Serum from patients with threatened abortion. Effect on in vitro development of mouse embryos.

The effect of serum from patients with threatened abortion on in vitro development of two-cell mouse embryos was evaluated. Embryos were cultured for 72 hours in 7.5% serum supplement from patients with threatened abortion and from women with normal pregnancies and in Ham's F-10 medium with no serum supplement. Significantly fewer embryos developed to the blastocyst stage at 72 hours in the threatened abortion group as compared with the normal pregnancy group and Ham's F-10 (P < .001). These results suggest the presence of serum factors that are toxic to early embryonic development and thus may play a role in the pathogenesis of threatened abortion.

Morphological assessment of polycystic ovary using transvaginal ultrasound.

A total of 104 patients with bilateral polycystic ovaries (PCO) diagnosed by transvaginal ultrasound were classified into four groups (A < 5 cysts, B 5-10 cysts, C > 10 cysts located under ovarian capsule, D numerous cysts distributed all over the ovary) according to the number of microcysts. Ovarian volume, clinical features (menstrual disturbance, obesity and hirsutism), endocrine levels [luteinizing hormone (LH), LH/follicle stimulating hormone ratio, testosterone, androstenedione and dehydroepiandrosterone-sulphate] and response to clomiphene were compared for each group and 17 control subjects with normal ovulatory cycles. Ovarian volume in PCO subjects was significantly larger than that in control subjects. As the number of microcysts increased, the ovarian volume enlarged and the endocrine abnormality worsened. All endocrine parameters in PCO subjects with menstrual disturbances were significantly higher than those in PCO subjects without menstrual disturbances and controls. In PCO subjects without menstrual disturbances only androstenedione was significantly higher than in controls. Among PCO subjects, the frequency of menstrual disturbances, the percentage of response to clomiphene and the pregnancy rate in groups C and D were significantly higher than in groups A and B. Therefore, screening the ovaries in infertile patients by means of transvaginal ultrasound and subsequent assessment of morphology in PCO may suggest endocrine abnormality and help in selecting the proper treatment.

Morular metaplasia of the endometrium in a patient with polycystic ovary syndrome.

A 31-year-old women with polycystic ovary syndrome underwent dilatation and curettage (D&C) as part of her infertility workup. Pathologic examination revealed endometrial morular metaplasia without malignant potential. Follow-up D&C had no evidence of endometrial hyperplasia or carcinoma. The diagnostic difficulties and pathogenesis are discussed.

CA-125 in menstrual discharge in patients with chronic pelvic pain.

CA-125 levels in menstrual discharge were determined in 55 patients with chronic pelvic pain to evaluate whether this test would be useful in differentiating between pelvic pain due to endometriosis and other causes. Of the 28 women with endometriosis, 25 (89%) had CA-125 concentration greater than or equal to 72,000 units/ml. The frequencies of elevated levels in Stage I, Stage II and Stages III/IV were 85.7, 85.7 and 92.8%, respectively. When used for the detection of endometriosis, the test had a sensitivity of 89.3% and a specificity of 96.3%. These results suggest that CA-125 in menstrual discharge may be helpful in the evaluation of women with chronic pelvic pain.

Endosalpingiosis in association with ovarian surface papillary tumor of borderline malignancy.

A 40-year-old woman underwent total abdominal hysterectomy, bilateral salpingo-oophorectomy, pelvic/paraaortic lymphadenectomy, and omentectomy for Stage IA ovarian surface papillary tumor of borderline malignancy. Microscopic examination revealed endosalpingiosis of ovaries, the peritoneum of uterus, pelvic lymph nodes, and omentum. There was no evidence of disseminated peritoneal malignancy. The several proposed theories of the pathogenesis of endosalpingiosis are reviewed, but that involving metaplasia of the multipotential peritoneal cells is presented as the most acceptable.

The effect of serum obtained before and after treatment for endometriosis on in vitro development of two-cell mouse embryos.

OBJECTIVE: To evaluate the effect of serum obtained before and after treatment for endometriosis on in vitro development of two-cell mouse embryos. DESIGN: Pretreatment and post-treatment comparison of mouse embryo development in serum supplements from patients with endometriosis; results were compared using chi 2 analysis. SETTING: Infertility Clinic, Shimane Medical University, Izumo, Japan. PATIENTS: Ten consecutive women with endometriosis who underwent treatment for the disease. Seven women who were managed expectantly served as controls. INTERVENTIONS: All patients underwent laparoscopic or conservative surgery. This was followed by a 6-month course of either danazol 400 mg/d (6 patients) or buserelin acetate 900 micrograms/d (4 patients). MAIN OUTCOME MEASURE(S): Total number of embryos that reached blastocyst stage after 72 hours of incubation were compared before and after treatment. RESULTS: Before treatment, 23% of the embryos reached blastocyst stage, whereas 8 of the 10 serum samples were embryo toxic. After treatment, significantly more embryos (59%) developed to blastocysts, and only one serum sample remained embryo toxic. In the control group, there was no significant difference in the number of embryos that reached blastocyst stage after expectant treatment. CONCLUSION: The embryo toxicity of serum samples from patients with endometriosis is lost after treatment.

Effect of serum from patients with endometriosis on the development of mouse embryos.

The effect of serum from patients with endometriosis on the in vitro development of 2-cell mouse embryos was evaluated in the present study. Embryos were cultured for 72 h in 5% serum supplement from patients with endometriosis, infertile patients with no evidence of endometriosis, and Ham's F-10 medium with no serum supplement. Significantly less embryos developed to the blastocyst stage at 72 h in the endometriosis group as compared with the nonendometriosis groups and Ham's F-10 medium (p less than 0.05). These results suggest the presence of one or more serum factors that are toxic to early embryonic development and thus may play a role in the infertility associated with endometriosis, especially with respect to early embryonic loss.

Pregnancy following transvaginal sonographic guided aspiration of endometrioma.

A 30-year-old infertile women with stage IV endometriosis was treated initially with endoscopic surgery, danazol and intrauterine insemination to no avail. Before starting ovulation induction for in vitro fertilization, transvaginal sonographic guided aspiration of endometrioma was performed. One month after the procedure the patient became pregnant and delivered a normal child at term.

Correlation between postswim-up hypoosmotic swelling test and in vitro fertilization results.

OBJECTIVE: To determine correlation between hypoosmotic swelling (HOS) test on post-swim-up sperm and results of IVF. SETTING: University clinic IVF program. PATIENTS AND INTERVENTIONS: Sixty-three women, with various infertility factors; Buserelin plus hMG and hCG used for all patients. HOS test performed on original and swim-up samples of husband's semen. OUTCOME MEASURES: Fertilization, as shown by two pronuclei and two polar bodies 18 hours post-insemination. RESULTS: Post-swim-up HOS test had a correlation (r) of .68 (P < .001) with fertilization.
