Reimbursement issues in advanced practice nursing: an overview.

In this era of rapid change and restructuring of the health care delivery system, APNs are faced with many new opportunities and challenges. This implies that APNs should know the federal and state laws, rules, and regulations that clearly define their scope of practice and prescriptive authority. They should familiarize themselves with Medicare and Medicaid payment policies, and the state laws related to the private insurance practices. By learning the basic language of third-party payers, APNs will find themselves in a better position of understanding the process that ensures reimbursement for the services rendered. Thus, APNs should keep up with the new demand for their expertise by having close ties with new knowledge and new technologies. In addition, APNs should have the skills, credentials, and the knowledge of computer technology for communication, distance learning, diagnosis, and treatment. The ultimate goal is to provide high-quality and cost-effective care.

Demand or ideation? Evidence from the Iranian marital fertility decline.

Is the onset of fertility decline caused by structural socioeconomic changes or by the transmission of new ideas? The decline of marital fertility in Iran provides a quasi-experimental setting for addressing this question. Massive economic growth started in 1955; measurable ideational changes took place in 1967. We argue that the decline is described more precisely by demand theory than by ideation theory. It began around 1959, just after the onset of massive economic growth but well before the ideational changes. It paralleled the rapid growth of participation in primary education, and we found no evidence that the 1967 events had any effect on the decline. More than one-quarter of the decline can be attributed to the reduction in child mortality, a key mechanism of demand theory. Several other findings support this main conclusion.

Bartholin's duct abscess and cyst: a case-control study.

Little is known about the epidemiology of Bartholin's duct abscess and cyst. We did an emergency department-based case-control study to examine demographic risk factors for this condition. We found that risk varied by age. After adjustment for pregnancy status, the relative odds (odds ratio [OR]) of occurrence of Bartholin's duct abscess for women 20 to 29 years old, compared to women younger than 20 years, was 2.3. The association was weaker among women in their 30s and 40s (OR = 1.4 and 1.1, respectively). The majority of patients were Hispanic whites (66% of cases and 79% of control subjects). Risk of Bartholin's duct abscess was greater among non-Hispanic white women (OR = 1.9) and black women (OR = 2.4). At lowest risk of this condition were women with high parity (> or = 4) or high gravidity (> or = 5). These findings suggest that the risk factor profile of this condition is similar to that of most sexually transmitted diseases. The advisability of presumptive antibiotic treatment of these patients and examination and treatment of their sexual partners should be considered.

Routine prothrombin time determination before elective gynecologic operations.

The prothrombin time (PT) test is routinely ordered to identify unsuspected bleeding disorders in patients before elective operations. We evaluated the usefulness of screening PT tests before elective gynecologic operations over a 6-month period at Women's Hospital in Los Angeles, California. Of 1546 patients, 1516 (98.1%) had normal test results. Charts of 25 of the 30 patients with abnormal values were available for review: 20 did not benefit from the test results, and five had indications for coagulation testing on history or physical examination. In the absence of specific indications, routine preoperative PT testing before elective gynecologic operations does not contribute to patient care and should be eliminated.

Parental education and infant mortality.

The effect of parental education on infant mortality in the city of Shiraz, Iran, is analyzed. The data are from a 1980 fertility survey of approximately 600 currently married women. "The data examined...show a high infant mortality rate and suggest considerable differentials by parental education and bio-medical factors such as birth spacing in urban infant mortality."

Socioeconomics of female-headed households in Shiraz, Iran.

"A significant portion of households in urban Iran are headed by females. This study explores the disadvantaged economic position of households headed by women vis-a-vis units headed by men in the city of Shiraz. Sex differentials in socioeconomic characteristics of household heads are discussed and housing facilities for households headed by males and females compared. Finally, an economic typology of female family heads is provided." Data are from a 1980 survey of 2,829 households, 235 of which were headed by females.
