A 21 y/o female presented with fulminant hepatic failure and hemolysis. On the basis of the clinical presentation, levels of ceruloplasmin and serum copper a presumptive diagnosis of Wilson's disease was made. In spite of supportive measures and hemodialysis, the patient died one week after admission. Postmortem examination showed cirrhosis and increased copper stores in the liver, corroborating the clinical diagnosis of Wilson's disease. Study of the four siblings revealed that two are carriers, one is healthy and one may have the disease. Wilson's disease is a rare cause of fulminant hepatic failure that must be suspected specially when hemolysis is associated to the clinical picture. This mode of presentation is virtually fatal and early liver transplantation is the best form of therapy.
ELISA has been used for the detection of IgG antibodies against PPD in 48 patients with pulmonary tuberculosis, in 10 individuals with previously treated tuberculosis and clinically recovered, in 22 PPD skin-test positive healthy volunteers and in 48 blood donors with unknown PPD skin-test. Positivity was obtained in 29 patients (83%) with active tuberculosis, in one patient (10%) with clinical recovery, in none of the healthy controls. This technique may have a potential diagnostic relevance in evaluating patients with suspected pulmonary tuberculosis.
The prognostic value of early clinical history, exercise testing and ambulatory electrocardiography was assessed in 263 men (mean age 50 years) recovering from an uncomplicated myocardial infarction (MI). During a mean follow-up period of 31 months, 11 patients died of cardiac causes, 22 developed a non fatal recurrent MI, 16 unstable angina (UA) and 16 underwent coronary artery bypass surgery. The appearance at the exercise stress test of an ischemic S-T segment depression of 0.2 mV or greater (P less than 0.001) as well as the achievement of a work load of 360 Kg-m/m' or less (P less than 0.01) and of a rate-pressure product of 200 Units or less (P less than 0.01), were found to be predictive of the future development of UA, but neither of cardiac death nor of non fatal recurrent MI. The ischemic response was also seen to be predictive of cardiac death (P less than 0.05). S-T segment depression of 0.1 mV or greater, angina and ventricular ectopic activity during the stress test and clinical history were not of predictive value. Complex ventricular ectopic activity (multiform extrasystoles, couplets and ventricular tachycardia) recorded during 24 hour ambulatory electrocardiogram was seen to be predictive of death and non fatal MI. Whereas some parameters such as the ejection fraction and the extension of coronary artery disease are generally accepted as good predictors for cardiac events, others, such as those derived from exercise testing, history and ambulatory electrocardiography may change their predictive value from one survey to another. These discrepancies are due to differences in patient characteristics, in methodology and in medical management.(ABSTRACT TRUNCATED AT 250 WORDS)

In the last years the determination of serum digoxin levels has become absolutely necessary not only in specialized centers but also in all medical departments for its pharmacological and clinical implications. In order to verify the reproducibility, accurateness and sensibility of the techniques currently employed we performed a comparative analysis of the serum digoxin levels obtained by using one immuno-enzymatic and four different radio-immuno-assay kits. The first in vitro trial was performed by evaluating serum digitalis on blood specimens from eight healthy human donors to which known doses of digoxin and beta-methyl-digoxin had been added. The second in vivo trial was performed by giving for three days to sixteen cardiac patients, previously off any digitalis treatment, only one oral morning dose of 0.125 mg of digoxin (8 patients), and 0.135 mg of beta-methyl-digoxin (8 patients). A monoexponential decay constant was calculated on the curves of serum digitalis levels, measured in the third day of treatment by means of the five different techniques. The statistical analysis, performed on the results we obtained, demonstrates a satisfying degree of reproducibility and accurateness only for some of the kits used. This is due to a poor sensibility of some kits in the presence of low digitalis levels, which can produce significantly different value for the decay constant (p < 0.02). Such a behaviour possibly due to different manufacturing processes of the kits raises some questions on the validity in comparing results from various laboratories when different testing procedures are employed.
A new x-ray diffraction procedure for the determination of cristobalite in the presence of amorphous silica is described. The standards are obtained by deposition of thin layers of watery suspensions of cristobalite and amorphous silica mixtures on silver or lead disks or cellulosa filters. Six calibration curves are then calculated to reduce weight and sampling errors. Six mcg of cristobalite mixed with 594 mcg of amorphus silica is the lowest detectable limit. The method may be used for the analysis of both raw materials and respirable dust.

Fibers of amphibolic and serpentinic asbestos may be optically distinguished by the method proposed by the AA. The method is based on the observation at the microscope in reflected fluorescence of fibers of asbestos previously treated with a fluorochrome colouring agent; it can be applied for the individual counting of fibers on the filters used for checking the environmental air and has already been applied with good results in asbestic cement factories.
The application of diffractometric techniques for the identification of crystalline compounds which are contained in paint pigments and are important from the hygienic point of view, is described. Particularly, it is possible to identify lead and zinc chromates either on untreated pigments, or on filters of air samples of work-places used for spray painting.
Analytical investigations were carried out on alteration crusts of "Pietra d'Istria" used for the construction of "Palazzo Papadopoli" in Venice. The chemical and mineralogical analyses of the microstructure showed that alteration process in mainly caused by the transformation of calcium carbonate into calcium sulphate and that the greatest damages are due to the subsequent detaching of sulphatic crusts according to a weakness surface. The isotopic analyses showed that sulphur present in the crust is mainly derived from the combustion of hydrocarbons. The investigations on the climatic conditions showed the meaningful importance of the climate parameters on described alterations.
