Hyperprolactinemia and psychological distress in women undergoing chronic hemodialysis.

In order to investigate the possible relationship of hyperprolactinemia to psychological distress in patients undergoing chronic hemodialysis, 19 uremic women were evaluated by a semistructured interview and administered the Kellner Symptom Questionnaire. Group A (10 uremic women with hyperprolactinemia) did not differ significantly in anxiety, depression, somatization and hostility from group B (9 normoprolactinemic uremic women). Both groups rated themselves more depressed and hostile than a normal control group of 10 women, and hyperprolactinemic patients were also significantly more anxious than the normal controls.

Hyperprolactinemia and sexual disturbances among uremic women on hemodialysis.

The investigation of a sample of 99 women on maintenance hemodialysis has shown the presence of sexual disturbances to a great extent: the rate of sexual intercourse and the ability to reach orgasm were significantly lower than in age-matched control women. 80% declared a reduction in their sexual desire and the frequency of intercourse was also lower as compared to the period prior to dialysis. Ageing decreased the sexual activity in both the ill and healthy population, but in uremic patients the sexual activity ended at an earlier age. The patients with hyperprolactinemia reported lower frequencies of intercourse and lower percentages of orgasm than normoprolactinemic ones. The incidence of sexual dysfunction and the role of hyperprolactinemia in this respect were similar to those which are found among male patients on hemodialysis.

[Our experience with cysts of the pancreas].

The Authors produced their series of pancreatic cysts and pseudocysts. In order to a correct surgical therapy they deem it essential to do the Wirsungraphia or the cysto-wirsungraphia to recognize the "communicating cyst" and the "non communicating cyst". The simple resection and the cystoparenchimal resection are the operations of choice but the cystoanastomosis provides suitable and definitive results in the same way. Whereas the marsupialization and the external drainage are operations to reserve for the high risk patients, for the hazard of the pancreatic fistula particularly in the "communicating cyst".

[Emergency or deferred treatment of perforated and bleeding gastroduodenal ulcer].

The treatment of perforated peptic ulcer, according to the experience of the Authors (327 cases), must be very pressing and always surgical. The closure of the perforation, the simplest treatment, seems the most suitable and the least dangerous for the survival of the patient, even though 68% of these patients will have to undergo surgery again in the course of 10 years. The radical treatment of resection, is reserved to few and carefully chosen cases, because it is loaded with sufficient risk. Acute bleeding peptic ulcer (86 cases) requires the operation: immediate or delayed according to the importance of the hemorrage: very serious, serious, moderate.

The cancer of the papilla of Vater. Diagnosis, treatment and long term results.

Survey of 18 cases of cancer in the Papilla of Vater, which were observed during the years between 1971 and 1979 at the Surgical Clinic of Padua University - Seat of Verona - with particular regard to the diagnostic means and their reliability. The histological investigations gave the following results: All lesions were found to be adenocarcinomas; 6 cases well differentiated, 4 cases fairly well differentiated, 2 cases little differentiated, 3 cases indifferentiated. The following operations were carried out: 15 Duodenopancreatectomies, 3 Internal derivations of the biliary flow, one of which associated with gastroenteroanastomy. Of all the patients who underwent to the radical operation, 1 survived 3 years after the operation, and then was lost to the follow-up, 2 were surviving 19 months after the operation, 2 were surviving 12 months after the operation, 1 was surviving 10 months after the operation, 1 died 18 months after the operation, 7 died between the 2nd and the 12th month after the operation, 1 died in the postoperative period. Of those patients who underwent the palliative operation, none survived longer than one year.

[Alteratiions of calcium and phosphorus metabolism in patients treated with duodenoencephalopancreatectomy].

In a followup study of 14 patients treated by duodeno-cephalo-pancreatectomy at least one year before, the authors detected radiological evidence of bone tissue reshuffling in 64 per cent of the cases. Of these, 70 per cent showed high serum alkaline phosphatase content not attributable to cholestasis, liver metastasis, or specific bone disease. The authors call attention to the significance of this biochemical parameter for diagnostic purposes and therapeutic guidance.

