegetative dystonia, 28 initial manifestations of cerebral circulation failure, and 27 persons appeared healthy. The determination of the character and intensity of vegetative manifestations has shown that in the formation of vegetative dystonia, of paramount importance are cardiovascular and gastrointestinal disorders. Vegetative manifestations associated with initial manifestations of cerebral circulation failure include, in addition, respiratory disturbances and impairment of sleep function, which may reflect more deep changes in the activity of the regulatory apparatus in dyshemias of organic nature. A relationship was discovered between vegetative lesions and emotional disorders, particularly the level of anxiety, as was the contingency of vegetative dysfunction and decreased emotional stress tolerance.

[Compression-ischemic peroneal neuropathy].

The clinical picture is described of 4 patients with the syndrome of lesion of the peroneal developing during fixation of some physiological postures (squatting position etc.). The pathogenetic mechanisms of the disease are discussed. The authors report a characteristic monotypical clinical symptom complex of compression-ischemic peroneal neuropathy.

[The efficacy of lincomycin in tick-borne encephalitis].

Lincomycin was found to inhibit tick-borne encephalitis (TBE) virus. To test antiviral potential of this drug, a clinical trial was initiated entering TBE patients from known focuses of the disease (Novosibirsk, Kemerovo, Perm and Irkutsk Provinces). The drug was given to 23 patients with meningeal and meningoencephalitic TBE. A control group of 22 matched subjects received specific immunoglobulin. Resultant efficacy of lincomycin appeared not inferior to that of anti-TBE immunoglobulin. Lincomycin can be successfully introduced in the treatment of meningeal and meningoencephalitic TBE.

[Hemostasis in patients with ischemic stroke].

Modern informative techniques were employed to explore the coagulation, fibrinolytic and aggregation platelet systems of hemostasis in 117 patients in the most acute , acute, early, late restorative and residual periods of ischemic brain stroke. Disorders of the hemostatic system in the form of hyperaggregation, hypercoagulation, depression of fibrinolysis, and an increase of blood viscosity were revealed as were some specific features marking different period of brain stroke. Mono- and multi-systemic disorders of hemocoagulation and three types of coagulopathies were distinguished. Emphasis is laid on the necessity and feasibility of a goal-oriented selective correction of the pathological conditions of hemostasis at different periods of brain stroke by means of administering appropriate drugs.

[Characteristics of cerebrovascular circulation and its regulation in early stages of cerebrovascular disorders].

Cerebral circulation studied in 121 patients with early signs of cerebrovascular insufficiency exhibited typical asymmetric slowing down which started in distal compartments of the cerebral vascular bed and advanced to predominance in the vertebrobasilar circulation. Autoregulation of the circulation was preserved though hemodynamic requirements of emotional, mental and physical activity were not met completely. Initial discirculatory encephalopathy involves generalization of dysgemic shifts with progressive circulatory slowing down along the internal carotid arteries. In this case autoregulation gets disturbed in inhibited cerebrovascular reactivity.

[Arterial hypotension and the initial manifestations of cerebral circulatory failure].

A study was made of the health status of 20 persons examined for the first time 10 years before for a long-term and steady decrease of arterial pressure classified at that period as vegetodystonia of the neurotic character. 71 healthy test subjects served as a reference group. In addition to the clinical examination, a specialized questionnaire was used to examine the status of the vegetative nervous system; the actual mental status and personality traits were investigated by the MIL test modified by F. B. Berezin and M. P. Miroshnikov; the level of reactive and personality anxiousness were estimated with the aid of Spilberger's scale; the life events and changes were assessed as well. It has been revealed that vegetodystonia recorded in the disease history produces no effect on the frequency and times of the appearance of the initial manifestations of cerebral circulatory failure (IMCCF). At the same time the intensity of vegetative disorders in the structure of the clinical manifestations of the IMCCF was higher in persons, in whose disease histories vegetodystonia was recorded.

[The cerebral mechanisms of the autonomic disorders in vibration disease].

The status of the nonspecific brain systems was analyzed in 72 miners. Of these, 41, were in the premorbid stage and 31 were with the initial manifestations of vibration disease that occurred as a result of exposure to local vibration. The miners demonstrated dysfunction of the limbic-reticular complex in the form of concomitant vegetative, psychoemotional and nonspecific activation shifts. The syndrome of vegetative dystonia characteristic of the early (preclinical and manifest) nonspecific manifestations of vibration disease was a clinical reflection of cerebral dysfunction.

[Changes in cerebral blood circulation and their correction in the interparoxysmal period of epilepsy].

