Locoregional IL-2 low dose applications for gastrointestinal tumors.

AIM: To explore the feasibility of local interleukin 2 (IL-2) in patients with different forms of abdominal cancer. This required experimentation with the time interval between IL-2 applications and the methods of application. METHODS: Sixteen patients with stages III and IV of gastrointestinal malignancies (primary or metastatic) who were admitted to our Department of Gastroenterology were treated with locoregionally applied IL-2 in low doses. RESULTS: No major problems applying locoregional IL-2 were encountered. In 6 out of 16 patients, a modest but clinically worthwhile improvement was obtained. Adverse effects were minimal. The therapeutic scheme was well tolerated, even in patients in a poor condition. CONCLUSION: This study demonstrates the feasibility of low dose locoregional IL-2 application in advanced abdominal cancer. Local IL-2 therapy gives only negligible adverse effects. The results suggest that it is important to apply intratumorally. Local IL-2 may be given adjunct to standard therapeutic regimes and does not imply complex surgical interventions. These initial results are encouraging.

Hepatitis C virus infection: a possible promoting agent in porphyria cutanea tarda.

The study aimed to differentiate the factors triggering porphyria cutanea tarda, paying special attention to the presumed role of hepatitis C virus infection. In a representative Bulgarian contingent, HCV-antibodies were identified using ELISA II and immunoblot. Seropositivity was significantly higher (p < 0.01) in the sporadic form (36 out of 57 patients; 63.2%) compared with the familiar form (4 out of 20 subjects; 20%). Alcohol abuse was the most common factor preceding the expression of open porphyria both in the sporadic and the familiar forms. In 10 sporadic cases, no precipitating factors were observed, except for the fact that they were anti-HCV positive. In another 2 anti-HCV positive patients, porphyria cutanea tarda was preceded by blood transfusions. Renewed consumption of alcohol after successful treatment was a common reason for relapse, but relapses were most frequent in anti-HCV positive patients (35 relapses in 12 patients), in whom other promoting factors were absent. Analysis of triggering factors shows that most probably hepatitis C virus infection could contribute to the expression of porphyria cutanea tarda and the association of both diseases is not coincidental.

Successful nonsurgical embryo transfer in buffalo (Bubalus bubalis ) in Bulgaria.

Forty-one Day 5.0 to Day 5.5 embryos and one unfertilized ovum were recovered nonsurgically from 24 superovulated, parous buffalo (Bubalus bubalis ) and transferred nonsurgically to 28 synchronized recipients by a team of Bulgarian and American scientists. Five pregnancies were established and four live buffalo calves were born at the end of normal gestation periods.

[Genealogic studies in genuine partial epilepsy].

Clinical--electroencephalographic (EEG) investigation was performed in 12 families. In the study were included 12 epileptics with partial seizures in whom the katamnestic follow up from 2 to 6 years provided clinical, EEG and genetic data for the so called primary partial epilepsy 24 parents and 14 siblings. In four of the Children with epilepsy the seizures were of the "Rolandic" type; in 5--"psychomotor"; and in 3--hemiconvulsive. No data for structural brain lesion investigation were found. Rapid and long lasting therapeutic effect was achieved in all of the cases. The EEG showed focal paroxysmal activity in all of the diseased and in 11 of the siblings, in 5 of them were found epileptic seizures of the same type as in the probands. The possibilities of a genetic predisposition to development of an epileptogenic focus and its clinical Expression in members of the families in discussed.

Studies on preimplantation development of buffalo embryos.

