Pulmonary complications in heart transplant recipients.

BACKGROUND: Heart transplantation is an important therapeutic option for patients with end-stage disease, but is associated with major pulmonary complications. PATIENTS AND METHODS: We retrospectively reviewed the posttransplant follow-up of 34 orthotopic heart transplant recipients. RESULTS: Two of the 34 patients died of cardiac failure in the early postoperative period. Among the surviving patients, 10 (31.3%) developed pulmonary complications, all within the first 6 months: hospital-acquired bacterial pneumonia in five, fungal pneumonia in three, posttransplant lymphoproliferative disease in one, and community-acquired pneumonia in one patient. None of the patients developed transplantation-related malignancy. The overall mortality was 35.3%. Pneumonia-related mortality rate of 14.7% was due to early-onset nosocomial pneumonias, which were caused by bacterial and opportunistic microorganisms. Extrapulmonary causes of mortality were cardiac failure, meningitis, septicemia, and acute rejection. Cytomegalovirus antigenemia in the first month was associated with a poor prognosis. The frequency of pulmonary complications was higher among older patients and those who developed moderate rejection in the first month (P=.014 and P=.036, respectively). CONCLUSION: Pulmonary infections after heart transplantation occurred more frequently during the first 6 months posttransplantation, accounting for a significant portion of the posttransplantation mortality.

Experimental Aspergillus fumigatus infection in quails and results of treatment with itraconazole.

This study was performed to investigate (i). the clinical, histopathological and biochemical changes in quails (Coturnix coturnix japonica) with experimentally induced aspergillosis; and (ii). the efficiency of itraconazole treatment on these infected birds. A total of 18021-day-old male quails was randomly divided into three groups (control, infected untreated and infected treated), each containing 60. The experimental infection was set by intratracheal inoculation of 0.2 ml inoculum of Aspergillus fumigatus (CBS 113.26 strain) consisting of approximately 2.7 x 106 spores/ml. Two days after the inoculation, general clinical signs of aspergillosis in the respiratory tract were observed. In the histopathological examination, caseous foci were found in lungs, trachea and on airsacs. All quails died in the infected untreated group. Aspergillus fumigatus was isolated from the various organs of all dead quails. There was no significant change in serum aspartate aminotransferase (AST) and gamma-glutamyltransferase (GGT) activities in infected untreated birds compared with controls. However, alanine aminotransferase (ALT) activity, albumin and calcium levels, and albumin/globulin (A/G) ratio were lower while phosphorus and globulin levels were higher in the infected untreated group than in controls. Each quail in the infected treated group was given 10 mg/kg/day itraconazole via drinking water for 7 days immediately after the first clinical findings. Although all quails died in the infected untreated group, 41 quails survived in the itraconazole treatment group. Biochemical values also returned approximately to the control levels after the treatment. The conclusion was drawn that aspergillosis in the quails might cause economical losses because of high mortality. Oral itraconazole treatment of aspergillosis might lower the mortality rate in quails.

Pathological, clinical and mycological findings in experimental aspergillosis infections of starlings.

A group of 24 wild starlings (Sturnus vulgaris), in undefined age categories but at least post-pubertal, constituted the material of this study. Six starlings were kept as a control group and 18 starlings served as the infection group. The starlings in the infection group were infected with inoculums of 1.35 x 10(6)/0.2 ml Aspergillus fumigatus via intratracheal route whereas the control group was administered only placebo in the same way. Six, four, two, four and two birds died on 2, 3, 4, 5 and 6 days post inoculation respectively. At the necropsy of the dead birds, caseous foci were determined in the lungs, on the air sacks, myocardium, thoracic wall and abdominal serosa. In the histopathological examination of the white-yellowish caseous foci ranging from pinhead to chick pean in size, necrotic granulomatous foci consisting of macrophages, heterophil leukocytes and gigant cells encapsulated with a fibrous tissue were observed. Hyphae and spores of A. fumigatus were determined in these foci using the Gridley staining method.

Endobronchial metastasis from osteosarcoma of bone: treatment with intraluminal radiotherapy.

Lung parenchymal metastases are common manifestations in patients with osteosarcoma; however, spread to the major airway itself is extremely rare. We present a young man who had been previously treated with surgical resection following preoperative chemotherapy and immediate postsurgical adjuvant chemotherapy for proximal tibial osteosarcoma. He developed metastasis to the major airways. The patient was treated with intraluminal radiotherapy (ILT) for the endobronchial metastasis. This is the first report of an endobronchial osteosarcoma that was treated with ILT with a complete endoscopic response. ILT provided excellent palliation in this particular case.

Clinical and electron microscope comparison of silk sutures and n-butyl-2-cyanoacrylate in human mucosa.

