Outpatient surgical treatment of lip carcinomas: immediate and long-term results.

From 1977 to 1987, outpatient surgery was performed under local anesthesia on 131 patients with squamous cell carcinoma of the lip without regional lymph nodes at the Day Hospital of the Istituto Nazionale Tumori (INT) Milan. Radical surgery was achieved in 117 cases (89.3%), independently of the size of the neoplasm. Of the 7 patients in whom radical surgery was not achieved at first operation, 3 were reoperated on as outpatients, 2 were hospitalized and reoperated, and 2 underwent X-ray treatment. Finally, in the remaining 7 patients radical surgery was not obtained or was near the borderline of the neoplasm. Complications (diastasis and/or suppuration) arose in 4.6% of the cases. During the follow-up (mean duration, 30 months) 7 patients developed a relapse and 2 manifested regional metastasis. Twenty-three patients developed a second tumor (13 on the lip, 3 in the lung, and 7 in other areas). These results prove that day hospital surgery of T1-T2 lip squamous carcinoma is feasible. It is appreciated by the patient, requires very little postoperative care, is less expensive than hospitalization, and involves less psychologic stress for the patient.

Analysis of a soluble lipid-protein complex carrying endogenous 11-cis retinaldehyde from bovine retinal pigment epithelium.

A soluble lipid-protein complex in bovine retinal pigment epithelium is shown to carry endogenous 11-cis retinaldehyde, in the extent of 15% of the total 11-cis retinaldehyde found in this tissue. The complex, analyzed with respect to its chemical composition, exhibits a lipid composition close resembling the lipid composition of the rod outer segment membrane; the SDS-PAGE evidences the presence of a number of protein bands, two of which of 34 and 27 kDa appear glycoproteins. Finally, the lipid-protein complex exhibits a discrete level of a Cathepsin D-like protease activity. From the above, the possibility is discussed that the soluble lipid-protein complex could represent some phagolysosomal inclusion occurring in the pigment epithelial cells upon rod outer segment phagocytosis.

Fine needle aspiration cytology in superficial lymph nodes: an analysis of 266 cases.

The results of fine needle aspiration cytology of superficial lymph nodes in 275 patients are discussed. Nine cases were lost to follow-up. Of the remaining 266 aspirates, 152 were classified as cytologically malignant, 79 as benign, three as suspicious for malignancy, and 32 as unsatisfactory owing to scanty cellularity. The sensitivity of cytology for metastatic cancer was 96.5%. The results of aspiration biopsy from lymphomas were less accurate (67.5% sensitivity). There were no false positives and nine (11.3%) false negatives. Open biopsy of three suspect lymph nodes proved them to be malignant. The aspiration procedure is easy, safe and reliable. The diagnosis of benignity cannot, however, be determined without an open biopsy, particularly for lymphomas.

Modulation by docosahexaenoic acid of the epinephrine-stimulated adenylate cyclase activity of the bovine retina.

This work shows that unsaturated fatty acids enhance the epinephrine-stimulated adenylate cyclase activity in bovine retina. The modulating effect on the epinephrine-stimulated formation of cyclic AMP seems to be linked to the degree of unsaturation of the fatty acid. Treatment of the intact retina with docosahexaenoic acid in the concentration range 0.5 X 10(-6)-1 X 10(-3) M does not affect the enzyme activity measured in the absence of the hormone but markedly increases the cyclase activity when the tissue is incubated in the presence of 0.1 mM epinephrine. Docosahexaenoic acid enhances the maximal response to epinephrine without affecting the apparent ED50 value for this effector. Docosahexaenoic acid at 0.5 mM also increases the hormone-stimulated adenylate cyclase activity in retinal cell-free homogenate, whereas it has no effect on the epinephrine-sensitive enzyme solubilized from the membrane fraction with 1% Triton X-305. When docosahexaenoic acid-preincubated intact retina and cell-free homogenate are incubated in the presence of defatted albumin, both the observed activating effect of the fatty acid on the epinephrine-stimulated adenylate cyclase activity and the enhancement of the enzyme response to the hormone significantly diminish.

Intravesical chemoresection with 4'-epi-doxorubicin in patients with superficial bladder tumors.

The authors report the preliminary data concerning a multicentric study on ablative effects of 4'-epi-doxorubicin (EPR) on superficial bladder cancer. Forty-six patients affected by multiple transitional cell superficial bladder carcinoma were treated by transurethral resection of all but one tumor, left as a marker lesion. Subsequently, an intensive local treatment with EPR and 3-month control cystoscopies with biopsies were performed. In the 47% of cases the control cystoscopy and pathology assessed the complete disappearance of marker lesions without any new occurrence whereas it is interesting to observe that the breakdown of the complete remissions into primary (67%) versus recurrent (37%) and in intravesical unpretreated (55%) versus pretreated (31%) showed important differences. It may be concluded that EPR shows a real activity against transitional cell superficial bladder carcinoma.

