[Cerebral hydatid cysts in children: 4 cases].

The cerebral localization of the hydatid disease is rare, under 2%, and it primarily affects children. We report 4 cases of children presenting with a cerebral hydatid cyst (2 boys and 2 girls, mean age 9 years). Clinical symptoms were very progressive, the disease was frequently diagnosed several months (1-12 months) after onset of symptoms most often headaches and vomiting. One patient presented with a right tonic-clonic seizure 3 days before hospitalization. The diagnostic was confirmed in all cases by Cerebral CT scan. All the patients were screened for other localizations. One patient also presented with pulmonary and hepatic hydatid cysts. Hydatid blood tests were positive in only one case. The treatment was surgical for all the patients (using hydropulsion) without complications in 2 cases. One child presented with meningitis, and the other child with signs of secondary ICHT related to residual mass which required its puncture.

[Candiduria in intensive care unit: significance and value of yeast numeration in urine].

JUSTIFICATION: Candiduria is increasingly frequent among patients admitted to intensive care units but its significance remains unclear. OBJECTIVES: Search for eventual correlation between quantitative candiduria and known risk factors for invasive candidiasis. STUDY DESIGN: Prospective. PATIENTS AND METHODS: A four-month study was conducted in 162 patients hospitalized in the intensive care unit for more than 72 hours. All patients underwent a weekly research of candiduria added to sampling from different body sites to determine the Pittet Candida colonization index. RESULTS: Candiduria has been proved in 56 cases (34%). It was superior or equal to 10(4) UFC/ml among 28 patients (50%). Candida tropicalis, Candida glabrata and Candida albicans has been isolated in 41, 22 and 20% respectively. All patients had at least one major and two minor risk factors for Candida infection. Six patients (10%) developed invasive candidiasis. The global mortality rate was at 52%. Pittet colonization index was significantly different between patients with candiduria and those with invasive candidiasis (p=0.01). There was a statistically significant correlation between candiduria superior or equal to 10(4) UFC/ml and Pittet colonization index superior or equal to 0.5 (p=0.01). CONCLUSION: Candiduria superior or equal to 10(4) UFC/ml associated with risk factors may predict invasive candidiasis in critically ill patients.

Selective mineral elements concentration of the intestinal mucosa role of the lysosomes of duodenal enterocytes in the handling of mineral elements after intragastric administration.

Intragastric administration to rats of four soluble lanthanides cerium, lanthanum, europium, thulium and of three soluble elements of group IIIA aluminium, indium and gallium has been shown in previous studies. In this work two new rare earths gadolinium and terbium were studied using the same protocols and the same methods (transmission electron microscopy and ion microanalysis). among the previously studied elements, some of them were administered simultaneously on the one hand aluminium and indium, and on the other hand, lanthanum and cerium. These metals were looked for in intestinal mucosa, liver and kidney. The results showed: a) gadolinium and terbium were selectively concentrated in lysosomes of duodenal enterocytes, precipitated as non-soluble phosphate salts and eliminated with the cell's turn-over in less than 48 hr; b) Administered simultaneously, they precipitated in the same lysosome. c/ none of them was observed in the liver or kidney even with high dose. This study brings up to nine the number of elements forming a non-soluble phosphate salts, explaining their precipitation in lysosomes. None of them have a physiological role, two are toxic (aluminium and indium). This rapid intralysosomal concentration is an efficient mechanism which limits the diffusion of the foreign substances through the digestive barrier, then permits their elimination along with the cytoptose phenomenon in the intestinal lumen.

Tissue transglutaminase antibodies in celiac disease, comparison of an enzyme linked immunosorbent assay and a dot blot assay.

