Insights into extreme thermoacidophily based on genome analysis of Picrophilus torridus and other thermoacidophilic archaea.

Thermoacidophiles are prokaryotic microorganisms with the stunning capability to survive and multiply at extremely low pH and simultaneously at high temperatures. The mechanisms by which these organisms, exclusively members of the Archaea, cope with their harsh surroundings are poorly understood. The genome sequences of several representatives of the thermoacidophilic genera Picrophilus, Thermoplasma and Sulfolobus have recently become available. Genome-wide comparison has revealed a number of features as possible facets of the overall acidophilic survival strategy of the most thermoacidophilic organisms known, such as a high ratio of secondary over primary transport systems, the composition of the respiratory chain, and the frequent genetic input via lateral gene transfer (LGT) during evolution.

Genome sequence of Picrophilus torridus and its implications for life around pH 0.

The euryarchaea Picrophilus torridus and Picrophilus oshimae are able to grow around pH 0 at up to 65 degrees C, thus they represent the most thermoacidophilic organisms known. Several features that may contribute to the thermoacidophilic survival strategy of P. torridus were deduced from analysis of its 1.55-megabase genome. P. torridus has the smallest genome among nonparasitic aerobic microorganisms growing on organic substrates and simultaneously the highest coding density among thermoacidophiles. An exceptionally high ratio of secondary over ATP-consuming primary transport systems demonstrates that the high proton concentration in the surrounding medium is extensively used for transport processes. Certain genes that may be particularly supportive for the extreme lifestyle of P. torridus appear to have been internalized into the genome of the Picrophilus lineage by horizontal gene transfer from crenarchaea and bacteria. Finally, it is noteworthy that the thermoacidophiles from phylogenetically distant branches of the Archaea apparently share an unexpectedly large pool of genes.

Lactic acid bacteria and yeasts in kefir grains and kefir made from them.

In an investigation of the changes in the microflora along the pathway: kefir grains (A)-->kefir made from kefir grains (B)-->kefir made from kefir as inoculum (C), the following species of lactic acid bacteria (83-90%) of the microbial count in the grains) were identified: Lactococcus lactis subsp. lactis, Streptococcus thermophilus, Lactobacillus delbrueckii subsp. bulgaricus, Lactobacillus helveticus, Lactobacillus casei subsp. pseudoplantarum and Lactobacillus brevis. Yeasts (10-17%) identified were Kluyveromyces marxianus var. lactis, Saccharomyces cerevisiae, Candida inconspicua and Candida maris. In the microbial population of kefir grains and kefir made from them the homofermentative lactic streptococci (52-65% and 79-86%, respectively) predominated. Within the group of lactobacilli, the homofermentative thermophilic species L. delbrueckii subsp. bulgaricus and L. helveticus (70-87% of the isolated bacilli) predominated. Along the pathway A-->B-->C, the streptococcal proportion in the total kefir microflora increased by 26-30% whereas the lactobacilli decreased by 13-23%. K. marxianus var. lactis was permanently present in kefir grains and kefirs, whereas the dominant lactose-negative yeast in the total yeast flora of the kefir grains dramatically decreased in kefir C.

Yeast strains from Livingston Island, Antarctica.

Five yeast strains were isolated from soil and moss samples from the Livingston Island (Antarctica) and identified according to morphological cultural and physiological characteristics. All strains had an optimum growth temperature of 15 degrees C: none grew above 25 degrees C. They assimilated D-glucose, D-galactose, sucrose, cellobiose, trehalose, 2-keto-D-gluconate, D-xylose, D-ribose and melezitose. Four of them were nonfermentative, only one, which formed pseudomycelium fermented glucose, galactose, trehalose. Two strains were identified as pink-red yeasts belonging to genus Rhodotorula--R. minuta and R. mucilaginosa; two were related to the genus Cryptococcus--C. albidus and C. laurentii; one was Candida oleophila.

[Iatrogenic lesions of hepatocholedocus in cholecystectomy].