The linear cerebral blood flow (LCBF) was explored in 75 patients with epilepsy according to 131I-albumin transition. LCBF acceleration on the side of the focus, during which the attacks occur rarely and the background EEG remains unchanged is regarded as the most adequate LCBF. The grave course of epilepsy is associated with a decrease of the LCBF on the side of the focus. The pattern of LCBF alterations depends as well on the form of epilepsy. Subclinical activation of the epileptic focus by corasole or hyperventilation gives rise to an increase of the LCBF on its side in patients with generalized secondary epilepsy, to a lowering in temporal epilepsy, and the lack of dynamics in the precentral disease pattern. Correction of the hemodynamic disorders can be attained with the aid of combining phenobarbital and caffeine or papaverine. The pathogenetic importance of the hemodynamic disorders is discussed in connection with functional hypoxia of neurons.

[Multiple sclerosis in childhood].

The authors discuss the problems of the age-associated range of multiple sclerosis, characteristics of its clinical manifestations and course in childhood. Provide two cases of multiple sclerosis that had set in 11-year- and 14-year-old children. The first case was marked by a polysymptomatic debut with retrobulbar neuritis of the optical nerves, astereognosis and atetoid hyperkinesis in the hand. The second case was characterized by remitting oculomotor, vestibular, pyramidal disorders associated with facial neuropathy.

[Hemolytic reaction of sheep erythrocytes as a method of diagnosing multiple sclerosis].

The ability of the blood sera from patients with disseminated sclerosis (DS) to lyse native sheep red cells but not any other red cells was revealed. A method for the diagnosis of DS was developed: sheep red cells hemolysis test (SRCHT). The test was positive in all DS patients (altogether 96 cases) and negative in the controls. The hemolysin titer correlated with the disease severity, SRCHT permitted the diagnosis of DS long before manifestation of clinical symptoms.

[Psycho-autonomic correlations in patients with neuroses and in persons with risk factors of cerebrovascular disorders].

Psychovegetative correlations were made in 78 patients with neuroses and in 72 normal persons working under the conditions of emotional stress. The characteristics of emotional and vegetative responses have been delineated in patients with the well-defined syndrome of vegetodystonia and in persons working under the conditions at risk for the development of neurotic and vascular disturbances.

[A method of immunodiagnosis of myasthenia gravis].

A high-sensitive "immune" proserine++ test was developed for the early detection of myasthenia, especially of its concealed and unclear forms. The results are derived from counting Ach-ROK immune to blood acetylcholine (Ach) before and 30 min after subcutaneous proserin administration. A high (over 10%) baseline Ach-ROK blood count which fell sharply after proserin injection was characteristic of myasthenia patients as compared to controls (under 10%). The reverse was found in healthy individuals and patients with other nervous diseases: an increase of ROK up to 24-39%.

[Experimental and clinical study of the effectiveness of the combined use of anticonvulsants and tranquilizers in epilepsy].

Using experimental and clinical methods, the authors studied the efficacy of combined use of anticonvulsants (phenobarbital, hexamidine, chloracon, trimetin, benzonal, diphenylhydantoin, carbamazepine) and tranquilizers (diazepam, chlordiazepoxide, meprotan, trioxazine, mebicar) in epilepsy treatment. The experimental findings showed that 13 combinations of the above drugs had a synergic effect. Five of these combinations (hexamidine-chlordiazepoxide, benzonal-chlordiazepoxide, phenobarbital-diazepam, phenobarbital-trioxazine, and phenobarbital-mebicar also proved clinically more effective than the use of anticonvulsants alone.

[Suprasegmental autonomic regulation in syringomyelia patients and its alteration during radionuclide therapy].

Limbic-reticular dysfunction due to pathologic rearrangement in functional intracentral relations was evidenced in 85 syringomyelic patients. The dysfunction was initiated by the syringomyelic inflicted focus within the brainstem (syringobulbia) or spinal cord. Displayed were the psychoemotional, unspecific activational and hemodynamic correlates of this pathophysiological phenomenon. This is the first description of the radioactive iodine positive effect on limbic-reticular dysfunction in syringomyelia.

[Disorders of thyroid function as a consequence of severe craniocerebral injury].

In 32 patients with acute severe head trauma (SHT) the venous plasma levels of thyroid and thyrotrophic (TTH) hormones were studied and the functional state of the hypothalamus-hypophysis-thyroid system assessed. TTH hypersecretion and steady thyroid hypofunction are regular responses of the thyroid endocrine complex to SHT. Pathogenic mechanisms of the adenohypophyseal and thyroid dysfunctions in acute SHT are discussed and the need in their correction suggested.

[Intrahospital outbreak of enterovirus encephalitis].

The authors examined 9 children aged from 13 months to 9 years who, while being in a general hospital, contracted enteroviral encephalitis induced by Coxsackie B3 virus with a possible involvement of Coxsackie B1 virus. This report on a group hospital outbreak of enteroviral encephalitis appears to be the first of the kind in the available literature. The authors discuss the possibility of enteroviral encephalitis and Economo's encephalitis being identical in nature.