Binding of 11-cis retinaldehyde to the partially purified cellular retinaldehyde binding protein from bovine retinal pigment epithelium.

11-cis retinaldehyde binding analysis was performed on a bovine retinal pigment epithelium preparation of cellular retinaldehyde binding protein (CRALBP), whose purity degree was estimated as 75%. Equilibrium binding studies were carried out measuring the replacement of tritium-labeled with unlabeled 11-cis retinaldehyde at 25 degrees C. Analysis of the experimental data both by a direct curve-fitting procedure utilizing a non linear least square regression analysis and by a conventional Scatchard plot revealed a single non-interacting binding site with an apparent equilibrium constant of 0.9 X 10(-7) M. A binding stoichiometry of approximately 1 mol of 11-cis retinaldehyde/mol of binding protein can be calculated from the experimental data. Competition studies carried out in the presence of unlabeled 'trans' and 'cis' isomers of vitamin A derivatives confirm the high degree of specificity of the 11-cis retinaldehyde binding.

Red cell peroxide metabolism in diabetes mellitus.

In a group of normal controls and in a group of diabetics subdivided for type we evaluated the following red blood cell parameters: superoxide dismutase (SOD), glutathione peroxidase (GSH.Px), catalase (C-ase), glutathione content (GSH) and membrane-protein-sulphydryl groups (P-SH). There was no difference in the enzymatic activities of superoxide dismutase, glutathione peroxidase and catalase in normals and in the diabetics. However, the erythrocyte GSH content as well as membrane P-SH groups discriminate the normals from diabetics and also the diabetics subdivided for type. None of these parameters was related to the erythrocyte filterability considered as a reflection of the red blood cell deformability. These findings reveal that the diabetic red blood cell is less protected from oxidant agents.

Effects of bendazac L-lysine salt on X-ray-induced cataract in the rabbit lens.

The effects of bendazac-L-lysine salt (bendazac-lysine) on some biochemical parameters (soluble and insoluble proteins, reduced glutathione, sulphydryl and disulphide groups, water content) in rabbit lens at different times after X-rays (2000 rads) were studied. The mature cataract (swelling and total lens opacity) developed 11-12 weeks after irradiation. In the mature cataract, the irradiated lenses not treated with bendazac-lysine (ILNTB) show a 32% increase in water content compared with controls; this increase is 12% in irradiated lens treated with bendazac-lysine (ILTB). Twelve weeks after irradiation the concentration of insoluble proteins in the controls, ILNTB and ILTB is 7.6%, 52.3% and 18.3% respectively. After 6, 8 and 12 weeks the concentration of reduced glutathione in ILNTB decreases by 23%, 81% and 92% as compared with the controls. In the ILTB the decrease is present only 8 and 12 weeks after X-irradiation and is of 55% and 69%, respectively. The sulphydryl-group content in the soluble proteins in ILNTB compared with the controls decreases by 26%, 38% and 47% after 6, 8 and 12 weeks, while in the ILTB a decrease is observed only after 8 and 12 weeks and is 6% and 12%, respectively. The decrease of the sulphydryl groups parallels the increase of the disulphide groups. This increase is already significant (P less than 0.01) after 6 weeks in the ILNTB, whereas it becomes significant in the ILTB only after 8 weeks.(ABSTRACT TRUNCATED AT 250 WORDS)

Growth rate of primary breast cancer and prognosis: observations on a 3- to 7-year follow-up in 180 breast cancers.

The disease-free probabilities after 3 to 7 years of follow-up of 180 breast cancers of known doubling times were studied to assess the prognostic significance and clinical implications of the growth characteristics of primary breast cancer. Fast-growing tumours, N+ greater than 3, showed a prognosis significantly worse (P less than 0.01) than that of slow-growing tumours of the same class; no significant differences were found among N- or N+ (1-3) fast-, intermediate- and slow-growing tumours. Highly significant differences were found among fast- and intermediate-growing tumours with different degrees of lymph node involvement (respectively P less than 0.0001 and P less than 0.001), with the worst prognosis for N+ greater than 3 tumours. In contrast, no significant differences were found among slow-growing tumours of the different N classes. When the Cox model was applied, the relationship between lymph node involvement and doubling time was significant, as was the interaction term. It is suggested that growth rate and metastatic potential are not the same in primary breast cancers, and their relation should be investigated.
