[Effect of various infections on the development, course and outcome of glomerulonephritis in children].

Bacteriological and virological tests were made at the disease onset and repeatedly in 100 children (66 boys and 34 girls) aged 2 to 15 years suffering from acute glomerulonephritis. The throat mucus was found to contain streptococci and staphylococci; direct immunofluorescence demonstrated antigens of influenza A1, A3, B virus, of parainfluenza virus, serotypes 1 and 3, of adenovirus and respiratory and syncytial virus; HBs-antigens, specific viral and bacterial antibodies were detected in the blood serum. At the onset of acute glomerulonephritis, 52.5% of the patients were diagnosed to have bacterial infection, 12.5% respiratory viral infection, and 27.5% mixed infection (respiratory viral and bacterial). Besides, 25.5% of the patients showed HBs-antigenemia. HBs-antigenemia and mixed (respiratory viral and bacterial) infection were found to produce an adverse effect on the course and outcome of acute glomerulonephritis. Long-term circulation of some viral antibodies (mostly to parainfluenza-3 antigen) was revealed on repeated studies as was their untoward effect on the outcome of acute glomerulonephritis.

[HBs antigenemia in glomerulonephritis in children].

Altogether 194 glomerulonephritis patients were examined by three methods: countercurrent immunoelectrophoresis, passive hemagglutination test, and enzyme immunoassay. Use of the most sensitive method, viz. enzyme immunoassay, has yielded the highest HBsAg detection rate: 29.1% in acute glomerulonephritis and 21.4% in chronic glomerulonephritis. This method may be recommended for the examination of glomerulonephritis patients whose sera contain HBsAg in low titers.

[Indicators of blood coagulation and fibrinolytic activity of the urine in children with calculous pyelonephritis].

Parameters of hemocoagulation and fibrinolysis in the blood and urine were studied in 108 children with calculous pyelonephritis. Fibrinolytic inhibition and signs of hypercoagulation were noted. Changes in coagulation were found to be corresponding to the activity of the disease. The changes were mostly pronounced in the urinary fibrinolytic activity, with almost zero levels in those with bilateral nephrolithiasis accompanied by renal dysfunction. Fibrinolytic and anticoagulatory medication was found to be mandatory for the aforementioned patients.

[Neurosomatic parallels in children with neurogenic bladder dysfunction].

Clinical, urodynamic and neurological examinations of 30 children with hyperreflex dysfunction of the bladder gave evidence for clinical symptoms of natal trauma sequelae in 23 of them. Diagnosis of polyvisceral functional changes and chronic urinary diseases was made significantly more frequently in children with neurological symptoms. High occurrence of neurological symptoms of natal subclinical cervical traumas in children with hyperreflex dysfunction of the bladder suggests their pathogenetic relations. Hemodynamic changes of the cerebral vessels with affection of the venous outflow may cause hypoxia of the hypothalamo-hypophyseal structures with emergence of symptom complex of vegetovascular dystonia and marked neurogenic dysfunction of the bladder. The authors conclude on validity of pathogenetic neurological therapy to coordinate the activity of spinal and supraspinal centers regulating function of the bladder.
