Tamponade-balloon for obstetrical bleeding.

OBJECTIVES: To study the effectiveness of a large volume, fluid-filled tamponade balloon in the management of post-partum hemorrhage originating from the implantation site of low-lying placenta/placenta previa. METHODS: A silicone, fluid-filled balloon was designed for tamponade function, with a filling capacity volume of 500 cc of sterile saline, and strength to withstand a maximum internal and external pressure of 300 mmHg. Five women with postpartum bleeding caused by low-lying placenta/placenta previa, and one woman with cervical ectopic pregnancy, underwent a tamponade balloon insertion as a conservative measure in the management of bleeding. RESULTS: The tamponade balloon was used in five women with post-partum bleeding caused by low-lying placenta/placenta previa, and in one woman with cervical pregnancy. The balloon was effective in controlling post-partum hemorrhage originating from the placental site of the lower uterine segment, and bleeding from the implantation site of cervical ectopic pregnancy. CONCLUSION: Hemostasis in cases of post-partum bleeding caused by low-lying placenta/placenta previa can be achieved by using a large volume, fluid-filled tamponade balloon.

TLM simulation of microwave sintering of ceramics using SiC stimulus.

Microwave heating technology is becoming a successful technique used for sintering ceramic materials. However, various aspects of sintering experiments, such as the use of process stimulus and the preparation of sample arrangements, depend mainly on human expertise. The Transmission Line Matrix (TLM) method is first used to solve the combined electromagnetic and thermal equations modeling microwave heating of dielectric materials. It is then used to simulate microwave sintering of a low-loss ceramic material in a multimode microwave cavity. To enhance the microwave sintering process, Silicon Carbid (SiC) was first used as a susceptor and in a picket fence arrangement. As multiple samples may be processed in a microwave oven, the TLM was used to model such a process, and the introduction of SiC as a stimulus was also examined. Results show the importance of the stimulus thickness and configuration on the uniformity and density of the electromagnetic field distribution and, therefore, on the power dissipation within the ceramic load.

[In vitro androgenesis response of durum wheat disomic substitution].

The variety 'Langdon' and its substitution series were used to evaluate the effect of each substituted chromosome of the A and B genomes on the in vitro androgenetic potential. This study showed the implication of chromosomes 1B and 5B in repressing embryogenesis. Genes located on these chromosomes seem to have an inhibitor effect. The substitution of these chromosomes by their homeologous ones from the D genome increased the number of embryos while with the presence of the original genes the number of embryos was less than in the control. Chromosome 5B is also especially involved in the regeneration of green plants. The genetic control is inhibitory; this explains the difficulty of obtaining good levels of in vitro androgenesis in durum wheat. In this study no effect of the D genome on the androgenetic response of the substitution lines was observed.

[Amniotic band syndrome. Case report].

Amniotic band syndrome is a rare fetal malformation involving limb ring strictions, cranio-facial and trunck abnormalities. The authors report a case of 5 months old infant with also facial cleft. The plastic reconstructive surgery leads to a better functional result. This affection represents a sporadic disease without genetic causes. The pregnancy ultrasonography visualise fetal abnormalities.

[Palpebral capillary hemangioma and amblyopia. Apropos of 2 cases].

Periorbital capillary hemangiomas of childhood can produce visual axis occlusion and anisometropia, resulting in amblyopia. We report our experience performing surgical resection of periorbital capillary hemangiomas. Partial resection of hemangiomas was performed in two infants under five months of age, and permitted clearing of the visual axis. This immediate effect is necessary to prevent amblyopia. Surgical resection should be considered a treatment option for managing periorbital capillary hemangiomas which threaten vision.

Membrane modifications of red blood cells in Alzheimer's disease.

Red blood cells (RBC) from 24 Alzheimer's disease (AD) patients, 18 age- and sex-matched nondemented (ND) patients, hospitalized in the same facility for orthopedic problems, and 18 healthy volunteers aged 30-52 years were studied in order to gain insight into the nature of RBC membrane modifications in AD. Significant differences were found between RBC from AD and ND patients or young controls respectively for annexin V-binding (45.5 +/- 18.0% vs 27.1 +/- 14.7 and 2.7 +/- 1.9, p = .003), fraction of glycerol resistant cells (30.8 +/- 11.1% vs 19.6 +/- 6.4 and 10.2 +/- 3.1, p = .026), cell electrophoretic mobility in polymer (1.028 +/- 0.022 microns sec-1 V-1 cm vs 1.046 +/- 0.022 and 1.053 +/- 0.021, p = .02) and only limited significance for the filterability (1.46 +/- 0.12 msec vs 1.58 +/- 0.11 and 1.54 +/- 0.11, p = 0.1). A logistic analysis, using simultaneously several features as independent variables, suggested the combined use of annexinV- binding, glycerol resistance, and cell filterability which allowed the assignment of 95% of patients from this cohort to the right group. A prospective analysis of a larger cohort is required for the estimation of the diagnostic value of this test battery. In addition, the high level of annexin binding is characteristic of a disruption of the phospholipid asymmetry in aged or damaged cells, while the high glycerol resistance combined with low electrophoretic mobility an rigidity characterize young RBC, thus indicating an enhanced turnover of RBC in Alzheimer's disease.

Pregnancy complicated by malaria, precipitate labor and uterine rupture.

A case of Plasmodium falciparum malaria is reported in a 25-year-old pregnant woman with a history of three previous cesarean sections. She developed premature precipitate labor which was complicated by stillbirth, uterine rupture, bladder and vaginal tears necessitating hysterectomy.

Gestational choriocarcinoma in a tubal ectopic pregnancy.

A case is reported of a gestational choriocarcinoma arising in a tubal ectopic pregnancy of 7 weeks' gestation. Unilateral salpingectomy and single-agent actinomycin-D chemotherapy achieved a cure. A follow-up 4 years later revealed no evidence of recurrence, with two normal births within that period. The current trend to treat ectopic pregnancy by conservative surgery necessitates monitoring of beta-HCG titres in order to avoid missing a choriocarcinoma developing in an ectopic gestation, even though this is a very rare condition.

Pregnancy complicating irradiation-induced constrictive pericarditis.

A case is reported of a 24-year-old primigravida who had severe effusive constrictive pericarditis secondary to mediastinal irradiation following chemotherapy for Hodgkin's disease. Pregnancy was threatened by serious maternal cardiovascular complications, and a non-viable fetus was born spontaneously and prematurely. Patient was completely asymptomatic before pregnancy.
