Physiological and haematological consequences of a novel parasite on the red-rumped swallow Hirundo daurica.

Parasite virulence has been hypothesised to increase with the degree of host sociality because highly social hosts have a greater probability of encountering horizontal transmission of parasites and experiencing infections with multiple strains of the same parasites than do solitary hosts. As compared with the defences of closely related social host species, we predicted that solitary hosts should have relatively weak defences against parasites, thus being relatively more affected when parasitised by a novel parasite. We tested this prediction by either experimentally infesting 12 nests of the solitarily nesting red-rumped swallow Hirundo daurica with 50 individuals of the generalist martin bug Oeciacus hirundinis or by fumigation of nine nests. Nestlings 13 days old from the parasite addition group experienced increased mortality, attained lower body mass and tended to have shorter tarsi compared to nestlings from fumigated nests. Surprisingly, nestlings from the parasite addition group had higher packed cell volume (cellular fraction of blood) and lower levels of heat shock proteins (HSP60) than nestlings from the fumigation group. A measure of immunocompetence was not significantly affected by treatment, but its magnitude was positively related to packed cell volume and negatively related to level of HSP60. Solitary hosts like the red-rumped swallow have weak immune responses and low levels of heat shock proteins when infested with ectoparasites while highly social hosts have strong immune responses and high levels of heat shock proteins when infested. These findings partially support the hypothesis that potential host species with weak defences are more susceptible to infection and the deleterious effects of evolving parasites than potential hosts with strong defences.

Sialic acid-dependent binding of baculovirus-expressed recombinant antigens from Plasmodium falciparum EBA-175 to Glycophorin A.

The Plasmodium falciparum Erythrocyte Binding Antigen-175, EBA-175, is a soluble merozoite stage parasite protein which binds to glycophorin A surface receptors on human erythrocytes. We have expressed two conserved cysteine-rich regions, region II and region VI, of this protein as soluble His-tagged polypeptides in insect cell culture, and have tested their function in erythrocyte and glycophorin A binding assays. Recombinant region II polypeptides comprised of the F2 sub-domain or the entire region II (F1 and F2 sub-domains together) bound to erythrocytes and to purified glycophorin A in a manner similar to the binding of native P. falciparum EBA-175 to human red cells. Removal of sialic acid residues from the red cell surface totally abolished recombinant region II binding, while trypsin treatment of the erythrocyte surface reduced but did not eliminate recombinant region II binding. Synthetic peptides from three discontinuous regions of the F2 sub-domain of region II inhibited human erythrocyte cell binding and glycophorin A receptor recognition. Immune sera raised against EBA-175 recombinant proteins recognized native P. falciparum-derived EBA-175, and sera from malaria-immune adults recognized recombinant antigens attesting to both the antigenicity and immunogenicity of proteins. These results suggest that the functionally-active recombinant region II domain of EBA-175 may be an attractive candidate for inclusion in multi-component asexual blood stage vaccines.

Cross-cultural variation in disease-related concerns among patients with inflammatory bowel disease.

OBJECTIVE: The aim of this work was to study cross-cultural variations in the impact of inflammatory bowel disease (IBD) on health-related quality of life by an international comparison of disease-related concerns. METHODS: Item and factor scores on the Rating Form of Inflammatory Bowel Disease Patient Concerns and overall mean concern levels were compared by analysis of variance among 2002 IBD patients in eight countries. RESULTS: The overall level of concern varied from 51 out of 100 in Portugal to 19 in Sweden, with intermediate scores for Italy (43), Canada (40), United States (39), France (39), Austria (33), and Israel (25). Having surgery, an ostomy, the uncertain nature of the disease, and medication side effects were each rated among the first five in importance in six countries. Other items varied considerably. For example, concern regarding pain and suffering was high in Israel and low in Portugal, whereas concern over developing cancer was low in Italy. Concern over financial issues and access to high-quality health care were inversely associated with measures of national economic prosperity. CONCLUSIONS: 1) Cross-cultural comparisons of patient concerns related to IBD are feasible using translated scales. 2) Reporting tendencies vary greatly; within Europe, patients from southern countries report greater overall concern. 3) The complications and the variable evolution of disease elicit general concern, but the importance of specific issues varies among countries. 4) The reasons for national differences may have social, cultural, and/or economic determinants with relevance to the patient-physician relationship, patient education, and therapeutic decision making.

