[Results of the transthoracic fine needle biopsy].

The transthoracic fine needle biopsy is a very efficient method in the differential diagnosis of pathological intraplumonary or mediastinal processes. If the material is representative (out of 918 patients in 71.6 percent), the cytological examination produced a correct result in 93.4 percent (in 614 cases out of 658). In 428 cases of intrathoracic malignant diseases an accurate diagnosis was established in 94.6 percent, and out of the patients with a benign inflammatory disease we could give frequently a hint at the definitive diagnosis in addition with other clinical and paraclinical dates. The false positive rate was 1.3 percent, and in 5.3 percent a false negative result was obtained. Our rate of complications was 16.7 percent.

Biopsy, Needle↗

[The bioptical exploration of non-tuberculous cavities in the lungs].

Considering the difficulties in X-ray differentiating, every lung abscess should be submitted to a bronchologic-bioptical control. In using the fine needle biopsy for the cytological evaluation, no severe complications could be observed so far. A promising therapy of the lung abscess is given by perthoracal local treatment.

Adenocarcinoma, Bronchiolo-Alveolar↗

[Biopsy of suspected metastatic lung lesions].

Among the tumours of the lung metastatic formations are to be expected in 7.5 to 10% of the cases. This is based on the anamnesis, on the X-ray findings or on the results from bioptical examinations. Compared with primary tumours, cytological investigations of the exfoliative material are less effective (positive results in about 50% of the cases). As metastatic formations primarily manifest themselves in the interstitium and only late touch the bronchial system, direct excision of material or methods of puncture respectively are from the first much more promising. Sixty out of 95 patients with pulmonary processes, supposed to show metastatic formations, had been submitted to a bioptical control, mainly by means of fine needle biopsy, with the following cytological evaluation of the material. In 50 cases it was possible to secure the finding morphologically, i.e. in 83.3% of the cases. The cytological appearance, too, leads to a clear position to the primary tumour, for example in hypernephroid carcinomas, pigment-storing melanomas, diseases of the lymphatic system and various sarcomas.

Adenocarcinoma, Scirrhous↗

[Laboratory staged-programme in diseases of the lung. Aspects and recommendation. (author's transl)].

The dynamic of biochemical processes opened recently an enlarged arsenal of new diagnostic possibilities for the physician. The exponential increase of scientific knowledge led to a progressive increase in the frequency of laboratory analyses. First the laboratories attempted to confront this development by means of rationalization of the structure, organization and analytic factors. Today the laboratories are confronted with a wide-spread polypragmasy, the demand of redundant parameters and irrelevant analyses increases. This contradiction can be solved only by a process of interdisciplinary responsibility. Very important is a change with the result of an improvement of the parameterpalettes leading to a rationalization of the diagnosis. Up to now there is a loch of systematical scientific investigations for the objectiveness and improvement of laboratory parameterpalettes concerning basis diagnosis, differential diagnosis and control of therapy effects in lung diseases. Obviously the cause for this problem in the structure and function of the organ "lung". In this special cases laboratory diagnosis possesses inbordinale relevance. As for the diagnostical process parameters of the function as well as X-ray diagnosis and morphological findings possesses priority. In this study an attempt is made to classify the diagnostically relevant parameters and to force them in a staged system of the medical supervision. With this conception the physician has the possibility to demand laboratory investigations on a scientifically founded basis.

3H-estradiol, 3H-testosterone and 3H-dihydrotestosterone localization in the brain of the lizard Anolis carolinensis: an autoradiographic study.

The presence and the neuroanatomical topography of sex hormone concentrating cells in the brain of the American chameleon, Anolis carolinensis have been demonstrated by these experiments. After 3H-estradiol administration large numbers of hormone concentrating cells were found in the amygdala, septum, medial preoptic area, anterior hypothalamic area, the ventromedial and periventricular nuclei of the hypothalamus, and anterior pituitary. In addition, labelled cells were found in the torus semicircularis, in and around the nucleus isthmus pars parvocellularis. A small number of labelled cells could also be found in the rostral pallium, motor nucleus of the fifth cranial nerve, the raphé nuclei, and the spinal cord. After 3H-testosterone or 3H-dihydrotestosterone administration the neuroanatomical pattern was very similar to that found after 3H-estradiol; however, fewer labelled cells were seen after the androgens were given. Two exceptions to the similarity of pattern were in a caudal part of the pallium and in the mesencephalic tegmental area. Hormone-concentrating cells were found after 3H-testosterone or 3H-dihydrotestosterone administration, while labelled cells in these two areas after 3H-estradiol administration were extremely rare. The pattern of hormone-concentrating cells was the same in male and female brains, for each of the hormones. The preoptic area, hypothalamus, and anterior pituitary have been demonstrated in reptiles to be involved in neuroendocrine regulation and in the control of sex behaviors. The presence and neuroanatomical pattern of sex steroid binding cells in the brains of a wide variety of vertebrates have been documented. Large numbers of hormone-concentrating cells were found in all of these species in the medial preoptic area, tuberal hypothalamus, specific limbic structures, the mesencephalon deep to the tectum, and the anterior pituitary. Most hormone-concentrating cells in the brain of A. carolinensis were found in these same brain regions, thus indicating a vertebrate-wide stable core of hormone-concentrating cells in neuroanatomically defined regions.

[Statement of the bioptic diagnostic in intrathoracic sarcoidosis--team work of 6 lung hospitals of the GDR (author's transl)].

A nearly nation-wide team-work of six chest hospitals was made with 4284 patients to analyse all performed diagnostic procedures up to the present. Its frequency and diagnostic importance in relation to the radiographic stages of the disease were examined. Especially the procedure of bronchologic examination, having been partly underrated with the morphological ascertainment of sarcoidosis, are now purposefully analysed. This showed that also with perbronchic punction-biopsy and bronchoscopic excision morphologically confirming of the diagnosis was possible at a high percentage. On the other side the right diagnosis was made clinically before bioptic procedures in the majority of the patients. The overall conclusion is that, according to the experiences made in our hospitals, the relatively harmless bronchologic procedures are in general sufficient to obtain bioptic verfication of the diagnosis "sarcoidosis". Other bioptic operations like mediastinoscopy or lung biopsy can be restricted to cases with reasonable doube of the diagnosis.

Biopsy, Needle↗

[Results of pertracheal punction (author's transl)].

36 pertracheal biopsies with following cytological control have been performed. The results - 21 cases with a confirmed diagnosis give raise to the conclusion, that this method is a suitable supplement of the possible bioptic investigations during the bronchoscopy.