The use of tissue adhesives as an alternative to or replacement for sutures in wound closure has long been an area of interest. One of these tissue adhesives is a cyanoacrylate. In 15 patients, who underwent root resections of the upper incisors on both sides, the incision lines were closed with silk sutures on one side and by n-butyl-2-cyanoacrylate on the other side of the frenum. Clinical comparison was made on the 1st, 2nd, 3rd, 7th, 14th and 21st postoperative days. On the seventh postoperative day following the removal of sutures and the coating, small punch biopsies were obtained from n-butyl-2-cyanoacrylate treated and sutured sides. The tissue specimens were examined under transmission electron microscope. Clinical observations revealed that on the third and seventh postoperative days epithelialization was better on the sides treated with n-butyl-2-cyanoacrylate. On the twenty-first postoperative day it was observed that the scar formation was significantly more marked and there was more local inflammation during the healing period on the sutured side. Electron microscopic observations of both tissue specimens revealed normal ultrastructural morphology.

An unusual insertion of accessory biceps brachii muscle.

In the course of a routine dissection of the anterior region of the arm an accessory muscle was observed in the flexor aspect of the right arm of a 64-year-old male cadaver. This accessory muscle, which was considered to be the third head of the biceps brachii muscle originated from the inserting tendon of the deltoid muscle. The muscle gave rise to three tendons as it approached the cubital fossa. Two of these tendons inserted to the bicipital aponeurosis whereas the third tendon inserted to the ulnar head of the pronator teres muscle. Dissection of the forearm on both sides revealed that the palmaris longus muscles was absent bilaterally. Mechanical effects of the insertion tendons of the accessory head of the biceps brachii on the median nerve, especially during the supination of the forearm, are discussed. For radiodiagnostic procedures, MRI images of the accessory head were obtained in the coronal and axial planes.

Comparison of silk sutures and n-butyl-2-cyanoacrylate on the healing of skin wounds. A pilot study.

The synthetic tissue adhesive n-butyl-2-cyanoacrylate (Histoacryl Blue) and silk sutures were compared on the backs of guinea-pigs as the means for closing skin incisions. On the post-operative 3rd, 7th, 14th and 21st days, specimens from each group were obtained and evaluated under light microscopy. Sections were analysed for the degree of inflammation, giant cell reaction, fibroblastic and capillary cell activity and were ranked statistically. The results showed that the silk sutures caused severe inflammatory and giant cell reaction when compared with Histoacryl and the healing process was slower than with n-butyl-2-cyanoacrylate.

Duplication of the jejunum in a dog.

Duplication of the jejunum in a dog was morphologically reported. A solitary lesion with a diameter of 4 mm was incidentally found near the mesenteric side of the intestinal wall. On the intestinal mucosa, a crater-like focus was observed. Histologically the nodular lesion was detected as a round and clearly circumscribed canal within the normal muscular layer of the small intestine. The inner surface of the canal had mucosa which was similar to the normal intestinal mucosa with villi in light and electron microscopy.

Morphological assessment in pinealectomy and foetal pineal gland transplantation in rats: Part I.

The effects of pineal gland, an endocrine organ known to affect the physiology of various organs and systems including the hypothalamo-hypophyseal axis, on the morphological characteristics of target organs were investigated in rats after pinealectomy and foetal pineal gland transplantation to a subpial cortical area close to the pinealectomized region. It was demonstrated that weight gain was significantly slower in pinealectomized male rats (p < 0.01); pituitary gland weight was lower in the pinealectomized group (p < 0.01) and transplantation had no effect on the weight of this organ; weight of adrenal gland and testis were lower after pinealectomy (p < 0.01) but restored back to control levels after transplantation; the mitotic activity in seminiferous tubules increased with pinealectomy and returned back to control indexes after transplantation. Research on hypothalamic catecholaminergic content revealed a diminished histofluorescence in pinealectomized rats which showed a potentiated restoration after transplantation. The innervation of the pineal gland was studied by anterograde and retrograde injections of Wheat Germ Agglutinin Horse Radish Peroxidase from superior cervical ganglion and pineal gland, respectively. It was demonstrated that cell bodies were traced both at the transplanted tissue and hypothalamus. In view of these data, the multifunctional and a probable high level homeostatic harmony regulator essence of pineal gland is discussed.

Labelling of neurons in the rat superior cervical ganglion after injection of wheat-germ agglutinin-horseradish peroxidase into the contralateral ganglion: evidence of transneuronal labelling.

