[Comparative characterization of life quality in patients with pulmonary tuberculosis after surgical treatment].

The parameters of life quality were comparatively analyzed in 59 patients aged 18 to 63 years, operated on for tuberculomas and cavitary masses of the lung in 1995 to 2002. Each patient underwent atypical pneumonectomy in the volume of not more than 2 segments via anterolateral access. Life quality was assessed, by using the Medical Outcomes Study Shirt Form (SF-36) developed by John E. Ware (Health Institute, New England Medical Center, Boston, Massachusetts). This study demonstrates that timely surgery in combination with chemotherapy can significantly improve all the constituents of life quality in patients with pulmonary tuberculosis. The assessment of the trend of life quality parameters helps to optimize a management policy and individualize further physical, psychoemotional, social, and working rehabilitation.

[Comparative clinical-economic analysis of antibiotic therapy in pyogenic inflammatory diseases of the lower extremities].

A randomized comparative investigation was carried out in two equal groups of patients with pyo-inflammatory diseases of lower extremities (the total number 50 patients) in order to study effectiveness and tolerance to Netilmycin (1st group) and Gentamycin (2nd group) given in combination with Cefasolin. Clinical symptoms were estimated immediately after operation, in 3, 6 and 10-12 days after it. Bacteriological investigations were fulfilled immediately after operation, in 72 h and in 6-10 days after the beginning of antibacterial therapy. Clinical and biochemical investigations of blood were fulfilled before and in 10 days after the beginning of the treatment. Effectiveness of the treatment in the first group was 100%, in the second group--80%. In the second group the antibiotics were changed in 20% of cases and the average duration of hospitalization among the patients of this group was reliably longer that in the first group. The eradication rating of Netilmycin was higher than that of Gentamycin (25 strains from 25 and 20 from 25 respectively). Gentamycin had a pronounced nephrotoxic effect (elevation of the level of creatinin and urea of blood in dynamics by 21% and 32%), as compared with Netilmycin (9% and 3%). Total expenses to antibiotic therapy in the first group made up 97,650 rub, and in the second group 106,245 rub. Netilmycin in combination with Cefasolin was more effective for acute pyo-inflammatory diseases of lower extremities than a combination of Gentamycin with Cefasolin, it more rapidly resulted in reduction of clinical signs of inflammation, was better endured and more economical.

[Adrenergic mechanisms of pain regulation].

The authors briefly account the opioidergic regulation of pain and postulate a concept of adrenergic pain-relieving systems. They also analyze the analgetic and vegetocorrecting effects of clofelin and its benefits as a centrally acting non-opiate analgetic. They discuss the status and prospects for optimization of non-opiate anesthesia in clinical practice.

[Radiologic diagnosis of cavernous lung cancer].

The authors evaluated the potential of x-ray tomography in diagnosis of cavernous pulmonary carcinoma under single or multiple examinations. The diagnosis involves combination of x-ray morphological indications characteristic of the cancer cavern wall. Out of 23 verified cases 20 were diagnosed basing on the results of multisection x-ray tomography images.

[Neurophysiological mechanisms of the antihypertensive effect of clopheline in pain].

In experiments on conscious and unanesthetized cats it was shown that clopheline in analgesic doses do not change the pain baroreflex blood pressure regulation and mild brain antinociceptive sympathoactivating influences. The clopheline antihypertensive effect was due to nonopiate direct sympathoinhibitory effect realized by suprasegmental level of vasomotor regulation.

[Isolation of monoclonal antibodies for the identification of strains of the causative agent of plague that do not contain a capsular antigen].

In most cases the immunological identification of Y. pestis strains is based on the use of capsular antigen as an immunological marker. However, there are Y. pestis strains without capsular antigen. For the immunological identification of such strains, homogeneous antigen with a molecular weight of 43 KD has been isolated and monoclonal antibodies to it have been obtained. The enzyme-linked immunosorbent assay, carried out with the use of these monoclonal antibodies and intended for the detection of antigen with a molecular weight of 43 KD, has been developed. The sensitivity of the assay is about 10 ng/ml.

[The use of clofelin in the acute period of myocardial infarct].

Data are reported on the action of intravenous clophelin in 30 patients administered within the first 24 hours of myocardial infarction. The agent exhibited a significant analgetic effect, sedative action and normalized the main hemodynamic values. The effect of clophelin on the hemodynamics depended on its initial state. The possibility is shown to predict the pain-killing and hemodynamic effects of the drug by the initial response.

[Effects of clopheline on impulse activity of neurons in the midbrain and dorsal horn of the spinal cord].

Clophelin (1-5 mg/kg) suppresses spinal dorsal horn neuronal nociceptive responses but does not change their touch stimuli reactions in unanesthetized curarized cats. This effect is steady to naloxone (1 mg/kg), yohimbine (5 mg/kg) and removal by prazosin (1 mg/kg). Clophelin depresses nociceptive responses of the central gray matter neurones which generalized pain impulses in the supraspinal structures.

[Effect of 5,7-dihydroxytryptamine on pain sensitivity and analgesic activity of morphine].

Electron microscopy of rats ultrathin sections from dorsal and central raphe nucleus and spinal cord after 5,7-dihydroxytryptamine intracisternal microinjection (200 micrograms) has revealed neurones and axonal terminals distruction, which associated with tail-flick hypoalgesia and blood pressure nociceptive reactions diminished. In this condition the morphine (2 mg/kg) analgesia and drug depressive effect on pain hemodynamic manifestations increase significantly.

[A new principle in the analysis of the analgesic action of morphine].

Possible application of sensory decision theory of pain for the experimental assessment of neuropsychophysiological mechanisms of opiate analgesia has been demonstrated. The analgetic effect of morphine was found to be mediated through the influence on the measurement and estimation of pain stimulus.

[The role of various opiate receptors in the regulation of the nociceptive reaction of the arterial pressure].

The experiments on alert rats have shown that dissociation in opioid regulation of behavioural and hemodynamic pain manifestations is determined at a spinal opiate receptor level. Opiates and opioids suppress blood pressure nociceptive reactions to mu-opiate receptors, while sigma-opiate receptors are involved into the generation of autonomic activating effect in opiate analgesia.