Gastric acid secretion, calcitonin and secondary hyperparathyroidism in uremic patients undergoing regular dialysis therapy (RDT).

Fortyseven uremic patients on RDT underwent a gastric secretion study and a contemporary evaluation of serum levels of Calcium (Ca), Phosphate (iP), Magnesium (Mg), Alkaline Phosphatase (AP), immunoreactive gastrin (Gas), parathyroid hormone (PTH), calcitonin (CT). Secretory test (pentagastrin 6 microgram/kg) was performed in the morning, after 12 hours of fasting, in the interdialytic interval. Female patients, male patients on RDT from less 1 year and hyposecretor patients were excluded from the study. On the basis of these criteria 25 normal or hypersecretor males between 20 and 55 years old were selected. A significant positive correlation was found between PTH and CT, while a negative significant correlation was found between CT and BAO and CT and PAO. Similarly, a significant negative correlation was found between PTH and BAO and PTH and PAO. Multiple regression study showed that the negative influence of CT on BAO and PAO is more relevant than the positive influence of PTH. These data suggest that PTH and CT are involved in gastric acid secretion in uremia. Since the inhibitory effect of CT is prevailing on the stimulating effect of PTH, patients with higher levels of PTH and CT have a lower gastric acid secretion. CT might therefore be considered as a protective factor against hypersecretion in uremia.

Vasodilating properties of a new non-steroidal anti-inflammatory drug, nitroflurbiprofen, on rat aortic rings.

The effects of nitroflurbiprofen (NFP), a new non-steroidal anti-inflammatory drug containing a nitroxybutyl moiety, on rat aortic rings were compared with those of flurbiprofen (FP) and glyceryl trinitrate (GTN). NFP and GTN relaxed, in a dose-dependent manner, either intact or rubbed aortic rings precontracted with epinephrine. Pretreatment with FP did not influence the relaxant activity of NFP in both endothelium-intact and -denuded arteries. In unrubbed preparations, FP did not affect contraction induced by epinephrine, while in rubbed ones a moderate relaxation was observed. Methylene blue and oxyhaemoglobin completely reversed the vasodilating effect of NFP in both rubbed and unrubbed preparations. Moreover, the addition of cysteine 5 mmol l-1 at the end of the cumulative application of NFP resulted in a further relaxation of aortic rings. These results indicate that NFP possesses vasodilatory activity which appears to be dependent on the release of nitric oxide or nitric oxide derivatives.

Effects on intestinal microflora, gastrointestinal tolerability and antiinflammatory efficacy of diclofenac and nitrofenac in adjuvant arthritic rats.

Since it is known that nitric oxide plays an important protective role in maintaining the tissue integrity and is cytotoxic for invasive micro-organisms, diclofenac and a new original diclofenac-derivate, nitrofenac (containing the nitric oxide group), was administered at doses of 0.3 and 3 mg kg-1 per os to adjuvant arthritic rats. At the 14th, 21st and 28th days after arthritis induction, the antiinflammatory efficacy and the effects on intestinal microflora of the two drugs were evaluated; moreover, at the end of the study period, the gastrointestinal tract was examined macroscopically for any presence of lesions. Daily oral administration of diclofenac and nitrofenac at 3 mg kg-1 markedly and significantly inhibited arthritis development until the end of the study period. Some significant changes were observed in anaerobic and Gram-negative bacterial flora, particularly the total disappearance, in all treated rats, of Escherichia coli 1, also 7 days after the last drug administration. Finally, no ulcers or severe damage were observed macroscopically with either drug, even if some alterations in the mucosa and haemorrhagic effusions were more evident in rats treated with diclofenac at 3 mg kg-1. In conclusion, in this chronic model a similar therapeutic efficacy of diclofenac and nitrofenac is shown in arthritic rats. The better gastrointestinal tolerability observed in nitrofenac-treated rats could be attributed to the release of nitric oxide.

[Anomic aphasia in a 5-year-old child following herpetic encephalitis].

