[The different ways of dying. Ethical reflections].

Death Process is seen influenced by multitude of factors. It's not habitual that the patient decides about the circumstances of his death. In the terminally ill situation, the Palliative Medicine is considered the best option. Sometimes the death is produced as consequence of an action or omission, which raises ethical important dilemmas difficult to resolve. The similarities and differences between active euthanasia and passive euthanasia are analysed.

[Nutritional support in terminally ill situation. Ethical dilemmas].

Malnutrition is a common complication in terminally ill situation. Artificial nutritional support in these patients create a host of ethical conflicts with difficult solutions. In this paper we give simple suggestions that might the help care provider in making a viable clinical decision. However, each case should be treated in an individualized manner taking into consideration the ratio between risks and benefits of this treatment. The patient's wishes should have the highest priority.

[Terminal sedation. Ethic aspects].

For better or for worse, bioethics is a discipline that, on occasion, presents more doubts and questions than solutions or answers for the variety of issues it implies. Sedation-as it concerns the terminally ill patient-is a clear example of one such issue. Sedation, used to diminish the intense suffering of those terminally ill patients who do not respond to conventional treatment, is a standard method by which the practice of palliative care presents often significant bioethical conflicts. I reconsider these ethical dilemmas that occur as a result of this decision.

[Bioethical principles: are they applied in terminal illness situations?].

Comprehensive practice of biomedical ethics principles (beneficence, non-maleficence, autonomy and justice) would guarantee appropriate assistance to patients with terminal diseases. Unfortunately today, these principles aren't put into practice in a systematic way. In order to guarantee quality care to this particular group of patients, it is necessary that all health care professionals dealing with terminal illness receive a comprehensive education in bioethics matters and that an appropriate methodology is followed in order to resolve subsequent conflicts. Furthermore, public institutions should be involved so that Palliative Care Units provide assistance to the entire population rather than select cases. If we can't provide comprehensive care, we are working against the basic bioethics principles, especially the fundamental principle of justice.

[Ethical and clinical decisions on artificial hydration and feeding via tube in the terminally ill].

Tube feeding is a widely used medical treatment for providing nutrition to terminally ill patients. Long term nutritional support in these patients or patients with irreversible neurological deterioration create a host of ethical and legal conflicts with difficult solutions. This work looks at the cultural and ethical-legal basis of these decisions and gives simple suggestions that might help the health care provider in making a viable clinical decision. However, each case should be treated in an individualized manner taking into consideration the rights and desires of each patient and their families.
