5-Aminosalicylic acid suppositories in the maintenance of remission in idiopathic proctitis or proctosigmoiditis: a double-blind placebo-controlled clinical trial.

Thirty patients with distal ulcerative colitis in remission (17 proctitis, 13 proctosigmoiditis) were randomly given either 5-aminosalicylic acid (5-ASA) or placebo suppositories, 400 mg bid. During the 1-yr follow-up, patients were assessed clinically every month, and flexible sigmoidoscopy with a rectal pinch biopsy specimen and laboratory data were carried out every 3 months. Two patients in the 5-ASA group chose to withdraw from the study, one relapsed, and 12 remained in remission. In the placebo group, one patient chose to withdraw, 11 relapsed, and three remained in remission. The cumulative remission rate at the 12th month was 92% in the 5-ASA group and 21% in the placebo group. Log rank test showed a significant difference in the relapse rate between the two groups (chi 2 = 14.26, p less than 0.001). No side effects were observed. We conclude that 5-ASA in suppository form (800 mg/day), administered for 1 yr, is safe and effective in maintaining remission of distal ulcerative colitis.

[Oral hairy leukoplakia].

We report our experience of oral hairy leukoplakia in HIV seropositive patients. Etiopathogenesis and relationship between oral hairy leukoplakia and HIV infection are briefly discussed, as well as its prognostic value.

[Idiopathic muscular hypertrophy of the esophagus. A report of a symptomatic case].

A case of idiopathic muscular hypertrophy of the esophagus in a 55 years old woman with a two months history of dysphagia is presented. Radiological, esophagoscopic and CT scanning appearance was of an infiltrating process, but intraoperative histological examination revealed the benign feature of the lesion. Thus adequate surgical treatment was performed.

Carcinoma of the urachus. Report of a case.

The authors report a case of urachal carcinoma of a 60 year old woman of particular interest because of the clinical history, biological behavior and histotype. The tumor presented as an umbilical mass of 25 cm which had slowly increased during 20 years without evident clinical signs or infiltration of the peritoneum and abdominal organs. Histologic findings revealed an adenosquamous carcinoma, which represents less than 5% of urachal neoplasms. Surgical excision is the treatment. The prognosis is poor because of regional recurrence and metastases. The authors review the literature analyzing the criteria for differential diagnosis between adenocarcinoma of vesical and of urachal origin.

Adenocarcinoma↗

Fibromatosis of the larynx in the adult.

We describe the fourth case of aggressive fibromatosis of the larynx arising in an adult. Most cases of laryngeal fibromatosis have been described in children. In the larynx it may behave the same as in other sites, with local aggressiveness but without metastasis. The possible differential diagnoses are discussed.

Verruciform xanthoma of the vulva: case report.

A rare case of verruciform xanthoma of the vulva is reported. Diagnosis was made possible by histopathological examination and immunohistochemical staining. Verruciform xanthomas generally occur in the oral cavity. To the best of our knowledge, this is the third reported case of the tumour located on the genital mucosa. Immunohistochemical study supported the histiocytic origin of the lesion. Clinically, verruciform xanthomas may mimic other verrucous lesions of the vulva, such as seborrhoeic keratosis, verruca simplex, condyloma acuminatum, verrucous carcinoma, or erythropasia of Queyrat, or conditions such as histiocytosis, cutaneous lipidosis, or granular cell myoblastoma. The characteristics that differentiate those conditions from verruciform xanthoma, which can be seen only on histology, are given.

Neurothekeoma of the thumb. A case report.

A benign tumor of nerve sheath origin occurring in the dermis of a thumb is described. Histologically, these lesions contained nests and cords of large cells surrounded by thin bands of collagen fibers scattered in a myxoid background. The immunocytochemical reaction for S-100 protein was positive.

Granular cell basal cell carcinoma of the skin. Report of a case with immunocytochemical positivity for lysozyme.

Granular cell tumors have been described in many sites throughout the body. To our knowledge, few cases have been described in the literature. We report here the sixth case, in which positivity for lysozyme was also observed for the first time. The problems of the histogenesis and differential diagnosis of this rare variant of basal cell carcinoma are also discussed.

Epithelioid hemangioendothelioma of the larynx.

Epithelioid hemangioendothelioma is an uncommon vascular tumor of the soft tissues. Several reports have described this tumor in visceral locations such as liver, lung, and brain. To the best of our knowledge this is the first report of an immunohistochemical and ultrastructural study of a primary epithelioid hemangioendothelioma occurring in the larynx. Difficulties concerning the differential diagnosis and clinical problems arising from surgical therapy in the laryngeal area are also discussed.

Blue nevus of the uterine cervix.

A case of blue naevus of the endocervix is reported with immunohistochemical and ultrastructural findings. The dendritic melanin-containing cells are analogous to the blue naevi cells of the skin. Electron microscopy showed melanosomes in dendritic cells but no cells with melanin-producing Schwann cells features were observed. Histogenetic hypotheses are discussed.

[Fibroma of the tendon sheath. Description of a case].

One case of fibroma of tendon sheath is described. This tumor is a distinct entity with a characteristic location and histological appearance. The lesion must be distinguished from giant cell tumor of tendon sheath, nodular fasciitis and fibrous histiocytoma. Recurrence rate is 24%. Local excision is the treatment of choice for primary and recurrent lesions.

Extraskeletal myxoid chondrosarcoma with intranuclear vacuoles and microtubular aggregates in the rough endoplasmic reticulum. Report of a case with fine needle aspiration and electron microscopy.

The findings of fine needle aspiration biopsy cytology, histology, immunohistochemistry and electron microscopy in an extraskeletal myxoid chondrosarcoma of the soft tissue of the thigh detected in a 65-year-old man are described. In addition to the more usual cytologic findings of chondroblasts and pleomorphic mesenchymal cells, this tumor displayed distinctive intranuclear cytoplasmic inclusions and microtubular aggregates in the rough endoplasmic reticulum. Cytologic study made the diagnosis of malignancy, suggesting a myxoid sarcoma; the precise diagnosis was made by the subsequent studies on the resected tumor and resin-embedded samples of the aspirate. The suitability of aspirated material in the differential diagnosis of myxoid soft tissue tumors is discussed.

Histiocytosis X of the thyroid gland.

Histiocytosis X rarely involves the thyroid gland and occurs as one of the unusual manifestations of systemic diseases. Diagnosis is based on the identification of the Langerhans' cells in an appropriate background. We report a case of histiocytosis X confined to the thyroid gland that represents the primary manifestation of the disease. A careful histological, immunohistochemical and ultrastructural study has been performed and problems of differential diagnosis are discussed.
