Observer variability in the diagnosis and management of the hysterosalpingogram.

OBJECTIVE: To determine the reproducibility of hysterosalpingogram (HSG) interpretation and clinical management recommendations among trained observers. DESIGN: Fifty HSG films were distributed to five fertility practitioners with a mean of 20 years clinical experience. Each observer evaluated components of uterine and tubal status and provided clinical recommendations for hysteroscopy and laparoscopy. SETTING: University hospital-affiliated reproductive endocrine practice. INTERVENTION(S): None MAIN OUTCOME MEASURE(s): The level of agreement among observers for each uterine and tubal category as determined by the kappa(kappa) statistic. Determinants of clinical recommendation for further diagnostic studies were assessed. RESULT(S): The level of agreement between observers as determined by kappa ranged from 0.645 in the hydrosalpinx category, indicating fair reliability, to 0.111 for pelvic adhesions, indicating poor reliability. The composite kappa for uterine status was 0.345 whereas the composite kappa for tubal status was 0.430. Agreement among observers concerning management showed marginal reproducibility with a kappa of 0.261. Overall, more than one abnormality of either the cavity or the fallopian tubes led to a diagnostic recommendation for further workup in > or = 90% of cases. CONCLUSION(S): In a group of five experienced clinicians, there was considerable variability in the interpretation as well as the clinical management of the HSG. Physicians caring for infertile couples should be aware of this discrepancy and should, if possible, review carefully both the original films as well as the report of the attending radiologist in formulating their diagnostic evaluation and management plan.

Reduction of an incisional hernia using the open laparoscopic technique.

Recently published cases of operative laparoscopy complicated by incisional hernia reemphasizes the potential hazard of the procedure. A new approach may be an alternative to traditional laparotomy for reducing small bowel herniation through the 12-mm cannula incision. Explorative laparoscopy by the open technique was performed and a loop of small bowel was reduced using an atraumatic grasping forceps. We find this approach safe and convenient.

The management of intrauterine devices following uterine perforation.

The generally accepted indications for removal of an intra-abdominal intrauterine device (IUD) are: a) a closed variety of IUD, b) medicated IUDs with copper or hormone release, and c) a medico-legal or psychological problem. A perforated IUD can be left in the peritoneal cavity if it is made of non-irritating plastic. The copper IUD causes adhesions, but these lead to few cases of serious complications. The reason for removal of the IUD stems only from the fear of adhesion formation. Our experience, supported by the literature, leads us to believe that this indication should no longer be accepted.

The use of Histoacryl for episiotomy repair.

Histoacryl-tissue adhesive (B. Braun Melsungen AG W. Germany) was used in place of skin sutures (2/0 chromic catgut, Ethicon Ltd, Edinburgh, Scotland) for episiotomy repair in a group of 20 women. This group was compared with two groups of women undergoing first and repeat episiotomy. Variables analysed included pain in the episiotomy site, pain while walking, sitting, sleeping, lying down, breast-feeding, micturating and defaecating. The Histoacryl group was superior with regard to all the variables. This simple, new method can reduce pain and inconvenience for patients, especially following the first delivery.

Adherence of Ureaplasma urealyticum to human erythrocytes.

Ureaplasma urealyticum (four serotypes and two clinical isolates) were metabolically labeled with radioactive methionine to a high specific activity. Labeling allowed the study of the mechanism of adherence to human erythrocytes. The adherence mechanism was complex and partially mediated by proteinaceous surface components. The binding sites on the erythrocytes were partially sensitive to neuraminidase treatment, and adherence was inhibited by glycophorin and dextran sulfate, indicating recognition of sialyl residues and sulfated compounds.

Prolongation of the latent period in patients with premature rupture of the membranes by avoiding digital examination.

Fifty three women with PROM between 26 and 34 weeks of gestation were examined. Twenty five women (group A) underwent one digital examination for cervical assessment while in 28 women (group B) the cervix was evaluated by speculum. No pelvic examinations were performed throughout the latent period. The latent period increased from 3.1 days in group A to 9.5 days in group B (P less than 0.005). Chorioamnionitis developed in 24% in group A and in 18% in group B (N.S.).

The management of persistent clear pelvic cysts diagnosed by ultrasonography.

