Botulinum toxin for treatment of craniofacial hyperhidrosis.

The effect of botulinum toxin A (BTX) was studied on 12 patients with idiopathic craniofacial hyperhidrosis. After confirming the diagnosis by Minor's iodine starch test we first treated one-half of the forehead with an injection of 2.5-4 ng BTX (Dysport) equidistantly intracutaneously. After 4 weeks we assessed the efficacy by another Minor's iodine starch test and then treated the other half. Another 4 weeks later a standardized telephone interview was carried out. After 1-7 days the craniofacial sweating in the area injected had completely ceased in 11 patients and was mildly reduced in the remaining one. The efficacy was confirmed by repeated Minor's iodine starch tests. Mild weakness of frowning was the only side effect, lasting 1-12 weeks and completely resolving in all patients. Although sweating has not yet recurred in most patients at follow-up periods up to 27 months, one patient had a relapse 9 months after treatment. Following reports on palmar and axillary hyperhidrosis and gustatory sweating (Frey's syndrome) this is apparently the first report on the use of BTX in the treatment of idiopathic craniofacial hyperhidrosis. BTX seems a promising new treatment for localized hyperhidrosis.

Chronic placentitis--a clinicopathological study.

Between 1978 and 1983 1240 singleton placentas were examined macroscopically and histologically. In 82 cases (6.6%) a "chronic placentitis" was diagnosed. The pathognomonic inflammatory foci were localized within the placental villi. The vast majority showed mixtures of both fibrohistiocytes and lymphoid cells, which were occasionally interspersed with plasma-cells. The clinical data relating to 67 placentas were correlated retrospectively with the degree of inflammation. 46 showed mild and 21 moderate to severe chronic placentitis. In both of these groups the proportion of preterm deliveries and cases with preeclampsia was equally high. With increasing severity of chronic placentitis, however, there was a significant reduction in weight and size of both, placenta and infant, and the incidence of perinatal asphyxia was significantly raised.

Benign spindle cell tumor of the male breast.

Benign spindle cell tumors of the male breast are definitely rare. They have a subareolar localization and are circumscribed but not encapsulated. Ultrastructurally the neoplasm consists of histiocytic cells, fibroblasts, myofibroblasts and their transitional stages. Myoepithelial cells or smooth muscle cells are lacking. Although myofibroblasts contain lots of filaments, aniline reaction, PTAH stain and iron hematoxylin impregnation remain negative.

Tissue damage by fatty acids released by lipolysis. Contribution to the pathogenesis of acute pancreatitis.

Local application of a mixture of emulsified olive oil and pancreatic lipase produces massive necrosis in liver, kidney, pancreas, skeletal muscle, and perivascular sheath within 3 hours with subsequent spread. The necrosis is accompanied by a marked leucocytic reaction and, - after 16-24 hours -, involves the walls of veins and arteries resembling the vascular necrosis of acute hemorrhagic pancreatitis. Electron microscopy shows necrosis of emigrated leucocytes after only 120 minutes. Vascular necrosis is not seen before the second day. The results clearly demonstrate the ability of free fatty acids to damage cells and tissue, and support the hypothesis that the morphological change of necrotizing pancreatitis are mainly produced by the release of fatty acids by active lipase.

[Electron microscopy of the lymphatics of the ureter (author's transl)].

Microvasculature of the mouse and human ureter was investigated by light and electron microscopy. Lymph vessels were only seen in the adventitial layer of the mouse ureter. Only one lymph capillary could be demonstrated in the submucosal and muscularis layer of the human ureter. Fenestrated blood capillaries were arranged just beneath the transitional epithelium. Numerous thin-walled postcapillary venules characterized by endothelial channels and fenestrations were seen in the adventitial layer. Apparently, a widespread ureteral lymphatic system is not necessary, since protein rich extracellular fluid can be reabsorbed by venous blood capillaries.

Rat liver alterations after chronic treatment with hexachlorobenzene.

