Risk factors for type 2 diabetes in women attending menopause clinics in Italy: a cross-sectional study.

OBJECTIVE: To analyze risk factors for type 2 diabetes among women attending menopause clinics in Italy for counselling about the menopause. SUBJECTS: Women attending a network of first-level outpatient menopause clinics in Italy for general counselling about menopause or treatment of menopausal symptoms. METHODS: Cross-sectional study with no exclusion criteria. Type 2 diabetes was defined according to National Diabetes Data Groups Indications and the fasting blood glucose at an oral glucose tolerance test within the previous year. RESULTS: Out of the 44 694 considered in this analysis, 808 had a diagnosis of diabetes type 2 (1.8%). In comparison with women aged < 50 years, the multivariate odds ratios (OR) of type 2 diabetes were 1.31 (95% confidence interval (CI), 0.99-1.74) for women aged 50-52 years, 1.66 (95% CI, 1.27-2.17) at 53-56 years and 2.84 (95% CI, 2.20-3.67) in women aged > or = 57 years. Type 2 diabetes was less frequently reported in more educated women (OR high school/university vs. primary school = 0.44 (95% CI, 0.36-0.55)). Being overweight was associated with an increased risk of type 2 diabetes. In comparison with women reporting a low level of physical activity, the multivariate OR of type 2 diabetes was 0.67 (95% CI, 0.54-0.84) for women reporting regular physical activity. In comparison with premenopausal women, the multivariate OR of type 2 diabetes was 1.38 (95% CI, 1.03-1.84) in women with natural menopause. This finding was present also after allowing for the potential confounding effect of age. The multivariate OR of diabetes for users of hormonal replacement therapy was 0.58 (95% CI, 0.46-0.73). CONCLUSIONS: This large cross-sectional study suggests that postmenopausal women are at higher risk of type 2 diabetes after allowance for the effect of age. Other main determinants of risk of type 2 diabetes in women around menopause were low socioeconomic status and being overweight. Diabetes was found less frequently in those taking hormone replacement therapy.

The effect of menopause on blood lipid and lipoprotein levels. The Icarus Study Group.

There is increasing evidence from epidemiological studies that exogenous estrogen (hormone replacement therapy) protects against the elevated risk of cardiovascular disease in women after the menopause. However, it is still uncertain whether the postmenopausal decrease in endogenous estrogen in itself contributes significantly to this increase in risk. Most of the studies that have provided evidence linking cardiovascular disease with menopause have involved North American women, who may differ significantly from Europeans in terms of lifestyle and diet. ICARUS (Italian Climacteric Research Group Study) is an observational study that involves Italian Menopause Clinics, with the objective of collecting observational data on menopause and its management. The results of a cross-sectional analysis of 9309 women, free from any hormonal treatment and enrolled up to March 1997, are reported here. Data show that the menopause has a marked effect on the circulating levels of lipids and lipoproteins. From pre- to post-menopause there are significant increases in total cholesterol (6.9% before and 4.4% after adjustment for covariates including chronological age, educational level, center, BMI, smoking habits, hypertension and diabetes, previous contraceptive use, and time since menopause), LDL (7.5% before, 4.0% after), and triglycerides (9.0% before, 3.2% (ns) after). However, there is no significant change in HDL. Among postmenopausal women, no effect on lipid profile of time since menopause was observed.

In vitro effects of histamine on human pregnant myometrium contractility.

The effects of histamine on pregnant human myometrium was examined by means of an in vitro technique. Samples of myometrium were taken from women delivered by elective caesarean section at various stages of gestation. Histamine (1 x 10(-6) to 1 x 10(-4) M) had good stimulant activity on all the strips examined. This contractile action was unaffected by H2 blockers but was competitively antagonised by H1-blockers. The H2-selective agonist, impromidine, was ineffective at molar concentrations between 1 x 10(-8) and 1 x 10(-3) while a specific full agonist of the H1 receptors, thiazole in the range of molar concentrations from 1 x 10(-6) to 3 x 10(-4), had an activity identical to that of histamine. The data obtained suggest that in the pregnant human myometrium the activation of H1-receptors is responsible for the contractile effect of histamine; a possible involvement of histamine in the pathophysiology of labour is discussed.