Purification, characterization, and immunogenicity of a disulfide cross-linked Plasmodium vivax vaccine candidate antigen, merozoite surface protein 1, expressed in Escherichia coli.

The Plasmodium vivax merozoite surface protein 1 (MSP-1) 42-kDa fragment (PvMSP-1 p42) is a promising vaccine candidate antigen against the blood stage of the malarial parasite. We have developed a process for the production of this vaccine target, keeping in mind its use in human volunteers. A novel strain, Origami(DE3), of Escherichia coli with mutations in the glutathione and thioredoxin reductase genes yielded 60% more soluble PvMSP-1 p42 than the conventional E. coli BL21(DE3) strain. Recombinant PvMSP-1 p42 was purified to > or = 99% purity with a rapid two-step protocol designed for easy scaling up. The final product had a low endotoxin content and was stable in its lyophilized form. PvMSP-1 p42 was found to have the predicted primary and tertiary structures and consisted of a single conformer containing one free cysteine, as predicted. The product was recognized by conformational monoclonal antibodies against P. vivax MSP-1. Immunogenicity studies of PvMSP-1 p42 were carried out with two strains of mice and the adjuvants Montanide ISA51 and Montanide ISA720. Both formulations were found to induce high levels of immunoglobulin G1 (IgG1), IgG2b, and IgG2a antibodies along with low levels of IgG3. Lymphocytes from animals in all the PvMSP-1 p42-immunized groups showed proliferative responses upon stimulation with PvMSP-1 p42; the cytokines interleukin 2 (IL-2), gamma interferon, IL-4, and IL-10 were detected in the culture supernatants. These results indicate that PvMSP-1 p42 in combination with both of the adjuvants elicited cellular and humoral responses in mice.

Analysis of human antibodies to erythrocyte binding antigen 175 of Plasmodium falciparum.

Invasion of human erythrocytes by Plasmodium falciparum merozoites is a multistep process. For many strains of the parasite, part of this process requires that the erythrocyte binding antigen 175 (EBA-175) of the merozoite binds to sialic acid residues of glycophorin A on the erythrocyte surface, a receptor-ligand interaction which represents a potential target for inhibition by antibodies. This study characterizes the reactivity of naturally acquired human antibodies with four recombinant proteins representing parts of EBA-175 (region II, regions III to V, and the dimorphic C and F segment region) in populations in which the organism is endemic. Serum immunoglobulin G (IgG) recognizing the recombinant proteins is predominantly of the IgG1 and IgG3 subclasses, and its prevalence increases with age. In a large population study in The Gambia, serum positivity for IgG or IgG1 and IgG3 subclass antibodies to each of the EBA-175 recombinant antigens was not significantly associated with subsequent protection from clinical malaria. However, there was a trend indicating that individuals with high levels of IgG to region II may have some protection.

[Risk behavior among Brazilian Military conscripts, 1998: an study of HIV infections following socioeconomic differences].

A study of HIV-related risk behavior was carried out in 1998 among Brazilian military conscripts aged 17-20 years. A sample of 30,318 subjects was selected in three strata, pertaining to counties from: 1) the North and Central-West (N/CW); 2) South (S); and the states of Rio de Janeiro and São Paulo. HIV prevalence rates were estimated in all strata. The objective of this paper was to analyze the results according to differences in socioeconomic status (SES). The statistical analysis used an index of sexual risk behavior and logistic regression models. The N/CW stratum showed the worst indicators for SES, sexual risk behavior, and sexually transmitted infections (STIs), as well as the highest HIV seroprevalence rate. The best indicators for all variables were found in the RJ/SP stratum. The South showed intermediate results. Level of schooling also played a relevant role. In all three strata the conscripts with an incomplete high school education displayed the worst sexual risk behavior index, shown to be a relevant predictor of STI-related problems, including HIV infection.