Recent studies have shown that injection of the tracer wheat-germ agglutinin-horseradish peroxidase (WGA-HRP) into the superior cervical ganglion (SCG) of one side results in labelling of neurons in the contralateral SCG and the stellate ganglion. This study was designed to verify whether or not bilateral projections from the superior cervical ganglion to the midline structures, particularly to the pineal gland, play a role in the transport of WGA-HRP to the contralateral SCG. One group of rats received WGA-HRP injection into the right SCG (group I). Four groups of rats underwent the following operations prior to the injection of WGA-HRP into the right superior cervical ganglion: transection of the external carotid nerve (group II), transection of the internal carotid nerve (group III), transection of the external carotid nerve combined with pinealectomy (group IV), transection of both the internal and the external carotid nerves (group V). The mean number of labelled neurons in the left SCG of each group were found as follows: group I, 1516 +/- 221 (mean +/- S.D.); group II, 861 +/- 122; group III, 543 +/- 99; group IV, 562 +/- 144; group V, 220 +/- 52. The results of this study suggest that the contralateral labelling depends on the transneuronal transport of WGA-HRP through the terminal fields of innervation of the midline structures that receive bilateral projections from both SCGs.

Lipid peroxide level increase in experimental hydrocephalus.

The aetiology of incomplete adrenergic denervation and reduction in the number and caliber of the cerebral vessels in hydrocephalus is still obscure. Stretching of the blood vessels alone is far from explaining these major vascular changes. Previous studies have shown that increased lipid peroxidation produces toxic effects on vessels. This experimental study was designed to investigate the possible aetiology of vascular changes in hydrocephalic rats with special reference to lipid peroxidation. Hydrocephalus was induced by injecting 50 mg/Kg sterilized kaolin suspension into the cisterna magna in 10 rats (Group A). A sham operation was performed for Group B. After three weeks the rats were anaesthetized and perfused transcardially. The brains were dissected, and cut to visualize the degree of hydrocephalus. The arteries of the circle of Willis were removed for light microscopic examination and the brains were kept for the measurement of lipid peroxidation levels. Light microscopic studies of cerebral arteries in hydrocephalic rats revealed spastic vessels with folding and corrugation of the lamina elastica. The level of lipid peroxidation in group A (260 +/- 9.129 nmol TBAR/gr wet tissue) was significantly higher than that of group B (106 +/- 3.59 nmol TBAR/gr wet tissue). It is suggested that vascular changes observed in hydrocephalic rats may be due to the high level of lipid peroxidation, which in turn may be the consequence of ischaemia caused by the hydrocephalus related stretching of cerebral vessels.

Arteria thyroidea ima arising from the brachiocephalic trunk with bilateral absence of inferior thyroid arteries: a case report.

The anomalous first parts of the left and right subclavian aa. had no inferior thyroid aa. in the neck region. The thyroidea ima a. was found to arise from the brachiocephalic trunk, and bifurcated into two branches almost immediately after its origin. These branches ascended in front of the trachea and entered the bases of the right and left lobes of the thyroid gland. The left vertebral a. arose from the aortic arch in the superior mediastinum. The possible existence of this anomaly is important for parathyroid localization studies, in neck surgery and especially in tracheostomy.

Evaluation by angiography of the lateral dominance of the drainage of the dural venous sinuses.

Venous drainage dominance of the dural venous sinuses may be defined as the drainage only or mainly into one of the transverse sinuses, as shown by bilateral carotid angiography. The aim of this study was to evaluate the venous drainage dominance in bilateral carotid angiograms of 189 cases retrospectively. Among these cases 41.3% showed drainage mainly to the right side, 37.6% showed equal drainage to each side, 18.5% showed drainage mainly to the left side, 2.1% showed drainage only to the right side and 0.53% showed drainage only to the left side. Cerebral venous drainage dominance is of great importance and should be considered before operations on patients for radical neck dissection, removal of tumors in the neck that invade the internal jugular vein or tumors of the glomus jugulare which may require ligation of the internal jugular vein.

Somatotopic organization of the axons innervating the superior rectus muscle in the oculomotor nerve of the rat.

It has long been known that the pupillomotor fibers have a localized course in the dorsomedial periphery through the subarachnoid portion of the oculomotor nerve. However we do not have detailed knowledge about the organization of the fibers innervating individual extraocular muscles. This subject is of clinical importance for locating the site of lesions in patients with partial oculomotor nerve palsies. In this study the peripheral course of the axons innervating the superior rectus muscle were studied in the subarachnoid portion of the oculomotor nerve of the rat by making multiple injections of horseradish peroxidase solution into the muscle and reacting the nerve with tetramethylbenzidine as a chromogen. Coronal sections of the nerve showed that the axons innervating the superior rectus muscle were localized in the dorsal part of the nerve. Within the precavernous portion these axons showed a wider distribution invading dorsomedial and dorsolateral parts of the nerve.

An unusual insertion of the palmaris longus muscle.

In the course of an anatomical dissection at the School of Medicine, a variant form of the palmaris longus muscle was found on the right side; the fellow muscle of the left side being normal. The palmaris longus concerned was found to arise by fleshy fibers from the common tendon for the flexores antebrachii muscles to be divided into a lateral and a medial tendon. The lateral tendon of the muscle was inserted onto the thenar fascia and the scaphoid bone while the medial tendon was being inserted onto the hypothenar fascia and the triquentral bone.
