Results of surgical treatment of 28 adolescent girls with vaginal aplasia and functioning uterus are analyzed. The uterus had to be removed in all the patients 1.5 to 6 months after abdominal colpopoiesis with preservation of functioning uterus; after sigmoidal colpopoiesis the uterus was preserved in 85% of patients. Analysis of the contents of the uterine cavity showed endometritis, which was evidently caused by the absence of the cervix uteri, a natural barrier preventing the penetration of intestinal flora of neovaginal mucosa into the uterine cavity. The studies permitted a new interpretation of the results of abdominal and sigmoidal colpopoiesis with preservation of functioning uterus in adolescent girls with vaginal aplasia.

On the basis of the results of rheotesticulographic studies in 38 children with left-sided II-III degree varicocele, the impairment in hemodynamics in the testicle at the involved side was established. No suppression of androgen function of the organ was revealed. The expediency to perform in patients of pediatric and young age the Ivanissevich operation, the effectiveness of which is increased in use of intraoperative phlebotesticulography was noted. The effective method for varicocele treatment in children has been suggested. The testicular vein is ligated near the deep inguinal ring as well as at the level of the upper third, then it is excised between the ligatures together with the deviating branches. The method was used in treatment of 120 patients with II-III degree varicocele. No disease recurrence is noted during 1.5 years.
The authors analyze the results of vaginal plasty with grafts from the rectosigmoid portion of the large intestine, carried out in 279 girls aged 3 to 18 with abnormalities of the genitals and vaginal aplasia, including that with a functioning uterus. They have developed new modifications of such surgery, that facilitate sexual social adaptation of the patients when they reach puberty.
The authors describe the data on clinico-laboratory and instrumental examinations and surgical treatment of about 5,000 children with different reproductive organ injuries including complex patterns of abnormal sexual differentiation. The designed diagnostic programs made it possible to distribute patients into groups and according to the disease entities in conformity with the classification suggested. It is emphasized that in the developing science concerned with sex, it is necessary to distinguish an area pertaining to childhood, namely pediatric andrology, and to delineate its subject matter and tasks. Skilled assistance can be rendered by a specialized center. A broad spectrum of improved masculinizing (including the design of an artificial phallus) and feminizing (including the design of an artificial vagina from the rectosigmoid segment of the colon) is provided. The catamnestic data are evaluated.
The experience with the treatment of more than 5140 children at the age of from 2 days to 16 years, and 76 patients at the age of from 15 to 42 years with different diseases of the genital organs and disorders in the sex formation is presented. The indications for surgical treatment are defined. The results of the masculinizing operations, including the phalloplasty and testicular endoprosthesis, and the feminizing ones, including the vaginoplasty with the use of a colonic segment are presented.

Interphase chromatin of peripheral lymphocytes was studied in patients aged 6 to 20 years with Turner and Morris's syndromes by AO labeled fluorometry using the authors' modification of DNP cell thermal denaturation. It was shown that the lymphocyte chromatin melting profiles represent the curves with seven maxima at the following temperatures: 47 degrees, 55 degrees, 65 (+/- 2)degrees, 78 (+/- 1)degrees, 82, 88 (+/- 1)degrees, 92 (+/- 2)degrees C (P less than 0.01). There were no statistically significant differences between clinically and karyotypically different groups either in the parallelism of the melting profiles or in the fluorescence intensity of AO connected with cell DNP. In the male control group, the similar curve was obtained for lymphocyte chromatin in 25 and for normal human spermatozoa in 100% of cases, i.e. deviation specificity was revealed in the lymphocyte melting profiles, from a "classical" normal variant to the so-called male variant with sex differentiation breaks (Turner, Morris and Klinefelter's syndromes). Possible cytogenetic mechanisms of breaks are discussed.
