[Diagnostic value of planar perfusion scintigraphy of the heart using Technetium Tc 99m with methoxy-isobutylisonitrile (MIBI)].

99mTc-MIBI (2 methoxy -isobutylisonitrile) belongs to the new generation of radiopharmaceuticals used for studies of myocardial perfusion. Particularly convenient characteristics of the compound prompted numerous studies of its usefulness for scintigraphic diagnostics of coronary artery disease and myocardial heart infarct (detection, localization, size). In the present paper the diagnostic utility of planar heart scintigraphy with 99mTc MIBI is assessed, the compound had been prepared in the Department of Nuclear Medicine, Medical University of Lódź. The scope of the study included detection of stress induced ischaemia and of permanent perfusion defects, treated as post infarct scars in myocardium. The study was made on 109 patients (78 males, 31 females; age 32-68 years). The first group comprised 56 patients with suspected or diagnosed coronary artery disease, however, without history of previous heart infarct. Results of stress and rest scintigraphy were juxtaposed with those of coronarography, which served as a reference method. In 25 patients the latter revealed critical (greater than 70%) stricture of 1 or 2 coronary arteries. In 31 patients of this group the vessels were either of normal appearance or only marginally stricture. Sensitivity, specificity and accuracy of the scintigraphy in diagnosis of ischaemia due to critical narrowing of coronary arteries amounted to 80, 84 and 82 per cent, respectively. Localisation of perfusion defects was adequate in 20 out of 31 vessels (64 per cent). The second group of patients included 53 individuals with previous heart infarct (25 and 11 had a transmural and nontransmural infarct of the anterior wall, respectively; in 12 and 5 a transmural and nontransmural infarct of the inferior wall was diagnosed, accordingly).(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluating patients' warfarin requirements.

Seventy consecutive patients receiving warfarin as an anticoagulant were evaluated for the relationship between the response to the loading dose of warfarin and the response to the maintenance dose. No patients were excluded because of complicating diseases or the concurrent use of other drugs. There was a moderate correlation (r = 0.54, p less than 0.01) between response to the loading dose and response to the maintenance dose. Thus, a relatively weak response to a loading dose of warfarin can be used to predict a need for a relatively large maintenance dose in unselected patients requiring anticoagulant therapy. Age seems to be a relatively weak determinant of warfarin sensitivity. Other factors such as the genetic control mechanism, concurrent drug therapy, and complicating diseases apparently are more important determinants.

Birth parents who relinquished babies for adoption revisited.

The fact that adoption records may be opened by court decree to enable adoptees to have access to identifying information about their birth parents makes it incumbent upon those concerned with adoption practices to study the impact of this on adoptees, adoptive parents, birth parents, and professional practice. This paper reports on research addressed to the attitudes and feelings of birth parents years after they relinquished babies for adoption.

The effects of the sealed record in adoption.

The authors found that 40 of 50 adult adoptees who had reunions with their birth parents found the experience satisfying. Only 10% of the birth parents reacted adversely, although negative response were somewhat more common among the adoptive parents. These findings confirm the author's belief that adoption practices, which in all but 4 states include permanent sealing of birth records, should be changed to recognize the life-long nature of adoption. Their recommendations include opening the records for adult adoptees, creation of agencies to be available to provide assistance and counseling for all involved (adoptees, their adoptive parents, and birth parents), and consideration of new adoption methods that would not require biological parents to forever relinquish their child and all knowledge of him/her at adoption.
