Immunogold-labeled S-phase neoblasts, total neoblast number, their distribution, and evidence for arrested neoblasts in Macrostomum lignano (Platyhelminthes, Rhabditophora).

Neoblasts in Platyhelminthes are the only cells to proliferate and differentiate into all cell types. In Macrostomum lignano, the incorporation of 5'-bromo-2'-deoxyuridine (BrdU) in neoblasts confirmed the distribution of S-phase cells in two lateral bands. BrdU labeling for light and for transmission electron microscopy (TEM) identified three populations of proliferating cells: somatic neoblasts located between the epidermis and gastrodermis (mesodermal neoblasts), neoblasts located within the gastrodermis (gastrodermal neoblasts), and gonadal S-phase cells. In adults, three stages of mesodermal neoblasts (2, 2-3, and 3) defined by their ultrastructure were found. Stage 1 neoblasts where only seen in hatchlings. These stages either were phases within the S-phase of one neoblast pool or were subsequent stages of differentiating neoblasts, each with its own cell cycle. Regular TEM and immunogold labeling provided the basis for calculating the total number of neoblasts and the ratio of labeled to non-labeled neoblasts. Somatic neoblasts represented 6.5% of the total number of cells. Of these, 27% were labeled in S-phase. Of this fraction, 33% were in stage 2, 46% in stage 2-3, and 21% in stage 3. Immunogold labeling substantiated results concerning the differentiation of neoblasts into somatic cells. Non-labeled stage 2 neoblasts were present, even after a 2-week BrdU exposure. Double labeling of mitoses and FMRF-amide revealed a close spatial relationship of mesodermal neoblasts with the nervous system. Immunogold-labeled sections showed that nearly 70% of S-phase cells were in direct contact or within 5 microm from nerve cords.

Two novel glycosides from the fruits of Morinda citrifolia (noni) inhibit AP-1 transactivation and cell transformation in the mouse epidermal JB6 cell line.

The fruit juice of Morinda citrifolia (noni), a plant originally grown in the Hawaiian and Tahitian islands, has long been used by islanders to treat diseases, including cancer. Two novel glycosides, 6-O-(beta-D-glucopyranosyl)-1-O-octanoyl-beta-D-glucopyranose and asperulosidic acid, extracted from the juice of noni fruits, were used to examine their effects on 12-O-tedtradecanoylphorbol-13-acetate (TPA)- and epidermal growth factor (EGF)-induced AP-1 transactivation and cell transformation in mouse epidermal JB6 cells. The results indicated that both compounds were effective in suppressing TPA- or EGF-induced cell transformation and associated AP-1 activity. TPA- or EGF-induced phosphorylation of c-Jun, but not extracellular signal-regulated kinases or p38 kinases, was also blocked by the compounds, indicating that c-Jun N-terminal kinases were critical in mediating TPA- or EGF-induced AP-1 activity and subsequent cell transformation in JB6 cells.

Altered myosin light-chain phosphorylation in resting platelets from premenopausal women with diabetes.

Gender-related differences in the rate of coronary heart disease (CHD) between premenopausal women and men are greatly diminished in women with diabetes mellitus (DM). This may be related, in part, to altered platelet function in premenopausal diabetic women. Hyperglycemia may contribute to increase platelet aggregation through enhancement of oxidative stress, increased nitric oxide (NO) destruction, and increased myosin light-chain (MLC) phosphorylation (MLC-P). Accordingly, we investigated functional and biochemical parameters of platelet function in 32 women (14 premenopausal and postmenopausal controls and 18 age-matched patients with DM); platelet MLC-P and cyclic guanosine monophosphate ([cGMP] reflecting NO) were assessed. Other parameters including age, body mass index (BMI), waist to hip ratio, total cholesterol, and platelet count were not different in the control and diabetic groups. In the premenopausal women, baseline MLC-P was lower in women with DM versus the control group (P = .02). GMP levels were similar in the two groups at baseline (22.7 +/- 3 fmol/mL in controls v 23.1 +/- 3 fmol/mL in diabetic subjects) and 3 minutes after insulin exposure. The platelet content of ascorbic acid (AA), an endogenous antioxidant compound, was elevated in premenopausal women with DM (P = .02) compared with the controls. Despite similar estradiol (beta,E2) levels, platelets of premenopausal women with DM exhibited reduced MLC-P. This paradoxic difference may be accounted for by an increase in platelet AA, as this suggests decreased platelet oxidative stress in this patient population. These observations indicate that an altered redox state and associated MLC-P of platelets does not contribute to enhanced platelet aggregation and CHD in premenopausal women with DM.

[Use of VR (virtual reality) software for preoperative implantation fitting with an implantable hearing aid as an example].

