Investigation of bacteremia after orthodontic banding and debanding following chlorhexidine mouth wash application.

This study investigates the prevalence of bacteremia after orthodontic banding and debanding, following the application of a 0.2% chlorhexidine gluconate mouthwash. The banding and debanding groups were each composed of 40 young adult patients. In the banding group, patients were asked to rinse their mouth with chlorhexidine gluconate for 60 seconds just prior to fitting of the bands. In the debanding group, they were asked to use the mouthwash immediately before removal of bands and brackets. In both groups pre- and post-treatment blood samples were obtained with a strict aseptic technique. In the banding group, no bacteremia was detected in the pretreatment sample and 2.5% post-treatment bacteremia was detected in the post-treatment sample. In the debanding group, 2.5% bacteremia was found in both the pre- and post-treatment samples. The prevalence of post-treatment bacteremia found in the present study were compared with the findings of 2 preliminary studies in which the prevalence of bacteremia had been investigated after banding and debanding without a prior application of chlorhexidine mouthwash. The application of chlorhexidine mouthwash resulted in a decrease in the prevalence of bacteremia after banding and debanding, but the decrease was not statistically significant.

Selectins in normal pregnancy, pre-eclampsia and missed abortus.

Selectins, are known to be increased in the serum of patients with pre-eclampsia, indicating that these molecules are possible markers of endothelial cell injury. In this study, we investigated P, E and L selectin levels in normal pregnancy, pre-eclampsia, and missed abortus. Plasma P and L selectins levels were significantly higher in normal pregnancy and pre-eclampsia than healthy controls; but plasma concentrations of E selectins were not different between these groups. Plasma P selectin was significantly higher in pre-eclampsia than normal pregnancy. Plasma concentrations of all selectins were significantly higher in missed abortus than healthy control. L selectin levels were higher in pre-eclampsia and missed abortus than normal pregnancy. We found the levels of selectins were increased in pre-eclampsia and missed abortus. Although selectins were suspected to play a role in the pathogenesis of pre-eclampsia, in conjunction with previous studies, we thought that elevated selectin levels are a non-specific consequence of endothelial injury rather than being a cause.

Investigation of bacteremia following orthodontic debanding.

The aim of this study was to investigate the prevalence of bacteremia after orthodontic debanding and debonding. The study group comprised 30 patients (10 men, 20 women). All were treated using the Edgewise technique. Patients with acceptable oral hygiene and fixed appliances in both jaws were included in the study group. Blood samples were obtained using a strict aseptic technique before and after removal of bands and brackets. A 6.6% bacteremia prevalence was observed in both preoperative and postoperative blood samples.

Chromosomal abnormalities in mentally retarded children in the Konya region--Turkey.

Etiology of mental retardation is diverse. 120 Students from 11 special training, education, and rehabilitation subclasses were investigated cytogenetically for determining the contribution of chromosomal abnormalities to mild mental retardation. 23 of the 120 children (19%) had chromosomal abnormalities: thirteen cases a classical trisomy 21 (the male:female ratio was 9:4), three a balanced autosomal reciprocal translocation, one a pericentric inversion of chromosome 9, and six fragile-X syndrome (The male:female ratio was 5:1).

Simultaneous occurrence of chronic myelogenous leukemia and non-Hodgkin lymphoma at diagnosis.

We describe a case with the simultaneous occurrence of chronic myelogenous leukemia (CML) and non-Hodgkin lymphoma (NHL). Peripheral blood (PB) and bone marrow (BM) smears showed typical CML features. Lymph node biopsy exhibited a large-cell NHL. The Philadelphia chromosome or its molecular counterpart, the BCR-ABL gene fusion, by detecting with dual color-(DC) fluorescence in situ hybridization (FISH), was detected reliably both in metaphase spreads from BM and in interphase nuclei from BM and follow-up PB cells, but was not detected in the lymph node cells. Clinical features and laboratory findings show this case having a coexistence of CML and NHL.