Determination of tetracycline and its major degradation products by liquid chromatography with fluorescence detection.

A liquid chromatographic method of tetracycline and its major degradation products on a C8-reversed phase column with acidic mobile phase and fluorescence detection is described. The quantification limit, measured as the amount of sample that gave a signal ten times the peak-to-peak noise of the baseline, was: 0.25 ng for tetracycline (TC) and epitetracycline (ETC), 25 ng for and 4-epianhydrotetracycline (EATC) and 50 ng for anhydrotetracycline (ATC) of injected standard. By means of this liquid chromatography (LC) assay TC, ETC, EATC and ATC as main degradation products of tetracycline, can be separated and determined with good sensitivity and specificity within 15 min.

Role of the spleen on the pathogenesis of schistosomal periportal (pipestem) fibrosis of the liver: an experimental approach.

Splenectomized and sham-operated mice with chronic Schistosoma mansoni infection were compared for the development of periportal fibrosis (pipestem fibrosis). Although this lesion appeared less frequently in splenectomized mice, it also developed in the absence of the spleen. The time of splenectomy, spleen weight, presence of anti-idiotypic antibodies, the number of eggs in the liver, and the type and size of periovular granulomas in the liver as evaluated by computerized morphometry did not show statistically significant differences between the two groups. It is concluded that the role of the spleen in the development of pipestem fibrosis seems ancillary and that multifactorial influences, including worm burden, hepatic vascular adjustment, and factors associated with the biology of extracellular matrix of the liver, probably play a more significant role.

Baculovirus-mediated expression of Plasmodium falciparum erythrocyte binding antigen 175 polypeptides and their recognition by human antibodies.

The erythrocyte binding antigen EBA-175 is a 175-kDa Plasmodium falciparum protein which mediates merozoite invasion of erythrocytes in a sialic acid-dependent manner. The purpose of this study was to produce recombinant EBA-175 polypeptide domains which have previously been identified as being involved in the interaction of EBA-175 with erythrocytes and to determine whether these polypeptides are recognized by malaria-specific antibodies. The eba-175 gene was cloned by PCR from genomic DNA isolated from the 3D7 strain of P. falciparum. The predicted protein sequence was highly conserved with that predicted from the published eba-175 gene sequences from the Camp and FCR-3 strains of P. falciparum and contained the F segment divergent region. Purified recombinant EBA-175 polypeptide fragments, expressed as glutathione S-transferase fusion proteins in insect cells by using the baculovirus system, were recognized by antibodies present in serum from a drug-cured, malaria-immune Aotus nancymai monkey. The fusion proteins were also recognized by antibodies present in sera from individuals residing in areas where malaria is endemic. In both cases the antibodies specifically recognized the EBA-175 polypeptide portion of the fusion proteins. Antibodies raised in rabbits immunized with the recombinant fusion proteins recognized parasite proteins present in schizont-infected erythrocytes. Our results suggest that these regions of the EBA-175 protein are targets for the immune response against malaria and support their further study as possible vaccine components.

Mapping in pig of genes involved in sexual differentiation: AMH, WT1, FTZF1, SOX2, SOX9, AHC, and placental and embryonic CYP19.

Intersexuality has been reported in pigs and investigations are in progress to identify and physically map the genes involved in sex-reversal. In this study we have mapped on porcine chromosomes seven genes which might be implicated in this developmental pathway. Four genes were mapped by radioactive in situ hybridization: AMH (Anti-Müllerian Hormone) and WT1 (Wilms' Tumor gene 1) were both mapped to pig chromosome 2 (SSC2) in the q14-->q21 and p14-->q11 regions, placental CYP19 (cytochrome P450, subfamily XIX) and FTZF1 (fushi tarazu factor [Drosophila] homolog 1, alias SF1 [steroidogenic factor 1]) to pig chromosome 1 (SSC1) in the q14-->q17 and q210-->q211 regions respectively. Four other genes were regionally located by PCR analysis on a cytogenetically characterized porcine somatic cell hybrid panel: a second CYP19 gene (expressed in embryo) was mapped to porcine 1q12-->q17, AHC (alias DAX1, adrenal hypoplasia congenital) to porcine Xp24, SOX2 and SOX9 (SRY sex determining region Y-box 2 and 9) to 13q23-->q41 and 12p13-->p11 respectively. These results are in global agreement with mapping data available in other mammalian species.

