["Hospital tourism". Case report of Munchausen syndrome].

We report on a 29-year old patient, who was treated as an inpatient in 95 different hospitals (altogether 173 times) within 7 years. The gynaecologist can suspect Münchausen's syndrome, if the following aspects are recorded: dramatic clinical admissions patterns, (frequently out of the normal consultation time); a history of several operations and many short stays in different hospitals. Special care in diagnostics should be taken to avoid further unnecessary surgical interventions.

Coping strategies and course of disease of breast cancer patients. Results of a 3-year longitudinal study.

In a prospective 3-year longitudinal study investigating correlations between coping strategies and course of breast cancer a consecutive series of 107 patients were assessed for biological and psychosocial data. Data analysis indicated no significant correlations between coping strategies and course of cancer. On the other hand, biological parameters such as size of tumor and lymph node stage at time of surgery correlated significantly with the course of disease. It was concluded that the assessed indicators of coping are of little importance in regard to the course of the disease and less predictive compared with somatical parameters of breast cancer.

[How do breast cancer patients experience participation in a psycho-oncologic follow-up study?].

62 breast cancer patients participating in a prospective psychooncological research project were asked about their attitude toward the study after a one year follow-up period. 40 percent of the women had an indifferent opinion about the study. They felt neither burdened nor unburdened by their collaboration on the study. 32 percent had a positive and 28 percent a negative attitude toward the study. Patients with a positive attitude were more often singles and were more often in unfavourable stages of their cancer when entering the study. However they felt the interviews and rating scales rather helpful for coping with their disease. The women with a negative attitude were more often burdened by psychological problems already before the diagnosis of cancer was made and showed more signs of depression, distrust and cognitive withdrawal in their coping strategies. The results indicate that in psychooncological prospective studies the development of patients' attitude toward the study should be considered more carefully.

[Ulcer disease. Studies of patients with chronic recurrent duodenal ulcer in an attempt to differentiate subgroups based on age at disease onset, psychopathologic and biographical data].

A group of 47 patients suffering from chronic recurrent duodenal ulcers was subdivided into two groups according to whether the first manifestation of the disorder occurred early or late in the life of each individual. A comparison of the two groups revealed that the patients in group I (early manifestation) had a larger number of constitutional handicaps along with a higher incidence of dispositional prior experience with regard to object loss and that they assumed social responsibility at an early age. Group II (late manifestation) was characterized by a larger number of depressive psychoses, a higher incidence of alcohol abuse, and by attempts at suicide. Apart from this, the patients in group II often complained of muscular pain syndromes. Patients in group I frequently exhibited the characteristics of the ulcer type described by Alexander as well as chronic anger. Patients in group II most often had the personality structure of so-called psychosomatic patients or suffered from depressive disorders affecting their personalities and from chronic anxiety. The two characteristics which were most typical for recidivation were: 1. Actualization of experienced bereavement and 2. unspecific activation as a result of the will to assert themselves in stress situations. In 41% of the cases there was evidence of somatic factors as e.g. starvation, abuse of alcohol or abuse of analgetics. Long-term prophylaxis has been effected by psychopharmacological agents as well as by psychotherapeutic techniques.

[Effect of the dopamine antagonist metoclopramide on uterine contraction].

BACKGROUND: Because of its activity on the gastro-intestinal tract metoclopramide is used also in pregnancy, though nothing is known about its effect on the pregnant uterus. Therefore it was of interest to look after a change in uterine activity after the application of metoclopramide in a clinical relevant dose. MATERIALS AND METHODS: 24 strips from pregnant uteri were removed during a cesarean section, brought into an organ chamber and changes of spontaneous activity after the application of metoclopramide were measured. RESULTS: A highly significant decrease of uterine activity was observed. CONCLUSIONS: It was demonstrated that metoclopramide, in contrast to stomach and ureter, exerts not a tonifying but a relaxant effect on the uterus.

[Effect of Listeria on contractibility of human uterine muscle].

In Austria the prevalence of listeriosis is 2.6 cases per million inhabitants yearly, hence rather rarely the cause of spontaneous abortion or premature birth. On the other hand, Listeria monocytogenes is found in 1% of the asymptomatic population as a component of stool flora. Since the cause of premature labour contractions remains unclear in about half of all cases, we examined 29 listeria strains for their ability to cause myometrial contraction by direct contact using an in-vitro uterine strip-model. Seven of nine L. monocytogenes strains were able to cause contractions; contractions were not inducible by an nonhaemolytic mutane (SLCC 53, avirulent) nor by a rough strain (SLCC 5779, only slightly virulent). Three of six L. ivanovii isolates also exhibited the ability to induce contractions. None of the apathogenic species (L. innocua, L. seeligeri, L. welshimeri, L. grayi and L. murrayi) was capable of activating contractions in our in-vitro model. Only L. monocytogenes and L. ivanovii cause conjunctivitis after being dropped in rabbit's eyes (positive Anton Test). The influence of listeria on uterine activity as found in our in-vitro model thus correlates with the classical pathogenicity test. We consider these in-vitro results as an additional argument to oppose the presence of L. monocytogenes in ready-to-eat foods.

[Effect of interleukin 6 on uterine contractility in the human in vivo].

The influence on uterine contractility of inflammatory cytokines like interleukin-6 is widely unknown. We were able to demonstrate in 16 human myometrial strips obtained during caesarean section in the organ chamber a highly significant (P < 0.001) increase of the activity of uterine strips after addition of IL-6. This further emphasizes the importance of subclinical infections for the etiology of so called idiopathic preterm labour.

[Induction of uterine contractions by Shigella].

The uterus-contracting properties of Shigellae in a clinically relevant dose of 10(5) organisms per ml was investigated in 17 uterine strips which where dissected during caesarean section from the lower uterine segment. A highly significant (p < 0.001) increase in uterine activity was observed.