Groups of female rats were treated orally with 0.5, 2.0, 8.0, and 32 mg/kg hexachlorobenzene twice a week for 203 days. The liver content of hexachlorobenzene was found to be dose-dependent. In the animals treated with the highest dose the concentration was 273 mug/g hexachlorobenzene. In the fresh and fixed hepatic tissue of the treated animals pink fluorescence was observed. Electron microscopy revealed a dose dependent enlargement of all hepatocytes due to proliferation of the SER in the centrolobular area or to increased glycogen deposits (beta- or alpha-particles) and SER in the intermediary and periportal area. Numerous porphyrin deposits and siderosomes, intimate disorganisation and moderate dislocation of the RER and a moderate enlargement of bizarre-sharped mitochondria were recognized. The relationship between porphyrin crystals and mitochondria on the one hand and between SER and glycogen deposits on the other is discussed.

Endometriosis of the ureter.

Endometriosis of the ureter is an exceptional cause of ureteral obstruction. Pathogenesis and clinical picture are demonstrated on the basis of 2 cases, giving an example for the spread of endometrial tissue via the lymphatics. It is emphasized that young women should have a gynecological examination in case of pelvic ureteral obstruction. Final diagnosis of the isolate ureteral endometriosis can only be made by histological examination. Therapy is usually surgical followed by hormonal treatment. The routine use of excretory urograms in women suffering from endometriosis should prevent urological complications.

[Epidermoid cyst of the tibia (author's transl)].

Case report on a large epidermoid cyst in the proximal tibia in a 42-year old woman. Scrutiny of the available literature shows that this is the first recorded case of an epidermoid cyst in a long bone.

The importance of smooth muscle cells in the development of foam cells in the gastric mucosa. An electron microscopic study.

Foam cells in lipid islands of the stomach can develop from both histiocytes and smooth muscle cells. With increasing storage of lipid vacuoles in smooth muscle cells, loosening of the myofilament arrangement and decrease of the dense areas subjacent to the plasma membrane occurs. Endoplasmic reticulum and the cisternae of the Golgi-apparatus dilate, the cell organelles increase initially and the basement membrane of the smooth muscle cells is fragmentarily formed. Only in incompletely formed foam cells can the origin from smooth muscle cells be recognised, in their final state their histiogenesis is seldom apparent.

[Distribution of the lymph vessels in the mouse heart. A light and electron microscopic study (author's transl)].

The lymphatic system in normal mouse heart was investigated by light and electron microscopy. Lymph vessels were only demonstrated in the subepicardial layer and the outer half of the myocardium. Here above all they were situated in close vicinity to arteries and thin-walled veins, rarely separated in the interstitial space, never in the subendocardial layer of the left ventricular wall. The myocardium contains only 2 lymph vessels per mm2. By means of electron microscopy all lymph vessels were regarded as lymph capillaries. Their ultrastructure is described in detail. This investigation shows that lymph vessels in normal heart are rarer than supposeed till now. It is possible that most of the ultrafiltrated fluid is drained by the cardiac venous system, while the lymphatic system only has to transport a small amount of protein-rich fluid.

New aspects of vulvar cytology.

Smears were taken from vulvar skin in 203 cases (83 normal-appearing, 23 benign, 12 precancerous and 85 malignant conditions). Three cases were analyzed by electron microscopy. The appearance of dyskeratotic cells was regarded as a sign of malignancy; this was supported by ultrastructural study. Smears from normal vulvar skin revealed a high correlation between age and presence of nucleated keratinized squames, which occurred most often in the third and rarely in the eighth decade. Benign conditions showed cellular features like those of the normal vulva. In precancerous lesions, 58% of the smears showed dyskeratotic cells. Malignant conditions showed dyskeratotic cells in 66% of the smears and true cancer cells exclusively in an additional 14%. Our observations indicate that vulvar cytology can be a major diagnostic aid in all suspicious cases, especially in the elderly. Vulvar cytology cannot replace an exact histologic examination, but it can provide a reason for taking a biopsy earlier than might otherwise be the case.