Effect of naltrexone treatment on the treadmill exercise-induced hormone release in amenorrheic women.

The effect of an acute physical stress on hormone secretions before and after a 10-day naltrexone treatment in untrained healthy and amenorrheic women was investigated. Plasma levels of pituitary (LH, FSH, prolactin, GH, ACTH, beta-endorphin) and adrenal (cortisol, androstenedione, testosterone) hormones were measured at rest and in response to 60 min of physical exercise. The test was done both before and after a 10-day naltrexone (50 mg/day) treatment. Graded levels of treadmill exercise (50, 70 and 90% of maximal oxygen uptake (VO2) every 20 min) was used as physical stressor. While mean +/- SE plasma LH levels in control women were higher than in amenorrheic patients and increased following the naltrexone treatment (p < 0.01), no significant differences of basal plasma hormonal levels were observed between amenorrheic and eumenorrheic women, both before and after naltrexone treatment. Physical exercise at 90% VO2 induced a significant increase in plasma GH, ACTH, beta-endorphin, cortisol, androstenedione and testosterone levels in controls before naltrexone treatment (p < 0.01). The mean increase in plasma androstenedione and testosterone levels in control women was significantly higher after naltrexone treatment (p < 0.01). In amenorrheic patients before naltrexone, physical exercise induced an increase in plasma prolactin and GH levels, but not in plasma ACTH, beta-endorphin, cortisol, testosterone and androstenedione. After naltrexone treatment, the exercise induced a significant plasma ACTH, beta-endorphin and cortisol levels, while the increase of plasma prolactin levels was significantly higher than before treatment (p < 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

Postpartum thyroid dysfunction in an Italian population residing in an area of mild iodine deficiency.

We have evaluated the occurrence of postpartum thyroid dysfunction (PPTD) in a group of 372 women residing in area of mild iodine deficiency. Thyroid function and autoimmune status were evaluated by means serum T4, T3, TSH measurement and detecting the presence of positive antithyroglobulin antibodies (AbTg), antimicrosomal antibodies (AbM) and thyroid-peroxidase antibodies (AbTPO) titers in women at parturition, at 1, 3, 6 and 12 months postpartum. New onset transient hypothyroidism occurred in 6.4% of women whereas transient thyrotoxicosis in only 1.8% of women. Transient hypothyroidism was not preceded by thyrotoxicosis as indicated by thyroid function tests and serum Tg concentrations. At parturition, the positivity of AbM and AbTPO titers and the presence of goiter appeared to be a risk factors for the development of PPTD.

Plasma beta-endorphin and beta-lipotropin levels increase in well trained athletes after competition and non competitive exercise.

Plasma beta-endorphin (beta-EP) and beta-lipotropin (beta-LPH) levels were measured in 15 healthy trained marathon runners. These hormones were evaluated in two different conditions: 1-before (1h) and after a marathon race (n = 10); 2-before, during and after a prolonged (90 min) submaximal exercise (bicycle ergometer at 50% VO2 max) (n = 5). In these latter group plasma beta-EP and beta-LPH levels were measured every 15 min for 165 min. In all the athletes, both plasma beta-EP and beta-LPH levels were significantly higher after the end of the marathon race than in basal conditions (p less than 0.01). The prolonged exercise with bicycle ergometer significantly stimulated plasma beta-EP and beta-LPH levels. Starting 60 min after the beginning of the exercise, plasma beta-EP and beta-LPH levels resulted significantly higher than basal values until the end of the exercise (p less than 0.01 at 60, 75 and 90 min). These data confirming that marathon running is a potent stress stimulus, showed that the duration and related factors but not the work load may be considered critical in stimulating beta-EP and beta-LPH release during physical exercise.

Effects of ovariectomy on blood and plasma viscosity, fibrinogen and whole blood filterability.

