[Italian Cystic Fibrosis Registry: 10 years of activity].

Cystic Fibrosis (CF) is a recessive autosomic genetic disease with an incidence in mediterranean countries of about 1:3500 born alive. In Italy the considerable genetic variability makes it difficult to identify all the homozygous subjects and, consequently, to estimate the incidence of the disease in healthy carriers. The disease is evolutive and affects various systems, most of all the respiratory and gastrointestinal systems. Not many years ago, when the clinical definition of CF was first introduced, average survival did not exceed the pediatric age. Nowadays with ever advancing diagnostic and therapeutical techniques many CF patients survive until an adult age. It is therefore necessary to plan adequate health service interventions so as to satisfy as much as possible the needs of both the patients and their families. To this end data collected since 1.1.1988 by the Italian registry for CF (year of birth, sex, region of birth and residence, diagnosis procedures, results of sweat test, pancreatic insufficiency, DNA analysis, status: alive, dead, lost to follow up) of all the patients, diagnosed in the 18 Reference Centres and the 3 local Centres for CF, have proved to be extremely useful. Since the birth of the Registry on 31.12.1997, data relating to 2458 patients alive on 1.1.1988 and 1159 born during the last ten years, for a total of 3617 subjects (1756 females and 1861 males), have been recorded. As already mentioned a considerable increase in life expectancy of CF patients (from 1988 to 1990 the average age of death was 14 years, from 1994 to 1997 it was 19) and a consequent increase in the percentage of adult patients have been observed.

Minimal requirements in prostate cancer irradiation: a consensus document by the AIRO Lombardia Cooperative Group.

PURPOSE: With the aim of establishing clinical and technical criteria to homogenize radiotherapy practice, a working group of AIRO-Lombardia (Associazione Italiana di Radioterapia Oncologica--Gruppo regionale della Lombardia) has tried to define minimal requirements for radical and postoperative irradiation in prostate cancer. The document has been structured in such a way as to be also of interest to the urological and medical oncology communities. METHODS: The working group, composed of representatives of most of the regional radiotherapy departments in the Lombardy region, had monthly meetings during 1996 and 1997. The document on minimal requirements has been derived from the participants' combined experience and knowledge, from review of the literature, and from a 1995 regional survey on current practice of prostate irradiation. RESULTS: Minimal requirements for radical and postoperative irradiation of prostate cancer have been defined with respect to treatment strategies, pre-treatment diagnostic evaluation and staging, treatment prescription, preparation and execution, and quality assurance procedures. CONCLUSION: Standards of reference for minimal requirements in prostate cancer irradiation adapted to the regional structures and resources have been defined.

[Fact-finding investigation of the social health conditions of the elderly].

PURPOSE: The intent of the present contribution is that to verify the social health conditions of the elderly present on national territory. MATERIALS AND METHODS: The Authors have thought up an anonymous questionnaire, made up of 63 questions, concerning the life-style, the social conditions and health of the elderly over 65 years old, and consequently by now on pension. Two thousand questionnaires have been distributed to medical officers, present in north as well as in the centre and in the south of the Country; of these we have then received 1386 questionnaires for statistical processing. RESULTS: The results have been about the same for both sexes as well as for geographical distribution. The greatest part of the elderly are self-sufficient from an economical point of view, in spite of a certain economical dissatisfaction present among the males in the north. The women appear to be more active, inclined to travel. The voluptuary habits, of which smoking and drinking are almost absent in all the national territory and in both sexes. As far as prevention in concerned, consequently submitting oneself to medical check-ups, is more accepted in the south by both males and females and in the north and centre only by women. CONCLUSIONS: Our fact-finding study has given good results, and a good conformity in the portion of people interviewed. There could be a greater impulse given to the prevention of pathologies, perhaps with the assistance of free prevention campaigns revolted toward the elderly, and still dedicating more time to our "grandparents" that must not be considered a burden, but rather as a resource.

[Hospices from the DPCM 20/1/2000 perspective: requirements to ensure a better quality of "residual life"].

INTRODUCTION: In this paper, the authors analyse the increasing number of oncological patients, above all the terminal ones. The aim of hospice is to guarantee a high quality residual life. Hospice must not be regarded as a place of sweet death; on the contrary it's a hospital where palliative cares take place. It is, then, an alternative to home in the most critical phases of illness. CONSIDERATIONS: Despite a lasting interest in the matter, a law setting up a standard has been lacked until DPCM of 20/1/2000 that established structural, technological and managerial requirements for the centres of palliative cares. New hospices must be built in sunny places, in close contact with nature and easily reachable both by private and public means of transport. The law provides for three areas: a residential area, an evaluation and therapy area and a general area of support.

[Evidence-based medicine: accurate review of the international literature].

The Authors emphasize the continual scientific technological advances of medical science, the appearance of new nosological extents and the increased health request by people. Physicians are called to up-to-date all the time to keep a high-level of professional competence also after the end of the university studies. Every medical staff can use EBM, setting aside all personal specialization and also with the limitations estimated in our research.

Rabies infection: epidemiology and prophylaxis.

Authors of this paper underline the importance of worldwide epidemiological rabies infection and the prevention. Specifically, a program of health education first aid md's, family doctors, veteriners, animal's traders, and to students of ground schools (primary and secondary) in a large medical regional area of Rome was developed. This article is informative to whom can concern rabies infection risks in Italy in order to reduce the abuse of vaccination influencing on behaviors and on the relation humans-animals.

[The flu syndrome: update and therapeutic and prophylactic approach].

