Use of a bioprosthesis to relieve tension in oesophageal anastomosis: an experimental study.

Collagen-coated Vicryl mesh was assessed as a prosthetic repair material in the oesophagus of growing piglets. In a group of six piglets (group A), a full thickness defect was created in the oesophagus without fully transecting the organ. The removed tissue was subsequently replaced with a patch of collagen-coated Vicryl mesh. Animals were killed at 4 weeks and in the five surviving piglets the prosthesis had been replaced by host scar tissue with a normal epithelial lining. Two further groups of six piglets (groups B and C) were used as controls. In group B the oesophagus was either partly transected (four animals) or completely transected (two animals) and an anastomosis was performed, whereas in group C a 2.5-cm length of oesophagus was removed so that the anastomosis was performed under tension. In group B, all six piglets survived, whereas in group C, where the anastomosis was performed under tension, only two piglets survived. Microscopic and macroscopic examination of all groups showed that the collagen-coated Vicryl prosthesis was replaced by scar tissue; moderate stenosis was observed in only one animal in group A. Collagen-coated Vicryl mesh may be used either to reinforce an anastomosis performed under tension or may find use in tubular form to bridge the gap between the two ends of the oesophagus to be joined. It is envisaged that such a material may find applications in the treatment of oesophageal atresia.

Hepatocarcinoma in a child: a clinical and experimental study.

An inoperable hepatocarcinoma in a 2-year-old female was treated with chemotherapy. Regression of the tumour allowed complete surgical excision. The tumour was successfully xenografted into athymic rats. Growth of tumour was monitored by external measurement and plasma alphafetoprotein (AFP) assays. AFP levels meanwhile in the patient fell from 2 x 10(6) international units/ml preoperatively to 10 units/ml post-operatively. The value of AFP as a tumour marker in hepatic tumours and the availability of xenograft material for experimentation enabled the measurement of plasma AFP levels as well as assessing the tumour mass.

Developmental aspects of gastroschisis.

The development of gastroschisis was studied experimentally as well as clinically, particularly concerning the characteristic fibrous coating of the protruding bowel loops, associated intestinal atresia, and postoperative hypoperistalsis without intestinal obstruction. Experimental investigation was carried out in the chicken embryo. The clinical study was a joint one, involving a total of 50 patients with gastroschisis seen at the Sophia Children's Hospital, Rotterdam, and the Royal Hospital for Sick Children, Glasgow, between 1972 and 1985. Some patients were followed antenatally. The results, correlating with previous experiments, were compared with data from the literature. The fibrous coating of the protruding bowel loops appeared to be a late occurrence and directly related to changes of the amniotic fluid secondary to the onset of renal function. Associated intestinal atresia and postoperative hypoperistalsis in the absence of an obstruction both appeared to be due to another late gestational event, consisting of ischemic changes of the bowel wall secondary to the compression of bowel loops and mesentery in the small abdominal wall defect.

Complications of rectal suction biopsy.

Complications following use of the rectal suction biopsy technique in the diagnosis of Hirschsprung's disease, are rare. In a series of 1,340 consecutive biopsies, complications included three clinical perforations of the bowel, one resulting in death, and three rectal hemorrhages requiring transfusion. A plea is made for the use of greater care in this technique.

A review of thermal injuries in young children.

A group of 472 children under 3 yr of age were treated for thermal injuries in a 5-yr period; 65% were boys. Injuries were due mainly to scalding (82%) and involved predominantly the upper part of the body. Full thickness skin loss resulted in 30% of cases, and skin grafting was needed in 29%. Forty-two episodes of infection arose, and 50 patients (11%) developed other complications--hyperpyrexia, respiratory problems, fluid imbalance and convulsions were the commonest. The mortality rate was 1% (4 cases) and the mean duration of stay in hospital was 17 days. The plan of management is outlined with emphasis on the essential differences in treatment of young children, and the results are reported.

Group meetings for parents in a children's burns unit.

Thermal injuries in childhood occur mainly in the first three years of life and often lead to hospital admission: most are due to scalds in the home. The distress and anxiety of parents bears on the management of the young sick child in hospital, on his subsequent recovery at home and on the welfare of the whole family. To help parents, a weekly meeting was held with a small group of staff in the Burns Unit at which there was free and open discussion of all problems and worries connected with the child's accident. This paper describes the working of the group and the contribution it made to the care of 149 children admitted with burns and scalds during one year.

Surgical correction of torsion of the penis.

Torsion of the penis is a rare anomaly, believed to be due to abnormal skin attachment and not to any structural abnormality of the corpora. The rotation is usually to the left in a counter-clockwise fashion. The urethral meatus is placed in a oblique position and the median raphe makes a spiral curve from the base of the penis towards the meatus. Three boys with congenital torsion of the penis are reported. A corrective operative for this condition is described. Penile torsion was first described by Verneuil in 1857, but in the past no operative correction was recommended, since attempts to move the skin around were believed not to correct the spiral alignment of the corpora cavernosa.

Colonic stricture following necrotizing enterocolitis.

There has been a steady increase in the incidence of necrotizing enterocolitis (N.E.C.). Early diagnosis and prompt surgical intervention have improved the mortality considerably (8). With the increasing number of survivors, late complications are more frequently encountered. Three neonates who had developed strictures of the colon 3, 4 and 6 weeks respectively, after the onset of the acute episode of N.E.C. are reported and showed interesting features, an extensive long stricture and a tight short stricture which simulates a colonic atresia.

Active treatment of ganglia in children.

One hundred and twenty-eight children, admitted for active treatment of ganglia around the wrist, hand, ankle or foot are reviewed. The methods of treatment are discussed. The most satisfactory results were obtained by excision (7-2% recurrence) and incision with a tenotome (11% recurrence). The overall recurrence rate was 19%. The ganglia around the wrist had the highest rate of recurrence.
