Liver function after bilateral nephrectomy.

Consequences of bilateral nephrectomy (NX) for liver functions and for hepatic excretion of various endogenous substances were characterized in rats 24 h after NX. Plasma concentrations of urea, creatinine, fibrinogen, and glutathione increased significantly after NX, whereas the concentrations of total protein, albumin, and lipids decreased. The hepatic excretion of urea, creatinine, phospholipids, cholesterol, and aldosterone significantly increased in uremia, and excretions of protein and glutathione diminished. Active biliary transport can be diminished after NX by the effects of uremic toxins on the liver cells or by the competition phenomena between endogenous substances, which are normally excreted in urine, at the hepatocellular level. Reduced glutathione content and increased lipid peroxidation in hepatocytes have been found. Changes in lipid and protein metabolism after NX can be proved.

[The diagnostic value of serum hormone parameters in hirsutism].

In 46 patients showing mild or moderate hirsutism, and 49 age-matched regularly menstruating women without symptoms the following hormone and protein serum levels were measured: Total testosterone (T), free testosterone (fT), dehydroepiandrosterone sulfate (DHEAS), androstendione (ASN), sex hormone binding globulin (SHBG), free androgen index (FAI) as expressed by the T/SHBG ratio, cortisol, Prolactin (PRL), LH/FSH ratio, 3 alpha-androstanediol glucuronide (ALG), basal and poststimulated 17-hydroxyprogesteron (17OH-P) and (in some cases) basal and poststimulated 11-deoxycortisol (S). The study was designed for exploring the diagnostic significance of these parameters and evaluating the extent to which they contribute in establishing the source of hyperandrogenism. The mean values of all but one (PRL) of the hormone and protein levels varied significantly from the controls, T, DHEAS, FAI and ALG showing the greatest differences. Most frequently elevations of T, DHEAS, and FAI values were established, the levels of which exhibited a highly significant correlation. In 96% of the hirsute women the elevation of 1 to 3 of these parameters was demonstrated. FAI was shown to be associated with a higher diagnostic accuracy than did SHBG or fT. In 9/43 patients increased values of ALG were found, which as an important metabolite of dihydrotestosterone reflects peripheral androgen activity, particularly of the skin. In every case these abnormal values were combined with elevated levels of other androgens (T, DHEAS, ASN). It is concluded from these limited data that an isolated elevation of ALG due to primary accentuation in 5 alpha-reductase is a rare or even non-existing occurrence.(ABSTRACT TRUNCATED AT 250 WORDS)

Hormone analytical and carrier protein alternations following administration of mifepristone (RU 486) for termination of early pregnancy.

Outpatient terminations were performed on 50 biochemically confirmed pregnancies of 33 to 43 days amenorrhea with a single oral dose of 600 mg of mifepristone (RU 486). 37 women were randomly assigned to an additional endocrine investigation, comprising serum levels of beta hCG, progesterone (P), estradiol (E2), cortisol (C), sexual hormone binding globulin (SHBG), and cortisol binding globulin (CBG) on days 0 and 7 prior to and following the drug administration. Of these 37 patients, 30 experienced a complete expulsion of the uterine contents, 28 until day 7, 2 subjects on day 9 and 10, respectively. The remaining 7 women either showed an unaffected pregnancy or the symptoms of incomplete abortion, which made a surgical intervention necessary. In the successfully treated cases the levels of beta hCG, P and E2 decreased significantly, the nonresponders showed unchanged or even increasing hormone values. No significant changes of cortisol concentrations were found in the group of nonresponders, while the successful cases exhibited a slight elevation probably due to the abortion-associated processes. No considerable therapy-dependent differences were observed in the levels of SHBG. A small but significant decrease in the mean CBG value could be calculated in the group of responders.

Diacetyl-splenopentin (BCH 069) did not affect the concentrations of various hormones in men.

We investigated the effect of BCH 069 (similar structure as thymopentin) on concentrations of hormones of the hypophysis, adrenals, gonads and thyroid glands in men after a therapy period of 4 or 6 weeks. No significant effects of BCH 069 three times weekly 50 mg s.c. for 4 weeks or i.v. for 6 weeks on serum concentrations of prolactin, LH, TSH, cortisol, testosterone, progesterone, estradiol and ETR were observed.

[Testicular function in obstructive azoospermia].

The normal histological findings of the testicles with simultaneous azoospermia according to the former opinion renders greatly probable an obstruction of the seminal ducts. The reversed case of an ascertained obstruction, however, must not necessarily contain also normal histological structures or normal hormone tests. In the obstruction syndrome in 6 out of 23 cases we find changes in the diameter of the tubule, reductions of the spermatogenesis and changes at the interstice. Imprint-cytologically an increase of pathological spermatides at the cost of normal cell developments is shown. According to this the FSH-values are slightly but statistically significantly increased. LH, PRL and testosterone are found within the normal. For the diagnostic delimitation of the obstruction syndrome from a production azoospermia according to these findings the performance of a biopsy of the testicles with histological evaluation and the determination of the FSH-value. The relatively discrete tubular changes alone cannot be used for the explanation of the discrepancy between anatomical and functional results of the operative reconstruction of the seminal ducts.

Transcortin as an indicator of estrogenic potency in oral contraceptives.

Serum cortisol-binding globulin (CBG) was measured before, during and following a 21-day application period of 5 oral contraceptives used in the GDR. In a second group of patients determinations were carried out at the end of a 24-month treatment period and the end of the first post-treatment cycle. From a third group of volunteers blood samples were taken at the end of the first, second and third treatment cycles. All the drugs tested produced a marked increase of CBG in serum, reaching its maximum at the end of the second treatment cycle. Within four weeks following termination of application the level fell to the control range. There were no significant differences between the various contraceptives.

[Comparative studies into effects of synthetic oestrogens and gestagens on certain serum protein fractions under standardised conditions (author's transl)].

Initial effects of the following hormonal contraceptives on certain serum proteins in 29 women were checked under standardised conditions through the first three cycles on the pill: Deposiston (ethinyloestradiol-sulphonate/norethisterone-acetate), Gravistat (ethinyloestradiol/norgestrel), Non-Ovlon (ethinyloestradiol/norethisterone-actate).--Effects were found to differ strongly by fractions. Differences were recorded also from action over time. Changes and deviations were confirmed to depend on oestrogen levels.

[Reversibility of alterations induced by sexual steroids in various serum protein fractions, following application of hormonal contraceptives (author's transl)].

Reversibility of changes induced by sexual steroids was studied in 19 different serum protein fractions of 20 women. The following preparations were available for testing: Gravistat (ethinyl-oestradiol/norgestrel), Non-Ovlon (ethinyloestradiol/norethisterone-acetate), Ovosiston (mestranol/chlormadinone-acetate), and Deposiston (ethinyloestradiol-sulphonate/norethisterone-acetate). The tests were made towards the end of the 24th cycle on the pill and in the first cycle thereafter.--The proteins tested were found to be affected in a differentiated way and, throughout, depending on oestrogen levels. Reversal was rapid, but the phase of restitution usually was longer than the period of testing and follow-up. The long-lasting action of Deposiston was visualised also in its "reversal effect".
