Hypergastrinemia, gastric endocrine cell hyperplasia, and intractable diarrhea.

A patient with pernicious anemia developed severe intractable diarrhea night and day. Investigation revealed chronic atrophic gastritis and a markedly elevated level of serum gastrin. No obvious explantation for the diarrhea was found, but after antrectomy, the gastrin level returned to normal and the diarrhea subsided. Possible mechanisms for an association between diarrhea and hypergastrinemia include colonic hypermotility secondary to release of acetylcholine and inhibition of fluid and electrolyte reabsorption within the small bowel.

Use of secretin in the roentgenologic and biochemical diagnosis of duodenal gastrinoma.

The use of secretin in the biochemical and roentgenologic diagnoses of a duodenal gastrinoma has been described. Preoperatively, the secretin test indicated that a gastrinoma and not a retained antrum was the cause of hypergastrinemia in a patient who had previously undergone Billroth II gastrectomy. Intravenous infusion of secretin during selective angiography resulted in greatly enhanced visualization of the tumor which allowed it to be localized to the duodenal stump. Several months postoperatively, the secretin test result had become negative, which presumably suggested that the tumor had been excised completely. Our experience has revealed that intravenous secretin might improve the diagnostic usefulness of selective angiography.

The butterfat absorption test in adults.

The usefulness of the butterfat absorption test in assessing fat absorption has been studied in 89 adult patients. In 42 patients who were shown to have steatorrhea by the criteria of the fat balance studies, 33 patients (78.6%) had abnormally low serum optical density values following butterfat ingestion. In 8 patients, steatorrhea could not be clearly explained on the basis of their underlying disease. If these 8 patients are excluded from consideration, then 28 of 34 patients (82.4%) had an abnormal response following butterfat ingestion. A maximum increase of optical density of 0.25 or less is a useful indication of fat malabsorption in adult patients.

Fecal chymotrypsin and trypsin determinations.

Trypsin and chymotrypsin concentrations were determined in 180 spot stool specimens from 110 control patients in hospital. The lower limit of normality for each enzyme was placed at the 5% level: 95% of this population excreted feces containing more than 100 mug. of chymotrypsin and 30 mug. of trypsin per g. of feces. Chymotrypsin concentrations appeared to be a more reliable guide to pancreatic function than trypsin concentrations.Fecal chymotrypsin concentrations were subnormal in five patients with chronic pancreatitis, borderline in one patient with relapsing pancreatitis, subnormal in one patient after pancreatectomy, and subnormal in five of nine with carcinoma of the pancreas. Subnormal concentrations of fecal chymotrypsin were found in seven of 21 patients with chronic liver disease related to alcoholism, eight of 32 with a partial gastrectomy, three of 10 with adult celiac disease and five of 16 with psoriasis. It appears that the determination of fecal chymotrypsin concentrations provides a valuable screening test for pancreatic exocrine deficiency. However, normal results may be found in some patients with pancreatic disease and subnormal values may occur in some patients with other conditions.

Microscopic examination of stools after partial gastrectomy.

The present study was undertaken to determine the frequency with which undigested meat fibres in the stool could be found after partial gastrectomy. A sample of stool was examined in 61 patients who had undergone a partial gastrectomy and in 92 control patients. The age ranges and average ages of both groups were similar. The stools of 77 of the 92 control patients contained well-digested meat fibres, 14 showed partially digested fibres, and one patient had completely undigested meat fibres. In the postgastrectomy group, 44 of 61 patients had well-digested meat fibres and 17 showed partially digested fibres in their stools. None showed undigested fibres. The present study demonstrated that the finding of undigested meat fibres is as infrequent in postgastrectomy patients as in control patients. If undigested meat fibres are found in the stools of postgastrectomy patients, it is suggestive of pancreatic exocrine insufficiency.

Metoclopramide for gastroparesis diabeticorum.

Three patients with uncontrolled diabetes mellitus and with symptoms due to gastric retention proved to have gastroparesis diabeticorum. The diagnosis was established by the beefsteak-barium meal. After the administration of metoclopramide, gastric emptying improved and the diabetes was easily controlled. Gastroparesis diabeticorum is one cause of uncontrolled diabetes mellitus and may be helped by the administration of metoclopramide.