Aphasia is a rare neurologic disorder in childhood. Nevertheless some authors believe that some subtle aphasic syndromes as anomic aphasia are not always identified and the real prevalence of aphasia in childhood is higher. We observed a case of aphasia during the acute period of Herpes Simplex Encephalitis in a 5 years old, right handed, boy with a good level of fluent speech. Six weeks after his initial assessment language was spontaneous, fluent, with normal prosodhy and articulation, but reduced, without content words, with frequent circunlocutory speech, semantic paraphasias, anomic latencies, real anomias, "pass partout" words. This picture was suggestive for anomic aphasia in a normally fluent context. Anomic aphasia can be produced by the disconnection between cortical and subcortical systems. Documented cases of anomic aphasia in childhood are rare. Our case present great similarities with adult cases in terms of localisation and denomination difficulties.

CD23 and CD69 expression on human neutrophils of healthy subjects and patients with peripheral arterial occlusive disease.

In this work we studied, on human neutrophils from healthy donors and patients with peripheral arterial occlusive disease, the expression of CD23 and CD69 and the modulatory effects of IFN-gamma, GM-CSF and IL-4. Neutrophils were isolated from 9 patients and 9 healthy subjects and cultured for 24 h in absence or presence of IFN-gamma (1000 U/ml), GM-CSF (10 U/ml) and IL-4 (10 ng/ml). Expression of CD23 and CD69 was analyzed by FACScan cytofluorimeter. Neutrophils of both patients and healthy donors resulted negative for CD23 and CD69 expression immediately after isolation. After 24 h without stimuli, neutrophils from some patients and healthy donors expressed CD23 and CD69. IFN-gamma and GM-CSF had opposite effects on these two antigens, down-regulating CD23 and up-regulating CD69. IFN-gamma, GM-CSF and IL-4 were not able to induce CD23 expression, while CD69 expression was induced in some negative healthy donors and patients by IFN-gamma, GM-CSF and IL-4 respectively. From our data, we identified two subpopulations of neutrophils that, independently from the vascular pathology, showed a different behaviour towards temperature and some cytokines.

[Syncopal fainting episodes and gastroesophageal reflux].

Fainting syncopal events are caused by a transient functional neuronal paralysis. Reflex syncope happens for brainstem involving mediated by peripherical afferents. Sometimes gastroesophageal reflux (GER) has been implicated in the development of obstructive apnea. Gastroesophageal reflux, despite the absence of a clinical history of vomiting and regurgitation, is observed in a significant proportion of infants presenting with ALTE (Apparent Life Threatening Event): an episode characterized by some combination of apnea, color change, marked change in muscle tone, choking or gagging. Though a cause-and-effect relationship between GER and the development of ALTE remains to be established a possible direct relationship between oesophageal acidification and the onset of alterations in cardiopulmonary function and impairment of consciousness can be hypothesized. We refer the case of two female infants that developed recurrent ALTE(s) characterized by paleness, change in muscle tone and loss of consciousness. The infants resulted affected respectively by a mild and severe gastroesophageal reflux (score: 40, > 50); in one case an episode of GER was recorded by the intraoesophageal pH-monitoring during a syncopal episode. The treatment with antiacid drugs was effectual and the infants did not present ALTE(s). The cases presented are in favour of a routine search of gastroesophageal reflux in infants presenting with one or recurrent ALTE(s). The identification of these infants will permitt to develop a correct strategy of treatment.

[Pain in the extremities as a symptom of the onset of Guillain-Barré syndrome in infancy].

Guillain-Barré syndrome's diagnosis is not usually difficult, but diagnostic difficulty occurs for the variable initial presentation. Pain may be predominant as an initial symptom. Pain may be the primary complaint in the week preceding the onset of paralysis in some children. When irritability and withdrawal from child's usual activities is predominant the clinician is first concerned about an encephalopathic process. When the primary problem at presentation is limb and back pain the pathology appears to be in the musculoskeletal rather than in neurological system. The awareness of this presentation of Guillain-Barré syndrome will eliminate delay in diagnosis.