Authors in the present work analyse the history of the flu from V century A.C. until our days. We can know the different flu epidemic the structure of the virus and to take note of the different preventing device; of these the core is the vaccine. Thanks the vaccine is possible to weaken the big epidemics. Authors attract the importance on the mass vaccine and conclude to mention the new antivirus drugs per os.

[Clinical investigation of hyaluronidase and nerve growth factor (NGF) at the ocular level: detailed review of the international literature].

In this paper the authors analyse the international bibliography about clinic tests on hyaluronic acid and Nerve Growth Factor at ocular level. This study was performed on request of a private oculistic clinic in the centre of Rome about the clinic test of the drugs mentioned above. Therefore, this paper is a joint work of Public Health physicians and oculists.

[Technology assessment and operating procedures for the technological resources in the new Community Health Service].

INTRODUCTION: The main purpose of this contribution is to set today's situation of technology assessment and point to some organizing and integrative ways with the evolution concern ASL (Local Health Firm). PATIENTS AND METHODS: We outline main critical processes that fix correct management of technological resource thought data available in national and international literature. We single out more common methods to quantify, evaluate, control and manage biomedical technologies. RESULTS: We expound decisional process that lead Viterbo ASL (Local Health Firm) to a progressive controll of tecnological resource and its management after aggregation of the five USL (Local Health Units) that constituted Viterbo ASL and describe procedures assumed. CONCLUSIONS: We underline the importance of full and complete information in business and decisional process. Inside the complexity of italian view, we outline an organizative solution with a low economic impact and with a high technical content.

[Preventive and therapeutic approach to tuberculosis in the general, and high-risk populations].

Authors in the present contribute illustrate the tuberculosis disease in all aspects, the infection, the disease in active phase and in the resistant phase, the law, the prevention and the professional risk in exposed subject, and to consider the epidemiological aspect. Authors hope a coordinate approach between the different figure involved in the therapy and prevention of tuberculosis disease.

[Legislative Decree 25 of 2/2/2002: prevention of chemical agents risks].

The Authors, with the present contribution, illustrate and comment the recent legislation aimed to protect workers from risks due to chemical agents and compare it with the previous legislation. A careful analysis makes it clear the urgent need to realize optimal safety standards for workers in the chemical industry, in order to protect them from avoidable risks.

[Safety and hygiene in activities of craftsmen: a survey in the province of Rome].

The artisan firms, in the past, have paid a poor attention to hygiene's problem and to workers' safety. For this reason the Authors have promoted an investigation in 342 activities of Rome's district to verify the correct observance of some preventionistic indicators such as the evaluation of the risks, the electric plant, the use of ipd, the application of the system of signs, etc. The results of the research have proved a situation that is better in comparison with the past; the Authors analyze the causes and they auspicate that the evolutional process can guarantee the minimum safety's conditions to the all employees in the small firms.

Immunocytological and mineralogical study of bronchoalveolar lavage in a group of subjects exposed to asbestos.

Twenty-five subjects (24 males and 1 female, mean age 57.4 years) who have been exposed to asbestos underwent chest radiography, high resolution computed tomography (HRCT) of the chest, lung function tests and bronchoalveolar lavage (BAL) for evaluation of cell components (total cell count, percentages of macrophages, lymphocytes, neutrophil and eosinophil granulocytes and the lymphocyte subpopulations CD3+, CD4+, CD8+, CD19+ and HLADR+), soluble factors (IL-8, IL-10, IL-12 and MCP-1 in the supernatant) and concentration of asbestos fibre. The subjects were subdivided according to the degree of their exposure, to the concentration of asbestos fibres in the BAL and to chest X-ray findings using the I.L.O. classification (0/0pl, 0/1 and 1/0 and above). According to the exposure index, we showed statistically significant (p < 0.05) higher lymphocytes percentage in the BAL of subjects with moderate exposure and significantly higher levels of IL-10 (p < 0.05) in the supernatant of subjects showing an absence of asbestos fibres in their BAL. In the group of subjects with a 0/0 and 0/1 radiological profile, the cellular component of the BAL was characterised by a higher percentage of lymphocytes (p < 0.02), whereas a trend toward an increase in the number of neutrophils was noted in subjects with obvious pulmonary fibrosis. The percentage of neutrophils was inversely correlated with some parameters of respiratory function such as vital capacity (p = 0.03) and the partial pressure of oxygen (p = 0.05) in the blood. Investigating the cytokines in the supernatant of the BAL, we found a trend toward lower concentration of IL-10 in the group showing the worst radiological picture (I.L.O. > or = 1/0), and a statistically significant negative correlation between this cytokine and pO2 (p = 0.048). Concerning the other cytokines and chemokines studied (MCP-1, IL-8 and IL-12), no significant differences were found to be associated with the radiological profiles. There were, however, positive correlations between the concentration of IL-8 and the percentage of neutrophils (p = 0.038) and between the concentration of MCP-1 and the percentage of lymphocytes (p = 0.006). A negative relationship between the concentrations of IL-12 and IL-10 has been also observed (p = 0.028). This research allows us to hypothesise that IL-10 may have a pathogenetic role in the evolution of asbestosis.

[Considerations about health risk management].

From the birth of doctrines of Risk Management to today a lot of time is passed. From the initial application in the field of the insurances and the management of the enterprises, theories inspired to the identification, evaluation and correction of connected risks to the activity and the industrial trial has been figurative to the health, field in which the application of these principles results to be how much never profit and productive of benefits above all for the patients that suffer consequences of errors but also for the physicians and the personnel that, perfectly inserted in an organization aware of the trials to put into effect, can bring their contribution to underline the weak points of the relief trial. The economic cost and consequences of errors can decrease if a new culture is established inspired to the learning and the communication of the adverse events, to minimize the possibility that they again occurs.
