Alternatives to DRGs: research issues.

Legislative mandates for the development of prospective payment methodologies for reimbursing inpatient care have spurred the psychiatric community into its own independent effort, applying both clinical and service research, to develop schemes which are fair to psychiatric patients, their providers and payers. Many of the issues involved in the development of an equitable prospective payment system (PPS) for psychiatric care are not new to the field, namely, problems of limited diagnostic accuracy, a limited knowledge of the outcomes of clearly defined treatment protocols, and a lack of integration, efficiency and efficacy in a highly differentiated service system. The research community needs to respond to these problems in the context of cost containment with an awareness that its response will influence care for psychiatric patients for the remainder of this century. In this paper, we will describe the background issues that have brought the psychiatric community to the present point in its response to PPS, and discuss some of the research problems posed by the need for a PPS with the right incentives for clinical effectiveness and efficiency.

Paranoid schizophrenia: prognosis under DSM-II and DSM-III-R.

DSM-III-R subtype-specific criteria and Tsuang-Winokur demographic criteria for paranoid schizophrenia were applied to records of discharged schizophrenic inpatients diagnosed according to DSM-II. Those meeting DSM-III-R subtype-specific criteria for paranoid schizophrenia and those who did not, ie, they received more hopeful diagnoses, had equally good prognoses. Good outcome was associated with the presence of prior affective symptomatology, absence of formal thought disorder, or other negative-type symptoms as well as Tsuang-Winokur demographic criteria of onset after age 25, married or employed status, and/or absence of family history. Patients with particularly poor outcome were identified by the coexistence of unsystematized or poorly developed persecutory ideation, symptoms of thought disorder, and/or disorganized behavior. The clinical and demographic descriptors discussed in this report may be particularly useful in creating homogeneous patient groupings for both clinical and rate-setting payment classifications.

Scholarship support for Indian students in the health sciences: an alternative method to address shortages in the underserved area.

The University of New Mexico Area Health Education Center was established in conjunction with the Navajo Health Authority to begin health manpower development immediately in the Navajo Nation and surrounding areas (a territory approximately the size of West Virginia). To this end, a student support program was established at the Navajo Health Agency to recruit and support Indian students with scholarships, to provide them with culturally based counseling, and to reinforce the students' intentions of ultimately returning to serve Indian people. No payback penalties or other forms of coercion were used in this program to encourage students to return to the underserved Indian areas. From October 1973 through September 1977, 124 students graduated with 125 degrees or certificates in all aspects of health care. Of these 124 students, 76 were employed. The remaining were continuing their education, unemployed, untraceable, or deceased. Of the 76 employed, 61 were from tribes within the Navajo Nation; of these 61, 56 returned to their area to serve Indians. This return rate to an underserved area is substantially better than anticipated from a review of programs that employ a variety of coercive methods to encourage recipients of loans to settle in specific underserved areas after the necessary training.
