Primary lymphomas of the gastrointestinal tract.

An institutional experience with primary gastrointestinal lymphoma (PGL) is reviewed. Included are 46 cases involving the stomach, 11 involving the small bowel, and two, the large bowel. The clinical, pathologic, and therapeutic aspects of PGL are discussed.

Idiopathic acquired sideroblastic anemia terminating in acute myelofibrosis: case report and review of leterature.

Acute myelofibrosis is a rare but distinct accelerated variant of agnogenic myeloid metaplasia that is characterized by marked anemia, peripheral blood myeloblastosis and normoblastosis, a lack of teardrop poikilocytosis, and prominent myelofibrosis. There is usually no palpable hepatosplenomegaly or lymph node enlargement. The clinical course is remarkable short. We describe a 63-year-old man who presented with idiopathic acquired sideroblastic anemia and subsequently developed acute myelofibrosis. Intensive polychemotherapy with vincristine, cytosine arabinoside, and prednisone and a later trial of oxymetholone therapy were ineffective. He died 134 days after the diagnosis of acute myelofibrosis was established. The 11 previously reported cases of acute myelofibrosis are reviewed, and the relationships of acute myelofibrosis to other myeloproliferative disorders and to idiopathic acquired sideroblastic anemia are discussed.

Dyskeratosis congenita: hematologic evaluation of a sibship and review of the literature.

Dyskeratosis congenita is a rare genodermatosis whose hematologic complications include pancytopenia of variable time of onset, a propensity for opportunistic infections, and neoplasia. A family in which the disorder segregated in 3 generations and involved 9 members is reported, and the hematologic data of the 46 previously reported cases are reviewed.

Bone marrow necrosis associated with a Mucor infection.

Peripheral blood cytopenia due to documented marrow necrosis is an unusual occurrence. This report describes a diabetic patient who presented with profound anemia and thrombocytopenia. Extensive bone marrow necrosis was demonstrated. He had a large renal cyst that contained hyphae later identified as Mucor species. It is postulated that the marrow necrosis was a direct or indirect result of Mucor infection. The phenomenon of bone marrow necrosis in reviewed.

Neutropenia: when and when not to treat.

When the number of neutrophils is substantially lower than normal or their function is impaired, the patient is at increased risk of infection. Antibiotic therapy is indicated only when infection is apparent.

Dyskeratosis congenita: clinical features and genetic aspects. Report of a family and review of the literature.

A large family with dyskeratosis congenita is reported. There were nine affected males, the findings in five of who are reported. We review 46 cases selected from the literature. The cardinal findings of this inherited multisystem disorder are delineated from these 51 cases. The complications of the disease, including opportunistic infection, are described. The parallel is made between dyskeratosis congenita and Fanconi's anaemia. The X-linked transmission of dyskeratosis congenita is confirmed by the family pedigree in this report. From the analysis of the families reported in the literature, there appears to be genetic heterogeneity in this disease. This study in our family indicates absence of close linkage between the Xga locus and the X-linked recessive form of dyskeratosis congenita.