Cephalometric evaluation of open bite treatment with NiTi arch wires and anterior elastics.

The aim of this study was to evaluate cephalometrically the effects of open bite treatment with NiTi arch wires and anterior elastics. The study group comprised of 17 patients who displayed a high angle skeletal pattern, along with an anterior open bite. After initial leveling, 0.016 x 0.022 inch upper accentuated-curve and lower reverse-curve arch wires were placed, with anterior elastics applied in the canine regions. Cephalometric assessment was carried out on lateral head films taken at the beginning of treatment and on average 2.8 months after open bite closure was obtained. The results of this study indicated that open bite closure had been achieved mainly by extrusion of the lower incisors and uprighting of the upper incisors. The functional occlusal plane was leveled by extrusion of lower premolars and uprighting of lower molars. Lateral cephalograms obtained from 10 patients who had been available after 1 year postretention were used to evaluate relapse changes. During the follow-up period, position of the upper and lower incisors and the inclination of the occlusal plane were maintained. However, extrusion of upper and lower molar teeth resulted in a reduction in overbite.

Investigation of bacteremia after orthodontic banding.

The aim of this study was to assess the incidence of bacteremia after orthodontic banding. The study was conducted on 40 healthy orthodontic patients with good oral hygiene. Venous blood samples were obtained with a strict aseptic technique before and after fitting of a molar band in each patient. Microbiologic evaluation of the samples revealed a postoperative bacteremia incidence of 7.5%.

Continuous vs. discontinuous force application and root resorption.

The aim of this study was to compare the effects on root resorption of continuous and discontinuous force application. The experimental material consisted of 22 first premolars that were to be extracted as part of orthodontic treatment. Prior to extraction, a 100 g tipping force was applied to the experimental teeth by means of elastics. One side was randomly selected to be the continuous force side, and the contralateral side became the discontinuous force side. Elastics were worn 24 hours per day on the continuous force side and 12 hours per day on the discontinuous side. The experimental procedure lasted 9 weeks. Composite electron micrographs of the buccal surface of each specimen were digitized and areas affected by resorption were determined. The degree of root blunting was assessed by visual scoring. Mean percentage of resorption-affected areas was smaller and apical blunting was less severe on the discontinuous force side. The results of this study show that the application of discontinuous force results in less root resorption than does the application of continuous force.

Comparisons between cervical vertebrae and hand-wrist maturation for the assessment of skeletal maturity.

It was claimed that, skeletal maturity could be determined by using anatomical changes of the cervical vertebrae observed on the lateral cephalometric radiographs. In this method of assessment cervical vertebrae C2, C3, and C4 are observed and each patient is placed in a cervical vertebrae maturation index (CVMI). Fishman developed a system of hand-wrist skeletal maturation indicators (SMI), using four stages of bone maturation at six anatomic sites. The purpose of this study was to analyze associations between cervical vertebrae maturation index (CVMI) and skeletal maturation index (SMI). The second objective was to determine the reproducibility of the identifications on the lateral cephalograms and hand-wrist films. Lateral cephalometric and left hand-wrist radiographs of 180 untreated subjects (99 girls and 81 boys) aged from 8 to 18 years were obtained from the files of the Marmara University School of Dentistry, Department of Orthodontics. The results of this study indicated that cervical vertebrae maturation and hand-wrist skeletal maturation were significantly related.

J incision in neck dissections.

Metastasis in the neck lymph system of primary tumours of the head and neck is frequently seen. In order to prevent this metastasis, neck dissection is carried out by various types of skin incisions. In this study, types of skin incision used in neck dissections were defined, and the advantages, disadvantages and results of J incisions, which have been performed on 320 radical neck dissection patients in our clinic between 1985-1996, were compared with those of other incision types.