AIMS: The purpose of our study is to determine the sensitivity, specificity and predictive values of an enzyme linked immunosorbent assay (ELISA) and a dot blot assay for the detection of IgA class anti-tissue transglutaminase antibodies (IgA-AtTGA) and to compare these results with those of IgA class anti-endomysium antibodies (IgA-AEA), IgA class anti-reticulin antibodies (IgA-ARA) and IgA class anti-gliadin antibodies (IgA-AGA). PATIENTS: Serum samples from 143 patients (97 children, 46 adults) with untreated celiac disease (CD) confirmed by intestinal biopsy and 74 disease controls (64 children, 10 adults) were studied. Methods. - The anti-tissue transglutaminase antibodies were detected by dot blot assay and an ELISA using guinea pig tissue transglutaminase (gp-tTG) as antigen. The anti-endomysium antibodies were detected by an indirect immunofluorescence technique on cryostat sections of human umbilical cord. The anti-reticulin antibodies were also investigated by indirect immunofluorescence on cryostat sections of kidney, liver and stomach of rat. The anti-gliadin antibodies were determined by an ELISA. RESULTS: The sensitivity of an ELISA for the detection of anti-tissue transglutaminase antibodies was 86% in children and 87% in adults and the sensitivity of dot blot assay was 57% in children and 54% in adults. The specificity of an ELISA and dot blot for the detection for anti-tissue transglutaminase antibodies was, respectively, 96% and 88% lower than that of anti-endomysium antibodies (100%). The sensitivity of anti-gliadin antibodies was 97% in children and 91% in adults and their specificity was 85%. The sensitivity of anti-reticulin antibodies was 94% in children and 87% in adults. Their specificity was 100%. CONCLUSIONS: The sensitivity and specificity of an ELISA for the detection of anti-tissue transglutaminase antibodies were better than that of dot blot assay. However, this dot blot assay could screen four celiac patients who have not had anti-tissue transglutaminase antibodies by an ELISA. The sensitivity of anti-endomysium antibodies was better than that of anti-tissue transglutaminase antibodies, anti-reticulin antibodies and anti-gliadin antibodies but in children aged less than 2 years, the sensitivity of anti-gliadin antibodies was better than that of anti-tissue transglutaminase antibodies.

[Phthiriasis pubis palpebrarum in children. Diagnosis and treatment].

INTRODUCTION: Phtiriasis palperarum is an unusual cause of blepharitis. This ectoparasitic infestation of the lashes is more frequent in adult; a pubic attack is usually associated. It is however rare in children. The goal of our study is to bring back four observations of infantile phtiriasis palpebrarum. PATIENTS AND METHODS: Four children presented themselves for palpebral itching and ocular redness. Slit lamp examination revealed evidence of small parasites attached to the proximal extreme of the eyelashes. A survey within the family was carried out and parasitological examination was realized. RESULTS: Parasitological examination had identified adult forms and nits of phtirus pubis. The bed linen was the way of contamination. Mechanical extraction of the parasite was tried but was very painful. Two patients were treated with a regimen of 1% yellow oxide of mercury ointment four times daily for 14 days, the two others was treated by Vaseline pomade because of a very important palpebral irritation. Evolution was favorable among all patients. DISCUSSION: We discuss in this work the ways of contamination of phtiriasis palpebrarum in children. In fact, eyelashes contamination in children is secondary to a contact with an adult carrier of a pubic phtiriasis. The transmission can be done following sexual maltreatments or by the means of infected clothing or bed linen. The diagnosis of phtiriasis palpebrarum is clinical confirmed by parasitological examination. Different therapeutic were proposed, the mechanical treatment must be carried out but it is seldom sufficient and it is often necessary to associate a chemical treatment (malathion to 1%, mercury oxide with 1%, fluorescein with 20%...) or physical treatment (cryotherapy, laser argon). CONCLUSION: Diagnosis of phtiriasis palpebrarum is easy and requires the detection of the source of contamination in order to prevent reinfestations.

[Malassezia fungal blepharitis: a case report].