[Chorioangioma of the placenta].

Chorioangioma is the most frequent nontrophoblastic tumour of the placenta, with a reported incidence ranging from 0.01% to 1.3%. The authors describe three cases of placental chorioangiomas, two of these diagnosed prenatally by ultrasound, and the clinical implications for both mother and fetus. Premature delivery and hydramnios were the obstetric complications associated with these vascular tumours. We confirmed this diagnosis by histologic examination, in all cases.

[Psychosomatic syndromes in clinical practice. A proposal of diagnostic criteria].

The following is a translation of the proposed diagnostic criteria established by a European group for twelve common psychosomatic syndromes found in general clinical practice. Many of them have the potential to occur during different stages of a large variety of diseases and their detection could lead to better holistic provision of health care.

[The alcohol-dependent outpatient: a clinical typology].

The aim of this study was to construct a typology integrating sociodemographic, clinical and personality data of alcohol-dependent patients. We studied a consecutive population attending an alcohol-dependent outpatient psychiatric department, assessed by instruments that measured psychiatric, psychopathological, personality and sociodemographic characteristics of alcohol-dependent patients. The factorial analysis allowed us to define three main expressions of alcoholism: physical, psychological and social. In the elicited typology, both physical and social appear mixed in their expression. The psychological expression is divided into two main categories: the alexithymic (with a depressive personal and family history) and the psychopathologic type (with actual depression and anxiety). This typology should allow us to correlate these types with neurophysiological and biochemical patterns and could be particularly useful in building an etiologic model of alcohol dependence which could contribute to a better prediction of prognosis and choice of the best therapeutic intervention.

[Education in liaison psychiatry in the psychiatric internship].

The development of consultation-liaison psychiatry in Portuguese general hospitals raises the need for specific training to be included in the preparation of psychiatric trainees. In order to contribute to a better quality of liaison psychiatric training we propose guidelines for this training period. We describe the aims of a training period, the core curriculum, the structure of this training and provide a bibliography. We also propose a systematic evaluation profile for psychiatric trainees and supervisor.

[Dermatitis artefacta].

We report a case of a 29-year-old man presenting skin ulcerations on both sides of the mandible. The diagnosis of dermatitis artefacta was based on the morphology and evolution of the lesions, on the patient's borderline personality, on the objects found in his possession, and a later admission of having an involvement in the aggravation of the lesions.

Reliability of environmental, biological and health characteristics of infants from a developing country.

BACKGROUND: Although most epidemiological data are gathered by interviews, few studies ascertain their reliability. This study quantified inter-observer reliability of environmental, biological and health characteristics of underprivileged children, in a southern Brazilian state. METHODS: Five health care professionals who had received extensive standardized interview and observation training interviewed mothers and observed the home environments of a random sample of 102 children. A second interview was conducted by a sixth health professional who had received separate but identical training without the initial interviewer knowing a follow-up interview would be conducted. Data from the two independent interviews and observations were compared for agreement using the kappa statistic. RESULTS: There was excellent agreement (kappa > 0.75) for most of the household characteristics, such as type of walls, ceiling, floor and windows in spite of the diversity of buildings. Presence of the parents, number of people in the house-hold, age of the mother and health assistance were reliable as well. However, information about skin colour of the mother and the number of cracks in the house only reached fair agreement. CONCLUSIONS: These results showed that data gathered by observation and interviews generate accurate information about environmental, biological and health care characteristics. Those based on written information, such as birth date, were highly concordant.
