[Xanthogranulomatous pyelonephritis in infancy and childhood. Report of two cases].

We are reporting 2 cases of xanthogranulomatous pyelonephritis in boys of 7 and 2 year-old. The inflammatory lesion had extended to perirenal tissues and appeared as a lumbar abscess in case 1. The kidneys affected were diffusely compromised by the lesion. Both cases showed Proteus mirabilis after microbiologic cultures. Definitive diagnosis was done by pathology examination. Nephrectomy was curative.

The importance of endoscopy in the diagnosis of neoplasms associated with diverticular disease of the colon, and its effect on surgical treatment.

The results of fibercolonoscopy performed in 149 patients with clinical and or X-ray appearances of diverticular disease are reported. Examination could not be completed in 16 per cent of patients because of impassable strictures in the left colon. Concurrent lesions were diagnosed in 69 per cent, and carcinoma was found in 6 per cent. The respective role of endoscopy and radiology are compared in the diagnosis of polyps, strictures, and cancer, coexisting with diverticulosis. It is concluded that colonoscopy is always indicated in diverticulosis with symptoms because it often provides a precise differential diagnosis thus obviating useless operations. Surgery is essential when impassable strictures prevent colonoscopy because the underlying conditions always require surgical treatment.

Histology of bronchial carcinoma and regional lymph nodes as putative immune response of the host to the tumor.

Two hundred forty-two patients who had bronchial carcinoma and who underwent radical surgery were studied in order to determine putative host resistance to the tumor at histologic level, i.e., lymphocytic infiltration in the center and around the tumor, together with sinus histiocytosis and follicular hyperplasia in the regional lymph nodes. These features were evaluated in a semiquantitative way, giving rise to three groups of patients: D- (reaction absent or poor), D+ (reaction present), and D++ (strong reaction present). Five-year survival rates and the incidence of metastases in regional nodes were significantly related to the putative host resistance against the tumor, but no clear-cut correlation between grade of malignancy of the tumor and histologic type was evidenced. The significance of these reactive changes is fully discussed.

Host resistance in relation to survival in breast cancer.

The degree of lymphocytic infiltration in and around breast tumours together with sinus histiocytosis and follicular hyperplasia in regional lymph nodes has been studied in 310 cases of breast cancer treated with standard radical mastectomy. The presence of these features was regarded as evidence of host resistance against the tumour and made possible the division of patients into three classes-no or poor reaction, good reaction, and strong reaction. The grading was shown to have a close correlation with prognosis. The relationship between host defensive factor grading and malignancy, nodal metastases, and survival was also examined. The results support the hypothesis that prognosis in breast cancer is closely related to a histological picture of cell-mediated immunity against the tumour.