PURPOSE: Blepharitis is a very common disorder but the role of certain mycoses in the etiopathogenesis of blepharitis remains controversial. OBSERVATION: We report the case of a 29-year-old man presenting with bilateral redness and intense itching of the edge of the eyelid. Slit-lamp examination showed vasodilation of the vessels of the edge of the eyelid associated with seborrhea and a sticky yellowish substance at the roots of the eyelashes. When the substance dried and fell, microulcerations were seen at the base of the eyelids, characteristic of follicular-seborrheic blepharitis. Otherwise the ophthalmological examination was normal. Treatment with eye drops (antibiotics, antiseptics, corticoids, antihistamines) was tried several times. The blepharoconjunctivitis regressed during these treatments without totally disappearing, recurring after the treatment was discontinued. Bacteriological and fungal exams were conducted, showing numerous Malassezia furfur spores and hyphae. Given that there was no antifungal eye drop preparation on the market, topical ketoconazole (Nizoral) was prescribed. Two weeks later, the condition had cleared, with no recurrence after 5 months of follow-up. DISCUSSION: Malassezia is a saprophyte species that may play a role in certain cases of chronic blepharitis, either through a reaction of intolerance and hypersensitivity or occasional proliferation. Treatment consists in eradicating the causal fungus using an antifungal agent associated with symptomatic treatment. We stress that fungal exams should be conducted whenever blepharitis recurs.

Identification of Malassezia species from Tunisian patients with pityriasis versicolor and normal subjects.

BACKGROUND: The genus Malassezia (Pityrosporum), recognized as a member of microbiological flora of the skin in humans and warm-blooded animals, has been recently revised to include 10 Malassezia species. The aim of the study was the isolation, identification and analysis of Malassezia species distribution in skin of healthy volunteers and lesions of pityriasis versicolor (PV). MATERIAL AND METHODS: Specimens were taken from 100 patients with PV and 30 healthy volunteers. Cultures were made in modified Dixon agar medium and the isolates were identified by morphological and physiological methods: macroscopy, microscopy, catalase, beta-glucosidase and lipid assimilation tests. RESULTS: Malassezia globosa was the predominant species in lesional skin of PV (65%). It was isolated alone in 47% of cases and associated in 18% with M. furfur (13%) or M. sympodialis (5%). In healthy skin M. globosa was found alone in 7.77% and associated in 15.54%, respectively, with M. furfur (4.44%), M. sympodialis (4.44%), M. restricta (3.33%) and M. slooffiae (1.11%). CONCLUSION: From these findings it was suggested that M. globosa presents the main species implicated in the pathogenicity of PV and M. furfur as the second agent of importance.

[Lanthanides and microanalysis. Effects of oral administration of two lanthanides: ultrastructural and microanalytical study].

The subcellular localization of cerium and lanthanum in the intestinal mucosa was studied after oral administration of cerium chloride or lanthanum chloride or lanthanum chloride followed 30 minutes after of cerium chloride to young adults Wistar rats. Two methods of observation and microanalysis were used. The transmission electron microscopy revealed the presence of dense electron granulations in the lysosmes of the duodenum enterocyte, when these elements were administrated simultaneously. The ion mass microanalysis permits to detect the presence of La and Ce as bright points outlining the intestinal villi. These points correspond to the lysosomes containing the granulations previously described. These granulations are formed by the cerium and the lanthanum associated to the phosphor and forming probably insoluble salts of Ce/La phosphate.

Fasciola hepatica: the development of tissue lesions and their frequencies in naturally-infected Galba truncatula.

A total of 530 Galba truncatula were collected from the oasis of Tozeur (south-western Tunisia) between February and July 1997 to detect snails with active or abortive infections with Fasciola hepatica, and determine the frequencies of their tissue lesions. Histological examination of 290 snails with active infections showed the presence of a multifocal or generalized necrosis in six viscera (albumen gland, digestive gland, gonad, intestine, kidney, and oviduct) and the absence of epithelial reconstitution. Tunnel-shaped lacunae and fibro-amoebocytic granulomas were found in the foot of all snails. Hypertrophied areas of the subepithelial muscular layer were noted in 16.7-71.4% of mantles. In the 240 snails with abortive infections, necrosis only developed in the albumen gland and the digestive gland from snails sacrificed in May and July. Contrary to this, epithelial reconstitution was observed in the digestive gland, gonad, and the albumen gland. In the mantle, the hypertrophy of the subepithelial muscular layer increased in frequency from February to July. Comparison of these frequencies using ANOVA demonstrated that the type of Fasciola infection (active, or abortive) had a significant effect on these percentages. In contrast, the type of the tissue and the period of snail collection had no significant effects. These results might be explained by the succession of two snail generations over time (overwintering snails, and spring-born generation), and probably by the fact that snail infections would be plurimiracidial and/or repeated over time in the oasis of Tozeur.