This is a report on nine patients presenting different intraoperative lesions to hepatocholedochus, some of them diagnosed in the course of operation, and others- after the intervention. A variety of plastic repairs of the hepatocholedochus, including simple suture, Saple's operation or hepatodigestive anastomosis after Longmayre are performed. The probability of inflicting lesions, the underlying causes and the therapeutic approach to concrete cases are analyzed. Abiding to the principles of surgery is strongly recommended, such as: sufficient and appropriate operative access, atraumatic manipulations, drainage of the gallbladder bed and the like, with a view to perclude serious and sometimes fatal complications' occurrence, as well as making early diagnosis and accordingly taking the most opportune surgical solution to the problem faced.

[Francke's operation--a resolution for deep small echinococcal cysts of the liver].

The operation of Francke in hydatid disease treatment is described--puncture, aspiration, sterilization--with emphasis laid on the technical details. The indications for its practical implementation are specified--small echinococcus cysts, deeply located within the parenchyma of the organ, or diffuse abdominal echinococcosis. In diagnosing such cysts intraoperative ultrasonography is considered as absolutely necessary and mandatory. The operation is practicable in handling first-stage cysts according to Volynkin, containing thin echinococcus liquid only, whereas in the presence of daughter vesicles it is not indicated. Postoperatively, dispensary outpatient observation and chemotherapy of the hydatid disease are recommended with a view to promote radical cure.

[Perforations of hepatic hydatid cysts].

Out of a total of 753 patients operated for liver echinococcosis, in 43 (5.71 per cent) various perforations are observed, namely: in the biliary apparatus, chest, abdominal cavity, gastrointestinal tract, subphrenic and subcutaneous tissue. Their incidence in bile ducts is the highest. The lethality is rather elevated with two patients dying on the operating table. In conclusion, proceeding from the severity of the complication emphasis is laid on the necessity of early diagnosis of the disease, before development of serious complications, successfully achieved in the last few years.

Radioimmunological and immunohistochemical study of carcinoembryonic antigen in pleomorphic adenoma and mucoepidermoid carcinoma of the salivary glands.

The aim of the present study was the evaluation of CEA-radio-immunoassay and CEA-immunohistochemistry in the management of pleomorphic adenoma and mucoepidermoid carcinoma of salivary glands. 23 pleomorphic adenomas, 9 mucoepidermoid carcinomas, and 7 normal salivary glands were examined. CEA-concentration in serum and saliva were assayed before and after surgery. Polyclonal CEA antibody was used for immunohistochemical CEA detection in the tumor tissue and in the normal salivary glands. The mean CEA concentrations were found to be 14.94 ng/ml in the serum and 216.67 ng/ml in the saliva of patients with mucoepidermoid carcinoma. These values were considerably higher compared to healthy controls (188.64 ng/ml in saliva) and in patients with pleomorphic adenoma - 7.65 ng/ml in serum and 189.35 ng/ml in saliva (P < 0.001). A correlation was found between high CEA concentration in the saliva and the intensity of CEA expression in the tumour tissue. An increased synthesis and secretion of CEA was determined by the prevalence of tubular structures, a high proliferative activity in pleomorphic adenoma, and its malignant transformation.

[Nonparasitic liver abscesses].

Abscess of the liver is a comparatively rate, but very serious disease, necessitating the undertaking of diagnostic and therapeutic measures on an emergency basis. The principal diagnostic method currently in use includes two imaging techniques--echography and CT. In therapeutic respect the target puncture under echographic or CT control has rather limited scope of application. Surgical therapeutic approach is the leading one--laparotomy, drainage of the abscess, and arbitrary use of a Kerr drainage for the choledochus and regional antibiotic therapy.

[The ultrastructural changes in type-2 pneumocytes in experimental heat and endotoxic shock].

The early ultrastructural changes in type 2 pneumocytes (PN2) as a result of burn and endotoxic shock in rats and rabbits were studied. Stereotype morphological damages in PN2 were established. Differences between experimental models were quantitative and depend mainly from the severity of shock and animals species. Morphofunctional heterogenity of PN2 population leads to the appearance of three groups of PN2: 1) PN2 with degenerative changes; 2) PN2 with "stress" hyperactivity and exocytosis of lamelar bodies; and 3) PN2 with increased number of lamelar bodies. Disturbances of lamelar bodies morphogenesis (conglomerates, giant lamelar bodies) as well as lamelar bodies formation by alternative way - directly from rough endoplasmic reticulum were observed. The predominant part of the described PN2 lesins were adaptive in character.