PURPOSE: To prove the feasibility of a preoperative fitting test for an implantable hearing aid using a VR environment. METHODS: A high-resolution spiral CT was performed after mastoidectomy in 10 temporal bone specimens. The bony structures were segmented and merged with the Computer-Aided Design (CAD) data of the hearing aid in a VR environment. For each specimen a three-dimensional fitting test was carried out by three examiners determining the implantability of the hearing aid. The implantation simulation was compared with the real implantation procedure performed by an experienced ENT surgeon. RESULTS: The used VR system enabled real-time 3D-visualisation and manipulation of CT- and CAD-data. All objects could be independently moved in all three dimensions. The VR fitting test corresponded closely with the real implantation. The implantability of the hearing aid was properly predicted by all three examiners. CONCLUSION: Merging CT and CAD data in a virtual reality environment bears high potential for the pre-surgical determination of the fit and mountability of medical implants in complex anatomical regions.

[Analysis of artefacts by virtual endoscopy visualization of spiral CT data].

PURPOSE: Analysis of patterns and causes of artifacts found in endographic visualization of spiral CT data. MATERIALS AND METHODS: A straight perspex tube with a diameter of 20 mm was scanned in three different positions with ten scan protocols of varying table feed, increment, kernel, and signal to noise ratio (Tomoscan AVE, Philips). The resulting 30 CT datasets were visualized as a virtual endoscopy (VE) with ten different visualization protocols (Easy Vision 4.2, Philips) of varying threshold, resolution, and perspective angle. 300 VE datasets were analyzed by two radiologists and compared with the visualization of a software-generated tube in order to differentiate scanning and software artifacts. RESULTS: Five different classes of artifacts have been identified. Two of them result from the scanning process and two from the specific visualization method. Spiral patterns and the unevenness of the tube wall vary with the scanning parameter. Moiré-like patterns are caused by the VE software and depend on the visualization matrix. A high perspective angle distorts the size and form of the tube and makes it difficult for the observer to locate his position within the tube. The appearance of pseudoforamina depends on both the scanning and the visualization parameters. CONCLUSION: The knowledge of the patterns and potential causes of artifacts in endographic visualization of spiral CT scans are the basis for interpretation and optimization of this visualization method.

[Hybrid 3D visualization and virtual endoscopy in cochlear implants].

PURPOSE: The aim of this study was to demonstrate on a complex anatomical structure the possibilities and the advantages of a superimposition of a color-coded surface and volume rendering (hybrid rendering) method with the possibility of the performance of a virtual endoscopy. MATERIAL AND METHOD: In 6 patients with cochlear implants a high-resolution spiral computed tomography of the petrous bone was performed. The cochlear implants, the middle and inner ear structures were visualized using a color-coded surface rendering method, either shaded or as a grid. The petrous bone was visualized using a transparent volume rendering method. RESULTS: The hybrid 3D visualization uses the advantages of both the color-coded 3D surface and volume rendering method. In comparison to the axial source images, the hybrid 3D visualization thus facilitates a clearer representation and better assessment of the complex topographical relationship without loss of diagnostic information. The virtual endoscopy facilitates an intraluminal visualization and inspection of all color-coded 3D surface- and volume rendered structures. CONCLUSIONS: The hybrid rendering and virtual endoscopy make the morphological assessment of cochlear implants easier by the simultaneous visualization of the surrounding structures and thereby support the diagnostic imaging methods. This image processing method can be used pre-operatively for the individual planning, simulation, training and further development of surgical procedures and interventions and post-operatively for the control of the position and further developments of implants.

[Multislice spiral CT of the paranasal sinuses: first experiences using various parameters of radiation dosage].

PURPOSE: To evaluate the potential of multislice CT in the diagnosis of the paranasal sinuses. METHODS: Dose measurements were performed exposing an Alderson Rando phantom in a four-slice spiral CT (MS-CT) while applying a variety of scan parameters. Additionally, 30 consecutive patients underwent a transversal examination by the MS-CT using 1/1/3.5/0.5 mm spiral parameters and an additional transversal or coronal scan on a conventional single slice spiral CT (SS-CT) with 2/3/1 mm. Coronal reformations of the MS-CT were compared with the primary coronal SS-CT, or coronal reformations of the transversal SS-CT, respectively, with regard to image quality and depiction of relevant anatomical details of the region. RESULTS: Superficial exposure values at the level of the eye lenses as well as for the thyroid gland were superior for MS-CT (3.62 mGy, and 0.12 mGy, resp.) as compared to SS-CT (2.96 mGy, and 0.07 mGy). Image quality was equal or superior for MS-CT as compared to SS-CT in all but one case. Drawbacks of SS-CT, such as dental amalgam artifacts, stair step artifacts or partial volume artifacts did not notably affect the coronal reformations of MS-CT. CONCLUSION: MS-CT seems to have the potential to replace primary coronal CT of the paranasal sinuses without any loss of image quality, but may even improve the overall diagnostic value. Radiation doses may still have to be reduced.

[Development of a clinical-radiological CD-ROM database for focal lesions of the liver].

