Diazepam facilitates acceptance of alien lambs by postparturient ewes.

The aim of this study was to determine the effect of diazepam on the behavior of parturient ewes towards alien lambs. There is evidence that benzodiazepines cause behavioral changes during the lactation period in rats. In two independent experiments, it was found that the ewes acceptance of alien lambs significantly increased following a single injection of the benzodiazepine, diazepam, given either 1 or 12 h after parturition. In a third experiment, in which the alien lamb was not permitted to suckle during a period of 2 h after the injection, the diazepam treatment did not provoke significant differences in maternal behavior of the ewes, although in the ewes treated with diazepam, suckling clearly tended to increase. At the dose employed (20 mg), administered 1 h after birth, diazepam caused no signs of sedation as assessed in an open-field test carried out 3 h after parturition. This doesn't eliminate the possibility of diazepam having a sedative effect in the period of 2 h immediately after its administration and before the test and, in this way, facilitating suckling which could be responsible for the maternal behavior observed after this period. As diazepam gives rise to an enhanced GABAergic activity in the brain, these observations suggest that a GABAergic mechanism could also play a role in the process whereby ewes form a selective bond with their own offspring.

[The orthosurgical correction of 2 cases of Angle class II div I].

An orthosurgical technique comprising maxillary expansion using Burstone screws associated with multiple corticotomy, repositioning of the bone basis and pre- and postoperative orthodontics was used to correct two cases of Angle's class dysgnathia with involvement of both the bone basis and alveolodental component. The outcome was satisfactory in that a good esthetic result was achieved together with normal and functional occlusion which was stable over time.

Radiologic-prosthetic planning of the surgical phase of the treatment of edentulism by osseointegrated implants: an in vitro study.

An original prosthetic planning involving CT scan is described; it provides the surgeon with a template for use during the surgical phase of osseointegrated implant treatment. The final prosthodontic reconstruction was planned in a preliminary in vitro study. The CT scan provided a three-dimensional study of the underlying bone, to determine the position, angulation, and depth of the fixture to be used. The specially developed "positioner" decreased the possibilities of error in perforating the mandible. A second CT scan verified whether there were errors in positioning the fixtures. The satisfactory results and the predictability of the final rehabilitation led to the adoption of this method in a clinical research program on human patients.

[Craniofacial fractures].

Results of early combined maxillo-facial and neurosurgical treatment of 53 craniofacial fractures are referred. The fracture location was in 31 cases central midfrontal, 10 lateral supraorbital and 12 combined central and lateral fractures. 35 fractures interested the floor and the posterior wall of frontal sinus, lacerating the underlying dura and cortical tissue. In 19 fractures orbital displacement was present. The key points in the management of these patients are: 1) Early (within 1 to 5 days) and one stage neurosurgical-maxillofacial procedure. Immediate intervention is indicated only in case of evolutive neurological lesions; 2) wide exposition of all the injuries through bicoronal incision and bone flap; 3) assessment of fractures pattern and amount of bone loss; 4) reconstruction of craniofacial frame with osteosynthesis and autologous bone grafts (35 cases iliac crest, 7 split calvarial graft); 5) interosseous wiring is used in sutured mosaic, small bone fragments and intraoperative temporary fixation; miniplates are used for rigid fixation of craniofacial pillars; 6) for optimal cosmetic result reconstruction of supraorbital ridge, nasoglabellar region and zygomatic arch is essential; 7) fractures involving the sinus floor, posterior wall and the nasofrontal duct result in direct communication between the nose and intracranial cavity with high risk of infection and mucocele formation. Cranialization of the sinus removing the posterior wall and all the mucosa is mandatory. The nasofrontal duct, the floor and sinus dead space are obliterated with autologous bone chips. Osteoneogenesis occurred in all the cases.

[Control of the condylar fragment position in interventions for sagittal osteotomy of the mandible].

The question of checking the position of the condylar fragment in sagittal osteotomy of the jaw is still controversial. The author describes an original device used by the Department of Maxillo-Facial Surgery at the University of Turin which allows precise replacing of condylar fragments. This enables a semirigid fixing of the jaw and rapid functional recovery with long-term stability of the new therapeutic position.

[Segmental surgery of the maxilla: the long-term assessment of the dental-periodontal integrity of the osteotomized sites].

Segmentary surgery of the jaws has increased the possibilities of therapeutic orthognathodontic surgery, allowing osteo-dental segments to be moved three-dimensionally to achieve a satisfactory occlusion. One of the potential limits of this method is the possibility of damaging the dental-periodontal structures bordering with the site of osteotomy. The aim of this study was to report the results of a long-term clinical and radiological survey in a group of patients undergoing segmentary surgery, performed without tooth extraction.

[Clinico-surgical evaluation of dental elements in bone inclusions].

Dental inclusions are assessed on the basis of clinical and operating statistics relating to the years 1981-1985 and comprising 815 patients and 1445 treated teeth. The results confirm the predominance of inclusions among female patients and the prevalence of the mandibular site; the most frequently included teeth are the lower eighths followed by the upper eighths, the upper canines and the second lower premolars. Inclusions are bilateral in most cases. These findings are in substantial agreement with the epidemiological data recorded in the literature. The operating series shows that the type of surgical treatment varies with the localisation of the tooth (anterior or posterior), its relationship with contigous teeth and with the surrounding anatomical structures and the age of the patient. Teeth in the posterior sector are usually extracted. At young age, germectomy is carried out for orthodontic purposes; at mature age, teeth are extracted to prevent the formation of periodontal pockets and the aggravation of caries and their complications. On the other hand, for teeth in the anterior sector, surgery is usually merely the first stage in treatment and is followed by treatment of orthodontic type with traction, in view of the important aesthetic and functional role of the labial teeth.

Relationship between postvaccinal anti-influenza antibodies, blood magnesium levels, and HLA antigens.

Seventy-eight healthy subjects belonging to 16 different families were submitted to an anti-influenza vaccination. The antibody titers and the red blood cell and plasma Mg concentrations were determined before and 30 days after vaccination. The population study performed on 32 subjects showed the occurrence of a higher antibody response (P less than 0.01) and a lower red blood cell Mg level, among the Bw35 individuals. These findings are confirmed by family studies: HLA identical sibs have values much closer to those of the propositi than to those of the HLA different sibs. The relationships between HLA, immune response, and Mg revealed by the present investigation are discussed in light of the literature together with the known associations between HLA Bw35 antigen and diseases.

[An evaluation of the postoperative stability of the dental arches following interventions for segmental osteotomy].

The study evaluates the long-term postsurgical stability of segmentary osteotomy and compares it to the orthodontic expansion of the jaw which is well known for its tendency to relapse following contraction. The study of the postsurgical changes of the transversal and longitudinal diameters of the dental arches and of the area covered was carried out in a group of 15 patients using rigorously standardised methods. Heterogeneous variations were observed and although there was a prevalent tendency towards contractions these were small in size and of minor clinical importance. Even if only major postsurgical changes were taken into account, usually following considerable expansion of the jaw, especially if associated with the jaw's advance, it is clear that there is a greater clinical stability in comparison to orthodontic expansions. This confirms the therapeutic value of segmentary osteotomy.

[Anatomo-radiological evaluation of the long term results of surgery of large odontogenic cysts].

Long term surgical results in 47 cases of great size odontogenic cysts are examined with an anatomo-radiological method. The reparative osteogenetic process of these great cysts with mandibular localization are compared with those with maxillary localization, following anatomo radiological and clinical-radiological methods. These results are also examined according to the surgical techniques used.
