Further evidence for involvement of both cell mediated and humoral immunity in generalized vitiligo.

Immunohistochemical and immunoserological evidence supports the involvement of both cell-mediated and humoral mechanisms in the pathogenesis of melanocyte destruction in vitiligo. Punch biopsies from depigmented vitiliginous skin (VS), normal-looking pigmented skin (PS), and marginal skin (MS) from patients with generalized vitiligo (n = 15) were labeled with K 1.2.58, OKM1 (CD11b), Leu 11b (CD16), Leu 19 (CD56), IFN-gamma receptor, IL-2 receptor (CD25), IgG, IgM, C3c, and C3d MoAbs. In addition, in vitro effects of vitiligo sera (n = 13) on human newborn melanocytes (HMel) under different culture conditions were studied. The immunohistochemical findings showed absence of K 1.2.58+ epidermal melanocytes in VS and abnormal morphology in MS. In these areas, a few CD11b+ cells in the dermis and epidermis could be detected but no significant numbers of CD16+ or CD56+ cells were seen among the mononuclear cellular infiltrate. IL-2 and IFN-gamma receptors were clearly expressed by the cellular infiltrate. No significant deposition of complement or immunoglobulin was seen. The addition of vitiligo sera to HMel cultures induced a significant cellular proliferation. The stimulation of cell proliferation occurred regardless whether the sera were added alone or when preheated (56 degrees C for 1 hr) and then supplemented with a complement source (P < 0.01 at 2%, P < 0.001 at 10%, and P < 0.01 at 20% for sera alone) (P > 0.05 at 2%, P < 0.05 at 10%, and P < 0.01 at 20% for decomplemented sera plus complement).(ABSTRACT TRUNCATED AT 250 WORDS)

Anatomical basis of latissimus dorsi myocutaneous flap: clinical applications.

The anatomical study was carried out using 40 preserved cadaveric specimens of latissimus dorsi myocutaneous flaps. It included dissections, angiographies and tissue clearing after arterial injection with latex. The results proved that the dominant vascular supply, the thoracodorsal a., has an almost constant pattern of intramuscular distribution. In addition, there are two sets of segmental pedicles, from the intercostal and lumbar vessels, supplying the medial third of the muscle. The accurate localization of the intramuscular distribution of the thoracodorsal a. permitted the proper design of several island flaps with competent vascularity. The designed flaps were successfully applied in 10 cases for reconstruction of breast, axilla, neck and chest wall.

Preliminary report on the therapeutic effect of khellin in psoriasis.

A double-blind study has been performed to search for the possible effect of khellin in psoriasis. 10 patients were treated orally with khellin and subsequently exposed to sunlight for 4 months; a total of 8 cases responded positively with variable degrees of clearance. None of 10 controls had any response. A relapse-free duration of 3 years was recorded for 6 out of the 8 responders. This study would introduce the nonpsoralen compound khellin as a possible agent for the therapy of psoriasis.

An approach to the treatment of vitiligo by khellin.

An anticipated effect of khellin, related to its chemical structure, in the photochemotherapy of amelanosis has been investigated. In a double-blind clinical study, khellin has been orally administered to 30 vitiligo patients for 4 months with subsequent exposure to natural sunlight. At the end of the trial period, 5 patients out of 30 (16.6%) repigmented 90-100%; 7 cases (23.3%) repigmented 50-60% of the vitiliginous areas treated; 11 (36.6%) repigmented 25% or less, and 7 subjects (23.3%) showed negative response. 30 control subjects failed to repigment at all. The achieved repigmentation was stable after drug cessation for a period of 1 year. These data add a previously unreported effect for the non-psoralen compound, khellin, in the therapy of vitiligo.

A double-blind comparison of a new combination (halcinonide-neomycin-amphotericin) and active controls in cutaneous candidiasis and steroid-responsive dermatoses.

One hundred patients participated in the studies of cutaneous candidiasis and steroid-responsive dermatoses. Seventy patients with the former diagnosis were treated with either halcinonide-neomycin-amphotericin or hydrocortisone-iodochlorhydroxyquin ointment combinations on a double-blind parallel comparison basis. Similar results were obtained after both therapies. Thirty patients with symmetrical bilateral lesions of steroid-responsive dermatoses were treated with halcinonide-neomycin-amphotericin cream on the lesions on one side of the body and hydrocortisone-iodochlorhydroxyquin cream on the opposite side. A double-blind design was used in directly comparing the response to each of the two drugs. Considering the results of all dermatoses together the test combination was statistically superior to the control cream (p less than 0-05). However, while numerical superiority of good responses to the halcinonide combination was recorded in psoriasis, no statistical significance could be derived due to the limited number of cases. No side-effects occurred with any drug combination use. In the opinion of the authors halcinonide-neomycin-amphotericin cream and ointment are safe and effective in the treatment of cutaneous candiasis and steroid-responsive dermatoses.

Primary cutaneous cryptococcosis in Egypt.

Neither systemic nor primary cutaneous cryptococcosis has ever been reported in Egypt. The case of a 14-year-old girl with a solitary primary cutaneous cryptococcosis lesion is presented. A surgical specimen proved the diagnosis, although the lesion resembled a rapidly growing epithelioma. Follow-up for 4 months did not reveal evidence of systemic involvement.

Tinea versicolor hypopigmentation: histochemical and therapeutic studies.

One hundred and thirty-four patients with tinea versicolor were divided into 3 treatment groups: (1) 30% sodium thiosulfate application, (2) 30% sodium thiosulfate application plus ultraviolet light, and (3) 30% sodium thiosulfate application, ultraviolet light, and meladinine tablets (amoidin 0.01 g and amidin 0.005 g). Analysis of the 79 patients who completed the 6-week study showed no statistical difference. Therefore, topical treatment should be instituted as quickly as possible to prevent pigmentary changes. Ultraviolet light should be used only after the fungus has cleared in order not to darken the skin further.