PURPOSE: The development of a CD-ROM database for continuous acquisition and archiving of interesting radiologic cases is described. As a pilot study radiologic images of focal lesions of the liver were collected. METHODS: The database runs on a PC (80486 DX2, 66 MHz, 16 MB RAM), which is connected to the clinic network, a high quality x-ray scanner and a CD-ROM writer. Radiologic images can be inserted into the database either by scanner or by image transfer via network. The database is designed using Access 2.0. Well documented cases were chosen retrospectively, manipulated and archived. RESULTS: 308 images of 12 focal lesions with a capacity of 802 MB were stored on CD-ROM. Per case a storage capacity of 40-75 MB is necessary for image raw material, 15-25 MB are necessary for the postprocessed data. Spatial resolution is 190 dpi (3000 pixel/40 cm). Clinical data, radiological diagnosis coded by modified ACR index and a compressed icon of the images are stored on PC, the image data are stored on CD-ROM. 2.5 hours per case are needed for the complete acquisition, manipulation and archiving procedure, but improved hardware configuration can reduce the time significantly. CONCLUSION: The described database allows continuous archiving of interesting radiologic cases on PC. ACR index is suited for a quick search of a specific combination of radiologic images and diagnosis from different modalities.

Improved maintenance of platelet in vivo viability during storage when using a synthetic medium with inhibitors.

We report on the storage of platelet concentrates and the effects on in vivo and in vitro platelet function tests of adding the platelet inhibitors prostaglandin E1 and theophylline to a plasma-free synthetic medium and storage in containers with reduced surface-to-volume ratio. Paired in vivo studies on platelets labeled with indium 111 after 14-day storage demonstrated higher recoveries (mean +/- SD) of 23% +/- 9% and longer survivals of 109 +/- 59 hours for the test group versus 8% +/- 10% and 19 +/- 22 hours, respectively (p less than 0.01), for the control group (synthetic medium with no inhibitors and use of standard containers). The improved viability was associated with a significantly lower glycolytic rate; better maintenance of other in vitro parameters including respiratory activity, adenosine triphosphate levels, hypotonic shock response, surface glycoprotein Ib (by flow cytometry); and improved preservation of morphologic integrity (p less than 0.05). In particular, a strong inverse correlation (r = -0.91) was observed between in vivo recoveries and the fraction of platelets negative for glycoprotein Ib. We conclude that avoidance of platelet activation by a combination of inhibitors, plasma removal, and reduced container surface improves platelet respiratory activity, adenosine triphosphate and glycoprotein Ib levels, and posttransfusion viability of platelet concentrates stored for 14 days.

Atropine methylbromide and glycopyrrolate. A comparative study during reversal of neuromuscular block.

Two quaternary anticholinergics, atropine methylbromide (methylatropine bromide, MAB) and glycopyrrolate (ROBINUL) were compared as adjuncts to neostigmine for the reversal of residual nondepolarising neuromuscular block. MAB 0.75 mg in combination with neostigmine 2 mg produced a marked initial rise in heart rate. This was significantly greater than that produced by the administration of glycopyrrolate 0.4 mg with neostigmine. The antisialogogue effects of the two anticholinergics were identical and the central nervous system status of the patients was similar. It is concluded that, with the doses used in this study, glycopyrrolate is a superior alternative to MAB and is the drug of choice if a quaternary ammonium anticholinergic is required.

[Lymphography in childhood (author's transl)].

Fifty-one lymphograms were performed in children; the indications, technique and results are discussed. Indications are the search for retroperitoneal involvement in lymphogranulomatosis, lympho-sarcoma and reticulum cell sarcoma; in the search for metastases from malignant tumours, particularly abdominal neuroblastoma, soft tissue sarcomas of the abdomen and lower extremities, testicular tumours and malignant melanomas and finally, for primary lymph-oedema and lymphangiomas. Technique is the same as for adults, but requires particular manual dexterity. Children under six years require general anaesthesia. Amongst 28 children with malignant lymphomas, pathological changes in the retroperitoneal lymph nodes were found in seven. In six, this resulted in a change of the staging. Five out of 16 lymphograms in children with malignant tumours showed evidence of lymph node metastases. All six lymphangiograms in children with lymphoedema and lymphangiomas were abnormal.

Computer-aided surgical planning for implantation of hearing aids based on CT data in a VR environment.

A study was undertaken to assess the feasibility of a preoperative fitting test for an implantable hearing aid in a virtual reality (VR) environment. High-resolution spiral computed tomography (CT) of the mastoid bone was performed, and the results of a mastoidectomy were simulated with manual segmentation on a standard medical workstation. CT was also performed on a temporal bone specimen obtained at real mastoidectomy, and the bone margins were segmented automatically with threshold-based techniques. A triangulated surface representation of the bone structures including the mastoid cavity was generated. These data as well as the computer-aided design (CAD) files of the medical devices were transferred into a VR environment. The CAD components of the hearing aid were manipulated to simulate the surgical implantation procedure. Merging CAD data of an implantable hearing aid with CT data of the temporal bone in a VR environment was shown to be a feasible method of providing three-dimensional information for the presurgical determination of fit and mountability. Advances in hardware and software are expected to improve the usability of this method. Although clinical studies are needed, these results may serve as an impetus for exploring the use of low-cost, widely available VR computer equipment in a potentially broad field of clinical applications.
