To the knowledge of ticks of domestic animals in Afghanistan.

Thirteen species and subspecies of ixodid ticks were found on cattle, zebu, buffaloes, goats, sheep and camels examined mainly at the abattoir of Kabul between September 21 and October 1, 1974. All of them are already known from the territory of Afghanistan.

Trematodes of domestic ruminants of Afghanistan and their role in pathology.

Seven trematode species were found in the bile ducts and rumen of domestic ruminants: Fasciola gigantica, F. hepatica, Gigantocotyle explanatum, G. siemense, Paramphistomum epiclitum, Fischoederius cobboldi and Gastrothylax cruminifer. The parasites caused marked pathological changes in the liver and rumen.

Incidence of toxoplasmosis in domestic animals in Afghanistan.

The authors investigated a total of 435 domestic animals in Afghanistan by micromodification of indirect hemagglutination test. Camels showed the highest incidence of infection (73.7%). Of 9 zebu 3 were positive. The incidence of infection in goats was 31.6%, in sheep 20.4%, in buffaloes 20.4%. The lowest percentages of positive titres was detected in cattle (15.7).

Nematodes parasitizing domestic ruminants in Afghanistan.

A total of 12 nematode species were determined in the collection recovered from domestic ruminants in Afghanistan. Ten of them were found for the first time in this region: Trichocephalus globulosus, Trichostrongylus probolurus, Cooperia bisonis, Nematodirus spathiger, N. abnormalis, Thelazia rhodesi, Bunostomum phlebotomum, Haemonchus contortus, Dictyocaulus filaria and Oesophagostomum radiatum. Morphological and metrical characteristics of T. globulosus, T. skrjabini and C. bisonis, based on our own material, are added.

Cestodes of ruminants in Afghanistan.

Five cestode species parasitizing ruminants were found for the first time in Afghanistan: Moniezia benedeni, M. expansa, Avitellina centripunctata, Stilesia globipunctata, and Thysaniezia giardi.

Cost-effectiveness of family planning and maternal health service delivery strategies in rural Bangladesh.

Two alternative service delivery strategies to improve the effectiveness and efficiency of the Bangladesh national Family Planning and Maternal and Child Health programme have been tested: (1) service delivery at cluster spots, a centrally located neighbourhood spot, rather than at the client's home, and (2) increased frequency of outreach clinics merged with immunization spots. The cost-effectiveness of these strategies was compared with baseline estimates of the cost of providing services. The data were collected in two rural sites of Bangladesh, Mirsarai Thana of Chittagong and Abhoynagar Thana of Jessore, in August 1996. The results of this analysis indicate that cluster service delivery of contraceptive services in their present form are not more cost-effective than home delivery services. The cost per birth averted was lower in only one out of three services in each of the field sites. When the cost-effectiveness of increasing the frequency of SCs combined with EPI services was examined, the service delivery was found to be more cost-effective for all services in one thana and for two out of three services in the higher performing thana, Abhoynagar. This implies that the provision of a wider range of services is improving overall cost-effectiveness.

Nephrocalcinosis, cholelithiasis, and umbilical vein calcification in a premature infant.

A case of calcification of the umbilical vein in a premature infant with nephrocalcinosis and cholelithiasis is described. Such an association might have resulted from prematurity, bronchopulmonary dysplasia, umbilical vein catheterization, total parenteral nutrition, and furosemide therapy. Follow-up ultrasound examination at 9 months of age revealed spontaneous resolution of calcification of both the gallbladder and the umbilical vein but not of the kidneys.

Plasma amino acid patterns in very low birth weight infants during parenteral nutrition.

Plasma amino acids were measured in eight very low birth weight infants (less than or equal to 1000 gm) before and after infusion of parenteral alimentations with Freamine III. Significant elevation in serum threonine, valine, isoleucine, methionine, serine, proline, glycine and ornithine was noted after twenty four hours of infusion. On the other hand, significant decreases in taurine and tyrosine levels were noted. Our study suggests that current solution is not optimal for premature neonates and the amount of protein administered during the first week in infants weighing less than or equal to 1000 gm should be decreased from the recommended 2.5-3.0 gm/kg/day.

Central pontine myelinolysis.

A 9 year old boy presented with fever, drowsiness, quadriparesis and facial myokymia. MRI showed demyelination in the pontine region. A diagnosis of central pontine myelinolysis was made. Literature review revealed the rarity of uneventful recovery as has been seen in our case.

Transcoronary sinus administration of autologous bone marrow in patients with chronic refractory stable angina Phase 1.

PURPOSE: Based on our preclinic studies with autologous unfractionated bone marrow (AUBM) via coronary sinus with transitory occlusion, a clinic study in patients with chronic stable angina was designed. The objectives were to evaluate safety, tolerance and feasibility. METHODS AND MATERIALS: A multicenter prospective study with inclusion and exclusion criteria defined by an Independent Clinical Committee was carried out. Fourteen patients underwent transcoronary sinus administration of freshly aspirated and filtered AUBM (60-120 ml). Safety and tolerance were evaluated. Feasibility was evaluated with Seattle Angina Questionnaire (SAQ), Canadian Cardiovascular Society (CCS) angina classification (baseline-Day 180), myocardial perfusion (baseline-Day 90) with independent core laboratory and coronary angiography (baseline and Day 30). RESULTS: There were no changes in the safety and tolerance parameters. Preliminary clinical efficacy at Day 180 disclosed a significant improvement of 38%, evaluated by the SAQ. The CCS angina classification shows that the mean angina class was 3.0+/-0.55 at baseline and improved to 2.0+/-0.00 at Day 180 (P <.001). Semiquantitative radionuclide perfusion imaging (core lab) showed a significant improvement at Day 90 in 13/14 patients, with a mean improvement of 24% at rest (P <.01) and 33% at stress (P <.05). Coronary angiography showed more collateral vessels in 9/14 patients. CONCLUSIONS: We can conclude that AUBM via coronary sinus with transitory occlusion is tolerable and safe. Significant improvement in the myocardial perfusion at Day 90 and in the quality of life at Day 180 was observed.

Molecular genetic analysis of multi-drug resistance in Indian isolates of Mycobacterium tuberculosis.

A total of 116 isolates from patients attending the out-patient department at the All India Institute of Medical Sciences, New Delhi and the New Delhi Tuberculosis Centre, New Delhi, India were collected. They were analyzed for resistance to drugs prescribed in the treatment for tuberculosis. The drug resistance was initially determined by microbiological techniques. The Bactec 460TB system was employed to determine the type and level of resistance in each isolate. The isolates were further characterized at molecular level. The multi-drug loci corresponding to rpo beta, gyr A, kat G were studied for mutation(s) by the polymerase chain reaction-single strand conformational polymorphism (PCR-SSCP) technique. The SSCP positive samples were sequenced to characterize the mutations in rpo beta, and gyr A loci. While previously reported mutations in the gyr A and rpo beta loci were found to be present, several novel mutations were also scored in the rpo beta locus. Interestingly, analysis of the gyr A locus showed the presence of point mutation(s) that could not be detected by PCR-SSCP. Furthermore, rifampicin resistance was found to be an important marker for checking multi-drug resistance (MDR) in clinical isolates of Mycobacterium tuberculosis. This is the first report on molecular genetic analysis of MDR tuberculosis from India, and highlights the increasing incidence of MDR in the Indian isolates of M. tuberculosis.