Intracranial venous thrombosis in relation to pregnancy and delivery.

160 women who had died during pregnancy or after delivery were studied. Intracranial venous thrombosis (ICVT) was found in 10 (6.25%) cases. Most often extensive dural sinus phlebothrombosis and rarely isolated meningeal or cerebral phlebothrombosis were observed. Multiple thrombi in cerebral microcirculation presented the most characteristic morphological feature in all cases. Disseminated intravascular coagulation (DIC) with microthrombi in two or more parenchymal organs was found in 8 cases. Delayed clinical onset and prolonged progressive course was most frequently observed. Persistent headache, haemiparesis or haemiplegia, convulsions, epileptic seizures, disturbances in consciousness and coma occurred most often. These clinical manifestations were due to multiple haemorrhagic or ischaemic cerebral infarctions and in a single case to haemorrhage. The discussion was centered on the assumption that ICVT in pregnant and parturient women might be a distinct clinicoanatomical form of DIC.

[Intravascular coagulation in amniotic fluid embolism].

Amniotic embolism (AE) was established in 13 (8.13% out of 160 dead pregnant women and parturients. The morphological examination and clinico-anatomical analysis showed that the degree of obstruction of lung microcirculatory bed was of substantial significance for the course of the disease. Intravascular coagulation (IC) was found in 11 (84.6%) of women with AE. Lungs were damaged by microthrombi most frequently and most severely, which was connected with direct contact activation of blood clotting system by the amniotic fluid. Characteristic peculiarities of AE were their jerk-like course with subclinical period of various duration and early secondary activation of the fibrinolytic system. The morphological sign of the latter was the presence of hyaline globules even within the first hour since the clinical onset of AE. IC and the connected with it acute respiratory insufficiency and consumption coagulopathy were the main thanatogenic factors in AE. This imposed dynamic follow-up of coagulation status and fibrinolytic activity combined with timely anticoagulant and antifibrinolytic treatment.

[Surgical procedures in duodenal ulcer hemorrhage].

Analysis was made of 893 patients operated for duodenal ulcer. Of these, 872 (97.6 per cent) were operated as nonemergency planned patients; the outcome was fatal in 5 (0.57 per cent). Emergency operative intervention at the acme of the bleeding was performed in 21 patients (2.4 per cent) because of failure to control the bleeding by conservative means. Six of these patients died (24.0 per cent) during the early postoperative period. Highest was the percentage of patients in whom extended gastric resection was made; there were no lethal cases among patients subjected to antrumectomy and truncus resection. Preference is given to truncus vagotomy with sparing resection of the stomach; this is considered a reliable and safe method for stopping the bleeding and curing the patients from the ulcer.

Intravascular coagulation in relation to pregnancy and delivery.

Incidence, risk factors and morphological features of the intravascular coagulation (IC) in 160 women who had died during pregnancy, after abortion and delivery were studied. IC was established in 118 (73.8%) of them. The main risk factors leading to IC were shock (59.3%), sepsis (28.8%), toxemia of pregnancy (incl. eclampsia) (25.4%), Caesarean section (19.5%), fetal death in utero (12.7%), amniotic fluid embolism (9.3%), and abruptio placentae (7.6%). Disseminated intravascular coagulation (DIC) was established in 66% of the cases, and local intravascular coagulation (univisceral localisation of microthrombi) in 28%. In the resting 6% of the cases there was consumptive coagulopathy without microthrombi. Lungs, pituitary gland, uterus, kidneys and adrenals were the most frequently affected organs. Necrosis in the parenchymal organs, hyaline membrane formation in the lungs and consumptive coagulopathy were particularly frequent in the cases with DIC. The leading causes of death were acute renal failure and ARDS. It was established that prolonged intensive care including artificial ventilation, massive blood transfusion, as well as surgical treatment, aggravate the course and morphological features of IC.