A total of 71 lactating and nonlactating buffalo-cows of the Murrah breed and F(1)-F(3) crossbreds of Murrah x Bulgarian buffalo were used for a year as donors of embryos after a preliminary treatment for superovulation induction with pregnant mare serum gonadotrophin (PMSG) or follicle stimulating hormone (FSH) in combination with prostaglandin F-2 alpha analog (PGF-2 alpha) according to general application procedures in cows. From 36 to 72 h following prostaglandin injection, the buffalo-cows were checked with the help of a teaser bull for detection of estrus. The animals in estrus were inseminated twice either naturally or artificially with frozen semen. Nonsurgical flushing of the uterine horns was done in 45 of the buffalo-cows between 108 and 162 h after the onset of estrus. After slaughter the uterine horns and oviducts of the other 26 animals were flushed separately between 74 and 108 h after the beginning of estrus. Seven late morulae and eight hatched blastocysts were recovered between 114 and 116 h from the onset of estrus as a result of nonsurgical flushing. All of the 40 embryos recovered after 117 h were in the hatched blastocyst stage. As a result of flushing the oviducts and the uterine horns of slaughtered donors between 74 and 100 h, eggs were obtained only from the oviducts, while flushing conducted between 102 and 108 yielded eggs from both the oviducts and the uterine horns.

Serum cobalt in children with essential hypertension.

The effect of cobalt on the cardiovascular system is one of many aspects of cobalt metabolism in humans. Elastin and collagen are the main proteins of the vascular wall. The aims of this study were: 1) to determine serum cobalt concentrations in children with hypertension; and 2) to study the correlation between serum cobalt and some biological markers of the extracellular matrix of the vascular wall, i.e., anti-elastin and anti-collagen type IV antibodies. Patients showed statistically significant higher levels of systolic and diastolic blood pressure, and significantly lower serum cobalt concentrations, than controls. Children with hypertension showed significantly higher levels of total cholesterol (P = 0.0003) and collagen type IV IgM (P = 0.04). Collagen type IV IgG levels (P = 0.027) were lower than in controls. Serum cobalt in patients showed a correlation with systolic blood pressure (r = -0.44, P = 0.05), elastin IgM (r = 0.60, P = 0.007), and collagen type IV IgG (r = -0.46, P = 0.04). Our data suggest the existence of a correlation between changes in levels of serum cobalt, total cholesterol, anti-collagen type IV antibodies, and essential hypertension in children. This is the first study of serum cobalt in children with essential hypertension.

A case of hepatocellular carcinoma (HCC): treatment with local application of alcohol and interleukin 2 (IL-2).

A case of an inoperable hepatocellular carcinoma due to liver cirrhosis is presented. Surgical treatment was not clinically warranted. So we decided to induce tumor necrosis by intratumoral injections of 10 mL of ethanol followed by two treatments with 9 x 10(6) U Chiron interleukin-2 with an interval of 1 month. This ethanol-interleukin-2 cycle was repeated three times with intervals of 6 months. Interleukin-2 injections were given by a fine needle, under ultrasound control in the periphery and in the center of the tumor. The initial size of the tumor was 55-60 mm. During the follow-up period of 2 years the tumor size remained relatively unchanged. The patient died due to gastric hemorrhage. The treatment elicited no adverse clinical effects. The clinical status improved greatly after this treatment. Local interleukin-2 application after alcohol-induced tumor ablation might be an alternative if surgical treatment is not warranted.

Quantitative assessment of severity of biliary tract infection.

BACKGROUND/AIMS: It is very important for physicians to evaluate the severity of the biliary infection. At the moment, there is no useful quantitative system. In this study, we propose a scoring system for assessing the severity of biliary infections and evaluation of the efficacy of antibacterial and endoscopic treatments. MATERIALS AND METHODS: We created a biliary tract infection score (BTIS) including local physical and ultrasound findings, signs of inflammation and hepatobiliary involvement. The BTIS was calculated in 317 patients: group I-155 pts with cholecystitis and cholangitis, treated only by antibiotics and group II-162 pts with acute cholangitis treated by endoscopic procedures. RESULTS: The BTIS allowed the differentiation of the severity of biliary infections: 15.50 +/- 0.52 in acute cholangitis group and 5.77 +/- 2.79 in group I (p < 0.001). The BTIS significantly decreased after antibacterial therapy (excluding only the cefotiam subgroup) and in endoscopicaly treated patients. CONCLUSIONS: The BTIS is a combination of simple, reliable, acceptable and low cost parameters, reflecting the principal pathological processes and degree of abnormalities. A BTIS facilitated the assessment of severity of biliary infection and comparison of the results of various methods of treatment.