Application of the multi-colour FISH to interphase nuclei and metaphase spreads for simultaneous examination of monosomy 7 and trisomies 8 and 11 in acute myelocytic leukaemia (AML).

We have used the multi-colour (three) fluorescence in situ hybridization (FISH) technique based on the ratio labelling for the detection of monosomy 7 and trisomies 8 and 11 in 13 cases of acute myeloid leukaemia (AML). Two out of the 13 AML cases showed monosomy 7 and two out of the remaining cases exhibited trisomy 8 in interphase nuclei. Three of these results were confirmed by metaphase-FISH study. Trisomy 11 was not found either by the interphase FISH study or in the metaphase FISH study. These results demonstrate the potential power of multi-colour FISH using ratio labelling to produce more fluorescein colour in interphase nuclei for the detection of aneuploidies in leukaemia.

Human leucocyte antigens in tympanosclerosis.

This study was designed to evaluate the association between certain HLA antigens and tympanosclerosis. The serum concentrations of HLA antigens were measured by a microlymphocytotoxicity technique in patients with tympanosclerosis and compared with a healthy control group. The serum levels of HLA-B35 and -DR3 were significantly higher in the patients with tympanosclerosis. This result suggests that certain types of HLA antigens may play an important role as an indicator or mediator in the pathogenesis of tympanosclerosis.

The frequency of consanguinity in Konya, Turkey, and its medical effects.

This study was conducted in the town of Konya, Turkey, on 1120 randomly selected women to find out the overall rate of consanguineous marriages among couples. The frequency of consanguineous marriages was found to be 23.2%. It was found that 14.6% of this figure was first cousin marriages and the rest was 8.6%. Consanguineous marriages were higher among women born in villages compared to those born in provinces and the town center. Based on the findings, it was not too difficult to say: the higher the level of education of women, the lower the rate of consanguineous marriages. The number of children with an abnormality was high in consanguineous marriages, while the frequency of spontaneous abortion, still-birth and infant death remained the same.

A new needle suspension procedure for genuine stress incontinence and anterior vaginal wall prolapse.

A new suspension method was developed for the correction of anterior vaginal wall relaxation and genuine stress incontinence. This procedure suspends the anterior vaginal wall to the anterior rectus fascia, and in doing so gives support to the bladder neck, anterior vaginal wall and vaginal apex. The procedure is performed at the time of vaginal hysterectomy or correction of anterior vaginal wall relaxation. The authors present their experience with this technique in 31 patients.

Epididymo-orchitis as a complication of brucellosis.

A study of 84 patients who were diagnosed with epididymo-orchitis between July 1987 and September 1993 is presented. Brucellosis was a complication in 14 cases (17%). All 14 cases had elevated agglutination titers. Brucella blood culture was found to be positive in 4 of 14 cases (28.5%). Standard therapy regime (streptomycin plus tetracycline) was effective in 13 of 14 (93%) cases.

Sweet's syndrome associated with G-CSF.

Sweet's syndrome (SS) developed in two patients with acute myeloid leukaemia (AML) treated with granulocyte colony stimulating factor (G-CSF) for febrile neutropenia due to AML chemotherapy. Fever, painful skin and conjunctival lesions developed and neutrophilic infiltration was detected at biopsy specimens. Neutrophilia was not detected. Skin lesions regressed within 1-2 weeks and conjunctival lesions within 4 weeks following the cessation of G-CSF. We conclude that SS may be a complication of G-CSF therapy and tender skin and/or conjunctival lesions developing during G-CSF therapy should suggest the possibility of SS.

An unusual case of Xanthoma disseminatum.

A 10-year-old boy was admitted to the University Hospital complaining of exhaustion, night sweats, coughing and dyspnea. Physical examination, laboratory findings and histopathological examination led to the diagnosis of Xanthoma disseminatum. Because of dyspnea a tracheostomy was performed 3 months before diagnosis was made. Outline of treatment and management of this patient is discussed in this paper.
