Polymorphonuclear cell function in the various polar types of leprosy and erythema nodosum leprosum.

Polymorphonuclear leukocyte motility, both in vivo and in vitro, and reduction of Nitro Blue Tetrazolium was studied in tuberculoid and lepromatous leprosy patients and a group of lepromatous patients with erythema nodosum leprosum (ENL). A profound defect in random migration, chemotaxis, and chemokinesis was found in lepromatous patients with and without complicating ENL, and marked depletion of skin window migration confirmed these in vitro findings. Tuberculoid patients exhibited a mild defect in polymorphonuclear leukocyte motility. Serum inhibitors of normal polymorphonuclear leukocyte chemotaxis were found in all types of leprosy, but sera from lepromatous and ENL patients were most inhibitory. Resting levels of Nitro Blue Tetrazolium reduction were normal in all three groups. Reconstitution of polymorphonuclear leukocyte cells from normal and ENL patients with ENL serum, however, showed increased Nitro Blue Tetrazolium reduction well above the normal range, whereas reconstitution with normal, lepromatous, and tuberculoid sera failed to increase Nitro Blue Tetrazolium reduction above the normal values.

Suppression of cytokine production by supernatants from CD8+ lymphocytes activated by mycobacterial fractions: the role of interleukins 4 and 6.

Supernatants derived from CD8+ lymphocytes treated with mycobacterial components, or the partially purified carbohydrates from these supernatants, increased the production of IL-4 and IL-6 by mononuclear cells. The addition of anti-IL4 or anti-IL6 antibodies to LPS stimulated MN cells incubated with supernatants from CD8+ lymphocytes or carbohydrates resulted in the restoration of other cytokine production by these MN cells. Recombinant IL-4 and IL-6 on their own suppressed the production of IL-1 beta, TNF-alpha, IL-2 and IFN-gamma by mononuclear cells. Such suppression could be reversed with antibodies to IL-4 and IL-6. The addition of rIL-4 and rIL-6 did not increase the suppression of cytokine production induced by suppressor supernatants or carbohydrates. Interleukin 4 decreased the production of IL-6 by MN cells; whilst IL-6 suppressed IL-4 production in a dose dependent manner. Both effects could be reversed with the appropriate antisera. Our results suggest that mycobacteria could evade host immunity by inducing the production of IL-4 and IL-6 by host mononuclear cells. These cytokines, in turn, would suppress the production of other cytokines necessary for effective cellular immunity.

HLA-A, B and C polymorphisms in three major populations living in South Africa.

The HLA class I antigen gene and haplotype frequencies of three major population groups living in the Transvaal province of South Africa were determined. The HLA-A, B and C antigens were tested in 877 South African Negroids, 381 Coloured and 771 Caucasoids. Differences in gene and haplotype frequencies between the various populations were observed. HLA-A11 appears with a low frequency in the Negroids (gf = 0.004) and the Caucasoids revealed an absence of HLA-Bw70. The frequencies of the HLA antigens in the Coloured population were generally intermediate between those of the other two groups. The most frequent haplotypes were A3, B7; A30, B8 and A2, B58 in the Caucasoids, Negroids and Coloured respectively. Genetic distance analysis reveals that Negroids and Caucasoids are distinct groups and the Coloured occupy an intermediate position.