[HPA-5b neonatal allo-immune thrombocytopenia: two cases described in Tunisia].

UNLABELLED: Alloimmune thrombocytopenia is due to feto-maternal incompatibility in the HPA systems and is usually considered in the diagnosis of neonatal thrombocytopenia after other causes have been excluded. We report on two Tunisian observations of alloimmune neonatal thrombocytopenia due to anti-HPA-5b (Bra) antibodies. CASE REPORT: Two neonates presented at birth with a thrombocytopenic purpura unexplained by usual causes of neonatal thrombocytopenia. Alloimmune neonatal thrombocytopenia was diagnosed by the determination of parental and neonatal platelets antigens phenotypes and by the presence of HPA-5b (antiBra) antibodies in maternal sera. A favourable evolution was obtained after maternal platelet transfusions. CONCLUSION: Alloimmune neonatal thrombocytopenia is a serious affection, which exposes to intracranial haemorrhage. These observations of HPA-5 neonatal alloimmunisation in Tunisia provide additional information on the geographic distribution of the disease and its prognosis.

Presence of dermatophytes on the nails of tinea capitis patients. Prospective study in the area of Sfax (Tunisia).

Between January 1995 and December 2000, 512 cases with mycologically confirmed tinea capitis have been diagnosed in our laboratory. In addition, finger nail samples have been taken from 211 patients out of 512. The culture of the nail samples taken was positive for 80 patients (38%). The isolated dermatophytes were the same in the nails and the scalp. Microsporum canis was isolated in 82% of nail samples. Trichophyton violaceum grew only in 18% and Trichophyton quinckeanum in one case. A T. violaceum onychomycosis was found in two patients with tinea capitis. Twenty-one cases of tinea were resistant to 6 weeks treatment with griseofulvin. Thirty per cent of them have had fungal elements in nails. Dermatophyte colonization of the nails in tinea patients could be a factor of tinea capitis transmission and could explain some recurrent tinea capitis cases and therapy resistance.

Favus due to Trichophyton mentagrophytes var. quinckeanum.

A case of inflammatory tinea capitis caused by Trichophyton mentagrophytes var. quinckeanum in a 3-year-old Tunisian girl is presented. The disease healed after treatment with griseofulvin for 6 weeks and topical application of econazole.

Strongyloidiasis in Sfax (Tunisia): a survey over 13 years period.

A retrospective study of strongyloidiasis cases was carried out in Sfax University (Tunisia), between 1989 and June 2001. Diagnosis was done by direct stool examination, concentration methods, Baermann larval extraction and fecal culture. Strongyloidiasis was diagnosed in 27 patients of whom 77% were males living in rural areas under low socioeconomic level. Mean age of patients was 47 years (8 to 85 years). Hypereosinophilia was present in 75 % of cases, and 7 patients were treated by systemic corticosteroids. So, it is important to detect this parasite by repeated parasitological stool examinations, particularly before a long corticosteroid treatment.

[Clandestine slaughtering in Tunisia: investigation on the knowledge and practices of butchers concerning hydatidosis].

UNLABELLED: Echinococcosis is one of the major parasitic diseases in Tunisia, accounting for a considerable public health problem. We propose in this work to analyse the behaviour of butchers and to evaluate its impact on the hydatic cyst transmission. MATERIAL AND METHODS: We carried out a national transversal descriptive survey that included 97 butcher shops using illegal practice, located in 18 regions (governorats) of Tunisia. The sampling procedure was implemented according to the level of disease endemicity. RESULTS: 60.8% of butcher shops were located along main roads and therefore supplying travellers. Among the animals slaughtered sheep come in first position (97.9%). More than 80% of the butchers kept aside a place around the butcher shop to sacrifice their animals. We noted the presence of dogs near butcher shop in 52.6% of cases and stray dogs in 43.3% of cases. Only 13.4% of the butchers knew the disease mode of transmission. Parasitized viscera were thrown into dustbins or further in the riverbed in 56.7%. More than 50% of butchers, having an inadequate behaviour in front of parasitized viscera or ignoring the disease cycle, were originating from an hyper or holoendemic area. CONCLUSION: Illegal slaughtering practices have an important impact on transmission of cystic echinococcosis in Tunisia.