Between December 1981 and June 1986, 52 women examined by ultrasonography were found to have clear pelvic cysts more than 5 cm in average diameter, without septa or solid areas. The cysts were persistent and did not respond to contraceptive pills for four to eight weeks. In all cases, there were histologic or cytologic results from surgical specimens or fluid aspirated from the cysts. In 15 women, the cysts were aspirated and revealed no malignant cells. Thirty-seven women underwent laparotomy, of whom 11 (29%) were found to have benign ovarian neoplasms (nine cystadenomas and two dermoid cysts). Because benign ovarian neoplasms are potentially malignant, and because a large number was found in our study, we recommend removal of the cysts by excision rather than aspiration.

Absence of HLA linkage in familial hypogonadotropic hypogonadism.

In a family with five daughters, three suffered from primary amenorrhea. The diagnosis of familial idiopathic gonadotropin deficiency (FIGD) was established. The entire family underwent HLA evaluation, and no linkage between the HLA and FIGD was found.

A statistical examination of the accuracy of combining femoral length and biparietal diameter as an index of fetal gestational age.

The use of femur length (FL) and biparietal diameter (BPD) for estimation of gestational age (GA) was investigated using 1123 ultrasound measurements between the 14th and 41st week of gestation. Femur length was found to provide a more accurate index than BPD for the entire range of gestational age considered. If an interval of approximately +/- 2.5 weeks is considered 'satisfactory', the accuracy of FL past the 31st week is not sufficient to justify its use. Combining the FL and BPD measurements via the formula: GA = 9.565 + (0.0414) FL + (0.0024573) FL2 + (0.15209) BPD - (0.00015827) BPD2 provides an index of gestational age accurate to approximately +/- 2.5 weeks up to the 32nd week of gestation and accurate to approximately +/- 2.75 weeks up to the 34th week of gestation. This is the first time that combining FL and BPD in a statistically rigorous manner has been proposed as an index of gestational age, and we suggest that this combination be used as a routine tool in its assessment.

Is conservative management of preterm rupture of membranes justified?

The management of preterm rupture of membranes (ROM) is controversial. Some advise immediate delivery and others advocate conservative approach in the hope of decreasing morbidity and mortality rates associated with prematurity. A retrospective review of 75 pregnancies with preterm ROM treated conservatively was performed to delineate the relative risk factors. All patients with preterm ROM were placed at bed rest until either labor or chorioamnionitis prompted delivery. Chorioamnionitis developed in 25% of the cases. The percentage of women developing chorioamnionitis increased with duration of ROM. Fetal morbidity and mortality was not increased in the group with chorioamnionitis. Hyaline membrane disease (HMD) and intra ventricular hemorrhage (IVH) were the major causes of neonatal death in the entire group of preterm ROM. There was a significant relationship between neonatal mortality and birth weight. The smaller the birth weight, the greater the risk of neonatal death. Our data support the conservative approach to preterm ROM especially in the very low birth weight group where extreme caution must be taken before a decision is made to terminate pregnancy.

Corpus luteum function in ectopic pregnancy.

Serum beta HCG, progesterone, and 17 alpha hydroxy progesterone values were determined in cases of ectopic pregnancy. They were found to be significantly lower than in normal intrauterine pregnancies of the same gestational age. No correlation was found between serum beta HCG and 17 hydroxy progesterone values or between serum progesterone and 17 hydroxy progesterone values. Our results tend to support the theory that a normal feto placental unit is needed for the preservation of the function of the corpus luteum.

Postoperative pleural effusion caused by dextran.

Dextran is widely used in order to prevent intraperitoneal adhesions during pelvic surgery. Of 25 women who underwent laparotomy and in whom dextran was instilled intraperitoneally, almost half developed pleural effusions. These were generally mild and required no treatment. None of a control group of 20 women developed a pleural effusion. The mechanism of this type of effusion is discussed.

Better postcoital test for oligospermic patients using split ejaculate artificial insemination.

In 25 oligospermic patients who had negative postcoital tests, artificial inseminations were performed with the whole semen and split ejaculates. After 30 minutes, a sample of cervical mucus was obtained and examined microscopically for the presence of motile spermatozoa. There was a significant improvement of 64% using the split ejaculate, as compared with 44% using whole semen. We found a significantly higher sperm count and progressive motility in the split ejaculate (18 patients, 72%).

Vulval eosinophilic granuloma.

A case of Eosinophilic Granuloma of the vulva, associated with Diabetes Insipidus, is presented. There is neither specific nor adequate treatment for the vulval lesions. Treatment with local steroids should first be tried, if this does not help Roentgen-ray irradiation should be given. Simple vulvectomy is a last resort with poor results.