Blood and plasma viscosity, fibrinogen concentrations and whole blood filterability (VB) were evaluated before surgery and 7 and 30 days postoperatively in 2 groups of subjects, one of which comprised 10 hysterectomized women and the other 14 women who had undergone both hysterectomy and ovariectomy. Oestrogens and various metabolic haematological parameters were measured and correlated with the haemorrheological data. By day 7 after surgery fibrinogen concentrations and plasma viscosity had increased, while VB had decreased. Plasma viscosity and fibrinogen levels correlated negatively with VB in the non-ovariectomized women. Three weeks later almost complete normalization of all parameters was observed in all of the subjects, except for the persistence of low oestrogen concentrations and a further decrease in VB values in the ovariectomized patients, these parameters showing a positive correlation. This study demonstrates that haemorrheological alterations occur after hysterectomy (with or without ovariectomy) in the early post-operative period. The striking decrease in VB values and its correlation with the oestrogen concentrations observed 1 mth after ovariectomy suggest that the fall in oestrogen levels might be responsible for the decrease in whole blood filterability.

[Predictive value of biometry in early pregnancy in comparison with birth weight].

The authors have compared the biometric measurement of the first trimester of gestation (CRL, gestation sac volume, GS vol./CRL ratio) with the respective weights at birth, in order to determine possible correlations. The result have showed no newborns SGA with biometric measurement greater than the 90 degrees percentile in the first trimester of pregnancy, and also no newborns LGA with biometric measurements smaller than the 10 degrees percentile. The authors nevertheless conclude for an insufficient prediction of the parameters considered, cause the high percentage of false positives and false negatives.

[Ultrasound in the study of the functional dynamics of the female genitalia].

The Authors have carried out this survey aiming at having at their disposal biometric indexes to be used in the dynamic and functional study of the female genitalia, since the purely anatomic data cannot be correlated white absolute certainty to the ultrasound data. The survey has been carried out on 42 women of an age between early childhood and late menopause. The patients in their fertile age were examined every other day, from the 10th day of the menstrual cycle; women in menopause or in their pre-puberal age were examined only once, to a total of 158 scans. In each woman the linear and volumetric measurement of the uterus, ovary, ovarian follicle and of the tube has been studied. The results were in conformity with those already reported in the literature.

[Gynecological celioscopy: indications and findings].

The Authors have critically reviewed 1000 consecutive laparoscopies, performed in the Department of Obstetrics and Gynecology, University of Parma. The most common diseases of the genital tract are represented into the population of the patients studied. Conclusions are the safety and the usefulness of the described technique, particularly in discrimination of pelvic pain, pelvic masses and sterility. Concerning the latest field, the importance of ovarian biopsy in distinguishing hypergonadotropic anovulations is confirmed.

Effect of anesthesia and surgical stress on prolactin levels in pregnancy.

The authors examine the variations of PRL plasma levels after the administration of sodium thiopentale 500 mg i.v. to ten patients undergoing voluntary pregnancy interruption between the 8th and the 14th gestational week; PRL levels which were already elevated due to estrogen action, as is physiological in pregnancy, further increased after the anaesthetic drug injection confirming the ability of sodium thiopentale to increase prolactin levels even in estrogen-induced hyperprolactinemic conditions; the possible ways through which the drug may act are then discussed.

Factors associated with climacteric symptoms in women around menopause attending menopause clinics in Italy.

OBJECTIVE: To obtain data on correlates of climacteric symptoms in women around menopause attending menopause clinics in Italy. METHODS: Since 1997 a large cross sectional study has been conducted on the characteristics of women around menopause attending a network of first level menopause outpatient's clinics in Italy. A total of 66,501 (mean age 54.4 years) women are considered in the present paper. RESULTS: The odds ratios of moderate and severe hot flashes/night sweats were lower in more educated women and (for severe symptoms only) in women reporting regular physical activity. Depression, difficulty to sleep, forgetfulness and irritability tended to be less frequent in more educated women and (depression only) in women reporting regular physical activity. Parous women reported more frequently these symptoms. CONCLUSIONS: This large study confirms in Southern European population that low education, body mass index and low physical activity are associated with climacteric symptoms. Parous women are at greater risk of psychological symptoms.
