Four consecutive years (1987-1990) outpatients attendance at a cardiology clinic was recorded in a district hospital with a catchment area of 200,000 inhabitants. The aim being to identify and evaluate the demand for cardiology consultations. A system for recording diagnoses was developed, based on the WONCA. The total number of consultations was 26,273, 27% being new patients, 78% of whom were referred by family doctors and health centers. Of the total, 57% were male. The most common age groups being from 60 to 69 (25%) and from 50 to 59 (23%); 22% were over 69. In 13.7% of the consultations, the find diagnosis was of "healthy hearts", in 16.7% valvular heart disease, in 31.5% ischaemic heart disease, in 15.1% arrhythmias, in 8.4% hypertension, and the remainder were other diseases. We carried out 4,406 echocardiograms, 1,408 exercise tests and 511 Holter monitoring traces. There were 322 direct hospital admissions, and 6,094 discharges in the four years period. We consider this type of records to be useful organizing cardiology outpatients clinics, analyzing the demand, and planning the use of health care resources, as well as estimating future needs and assessing quality of care.
We report the case of a young man with severe pulmonary hypertension caused by multiple thromboembolism subsequent to congenital protein C deficit. During the previous phase to haematological treatment we showed a very poor response to vasodilators evaluated by Doppler-Echocardiography. The etiological diagnosis was confirmed and initialized the anticoagulant treatment. We had observed a significant regression of peak gradient atrioventricular by continuous wave Doppler and a complete clinical normalization.
We report the case of a woman who after taking ampicillin and amoxicillin developed two severe cutaneous reactions with generalized erythema and maculopapular rash, which later turned into blisters with subsequent desquamation. She was clinically diagnosed with erythema multiforme syndrome after positive delayed cutaneous tests with benzylpenicillin, amoxicillin and ampicillin allowed for a more specific diagnosis. There are very few references to patients with drug reactions to betalactams who have positive skin prick or intradermal test but show delayed hypersensitivity, as was the case in our patient.
OBJECTIVE: To describe and assess the clinical usefulness of a new posterior approach for sciatic nerve block to the gluteal zone. PATIENTS AND METHODS: This prospective descriptive study of 40 adults undergoing lower limb surgery measured the difficulty, efficacy and success of the described sciatic nerve block. TECHNIQUE: the patient was placed in Sim's position for a posterior approach. A line was then drawn from the upper part of the iliac spine to the sacral hiatus. From the midpoint of that line, a second line of equal length, perpendicular to the first and medial, was then drawn. The end of that line was the point of puncture. The needle was inserted perpendicular to all planes. Mepivacaine 1.5% (0.3 ml/kg) was injected. RESULTS: The sciatic nerve was located upon the first attempt in 90% of the patients within 21.86 +/- 12.62 s. The block failed in 2.5%. Time taken to establish the block was 19.17 +/- 9.62 m. One transitory, mild complication occurred. No vascular puncture occurred. CONCLUSIONS: We describe a new approach for sciatic nerve block using only two points of osseous reference identified by palpation. The level of efficacy and safety is high.