[Otomycosis due to Scopulariopsis brevicaulis].

We report a case of otomycosis due to Scopulariopsis brevicaulis in a patient with left cholesteatomatous chronic otitis media who presented with otorrhea associated with left otalgia. Scopulariopsis brevicaulis is a fungus which is rarely described as causing otomycosis although it is a saprophyte and widespread in the environment. In this case the contamination could be due to intensive gardening with poor hygiene. Its mycologic diagnosis is relatively easy and nystatin remains the most effective treatment in this region.

Net-targeted mutant mice develop a vascular phenotype and up-regulate egr-1.

The ternary complex factors (TCFs) Net, Elk-1 and Sap-1 regulate immediate early genes through serum response elements (SREs) in vitro, but, surprisingly, their in vivo roles are unknown. Net is a repressor that is expressed in sites of vasculogenesis during mouse development. We have made gene-targeted mice that express a hypomorphic mutant of Net, Net delta, which lacks the Ets DNA-binding domain. Strikingly, homozygous mutant mice develop a vascular defect and up-regulate an immediate early gene implicated in vascular disease, egr-1. They die after birth due to respiratory failure, resulting from the accumulation of chyle in the thoracic cage (chylothorax). The mice have dilated lymphatic vessels (lymphangiectasis) as early as E16.5. Interestingly, they express more egr-1 in heart and pulmonary arteries at E18.5. Net negatively regulates the egr-1 promoter and binds specifically to SRE-5. Egr-1 has been associated with pathologies involving vascular stenosis (e.g. atherosclerosis), and here egr-1 dysfunction could possibly be associated with obstructions that ultimately affect the lymphatics. These results show that Net is involved in vascular biology and egr-1 regulation in vivo.

[Anti-endomysium, anti-reticulin and anti-gliadin antibodies, value in the diagnosis of celiac disease in the child].

Coeliac disease is associated with gluten intolerance in genetically predisposed subjects. Environmental factors, particularly of viral origin, may also play a major role. In this study, the presence of IgA class anti-endomysium antibodies (AEA-IgA), IgA class anti-reticulin antibodies (ARA-IgA) and IgA class anti-gliadin antibodies (AGA-IgA) was investigated in 120 serum samples from 120 children (60 patients with coeliac disease and 60 control subjects). The AEA were detected by indirect immunofluorescence on sections of human umbilical cord. The ARA were also investigated by the same technique in rat kidney, liver and stomach. The AGA were determined by an enzyme-linked immunosorbent assay (ELISA). In the patients with coeliac disease, the sensitivity of AEA and ARA was 86% and 76% respectively. In both cases, the specificity was 100%. In children below two years of age, the sensitivity of AEA and ARA was too low, i.e., 57% and 35% respectively. In children aged between two and 15 years, the sensitivity of AEA and ARA was 95% and 89% respectively. The sensitivity of IgA class AGA was 86%, and their specificity was 83%. In this study population, these results show that IgA class AEA are interesting markers for the diagnosis of coeliac disease in the child, and could be used in screening for coeliac disease in a high-risk population.

Root growth and lignification of two wheat species differing in their sensitivity to NaCl, in response to salt stress.

Application of a 100-mM NaCl salt stress to wheat seedlings of a salt-tolerant (Triticum durum var. Ben Béchir) and a salt-sensitive (Triticum aestivum var. Tanit) species decreases the fresh and dry weights of roots especially in the salt-sensitive species, and slightly increases the ratio of dry to fresh weight, especially in the salt-resistant species. All peroxidase activities are increased by salt stress, the water-soluble peroxidase activity being increased much more in the salt-sensitive than in the salt-tolerant species, while the opposite result is observed with the cell-wall peroxidase activity. Some water-soluble peroxidases have been hypothesised to have auxin oxidase activity (which might explain the effect observed on the root biomass), while the cell-wall peroxidases would be involved in lignification. Histochemical observation confirms a more intense lignification in the root cells of the salt-tolerant species compared to the sensitive species, under the effect of NaCl.
