Controllable stone baskets can capture the elusive ureteral stone.

Poor expansion of a spiral stone basket within the ureter often results in failure to engage a ureteral calculus. A special pull-wire incorporated into the design of such a basket facilitates predictable expansion of the basket even when the ureteral lumen seems narrow. Excellent results in capturing and removing ureteral calculi have been achieved with this controllable basket, including stones in several patients in whom standard spiral baskets failed to engage the calculus.

Ureteral balloon dilatation and stone basketing.

Dilatation of the ureter with a specially designed angioplasty balloon below large impacted ureteral calculi enhances the possibility of successful cystoscopic stone extraction. Current techniques, results, and conclusions are reviewed.

Percutaneous pigtail nephrostomy.

Practicing urologists have become more familiar with the use of angiographic equipment and techniques and proficient in their application to percutaneous nephrostomy. The advantage of pigtail catheters for that purpose is noted. Modification of available vascular equipment applicable to percutaneous nephrostomy is discussed, and a new standardized kit for such purposes is described. A logical sequence of steps taken to ensure proper placement of the tube and adequate urinary drainage is considered.

One-handed prostatic biopsy needle.

A dependable and disposable one-handed perineal prostatic biopsy needle has been developed. The needle makes it relatively easy to obtain multiple, good cores of tissue from the desired areas of the prostate gland without contamination of the needle or field by the rectal finger or hand.

Flexible tip ureteral catheters in clinical practice.

An ideal ureteral catheter has been produced and subjected to extensive clinical trial. Its safe, flexible tip permits passage into almost every orifice and ureter, regardless of its configuration, tortuosity or degree of obstruction. The flexi-tip catheter meets all of the criteria of the theoretical ideal ureteral catheter. It would appear to be the catheter of choice for most clinical situations and should be a welcome addition to the urologist's armamentarium.
